Job Summary
Provides spiritual/pastoral care and spiritual ministries programming for residents, residents family members and employees. Supports the organization's mission and vision by providing spiritual care, including individual pastoral care, for all residents and residents' family members to address resident's spiritual needs, including those with varying levels of dementia. Administers and coordinates the administration of sacraments. Helps ensure appropriate worship and Bible study opportunities are available to all residents. Helps ensure development of and participates in the spiritual life programming. Demonstrates ability to minister to people of diverse faith traditions, displaying respect for cultural diversity. Invites and encourages employees to participate in the spiritual life of the location, and encourages, trains and empowers them in their providing spiritual care. Is available to staff for pastoral care as needed and refers to appropriate resources. Documents visits and care provided to residents. Works collaboratively with others in ensuring the meeting of residents' spiritual needs.
Educates internal and external healthcare groups on spiritual and religious care and responsibilities of chaplains. Networks with community clergy and other faith leaders as needed. Serves as a member of the interdisciplinary team, collaborating with other members to develop and provide spiritual support to residents, patients, clients, families and staff.
Serves to support, teach, and mentor peers and students when appropriate and requested. When requested, provides spiritual leadership at organizational events such as memorial services, dedications and public events. Manages complex and challenging cases by consulting with interdisciplinary team.
Follows the Association of Professional Chaplains Code of Professional Ethics. Addresses the spiritual care referrals and consults in a timely manner. Follows spiritual ministries policies and procedures. Demonstrates critical thinking skills under adverse situations in order to respond appropriately to residents, patients, clients and families. Possesses awareness of personal biases and demonstrates the ability to set them aside while at work. Possesses ability to establish and maintain effective professional relationships. Exhibits initiative with events. Provides leadership qualities when necessary. Participates in staff meetings as scheduled. Depending on location, may be required to serve on call as assigned.
Qualifications
Graduate of an accredited School of Theology with the Association of Theological Schools (seminary or equivalent) with a Master of Divinity or a Master of Theology degree strongly preferred.
Completion of two units of Clinical Pastoral Education or being certified or credentialed as a chaplain, along with being endorsed or affirmed for this ministry by denomination is preferred.
One Unit = 400 hours, 300 clinical ministry, 100 group experience under supervision of certified supervisor of CPE program.
Previous and extensive pastoral experience preferred.
Must be an active member in good standing of a Christian denomination per Good Samaritan Society s "A Description of Being Christian in the Lutheran Tradition.
Required to be ordained, licensed or commissioned to function in a religious vocation by an appropriate religious authority. Must provide written verification of ordination, licensed or commissioned status prior to being hired.

* Provides spiritual care support to the patient and family; supports the involvement of community clergy as the patient/family request.
* Assists in identifying the need for intervention of other IDT members.
* Effectively communicates patient and family needs to IDT.
* Collaborates with IDT to coordinate spiritual care support for the patient and family to ensure appropriateness, continuity, and quality of care.
* Maintains updated, comprehensive care plans which reflect current problems, goals and interventions for patients.

**Education and/or Experience**

Job Summary and Qualifications
Under supervision of Director of Pastoral Care, the PRN On Call chaplain is responsible for working primarily in an on-call capacity as a representative of the Department of Pastoral Care, to provide Spiritual Care and support to patients and families especially in times of crisis. This chaplain is expected to be competent and calm in both emergent and non-emergent situations. This position requires a strong sense of pastoral identity and an ability to minister ecumenically in an interdisciplinary setting, providing pastoral care in terms of the spirituality of the receiver. Chaplain will demonstrate and ability to anticipate problems and works effectively to resolve issues, and communicate effectively both verbally and in writing to patients, physicians, administration, peers and staff. Pastoral care with patients, families and staff requires integrity, the maintenance of confidentiality and the ability to work as a member of the team. Planning, judgment and independent initiative are required to serve responsibly as a chaplain to persons of all ages with appropriate understanding of developmental and situation needs and differing cultural, religious and socio-economic backgrounds. Chaplain assures a high level of customer satisfaction, manages productivity and will participate in other chaplain duties at the request of Director of Pastoral Care, as mutually agreed upon by both parties.
Fluency in more than one language is welcomed.
Work Hours: Must be willing to work on call shifts nights, weekends and holidays in combination with other on call chaplains. On call day shifts are occasionally offered, as well as routine visitation, when requested by Director of Pastoral Care. Must be able to work at least 6 shifts each month, and be able to offer sacramental, spiritual and liturgical services for shifts as well as special events and holidays, as mutually agreed upon between Director of Pastoral Care and chaplain.

* Visits and performs counseling/faith service to participants on spiritual care list, recording these contacts in the InnovAge Electronic Medical Records in a timely manner, as well as for those who have requested such services in collaboration with the Director of Spiritual Services.
* Functions as part of the Palliative care team by providing faith services to participants who are making the transition and their family member(s) and caretaker(s).
* Plans, prepares, conducts and attends religious services as requested or needed.
* Contacts loved ones of deceased participants during their bereavement period as deemed appropriate by the Chaplain or Director of Spiritual Services.
* Responds to emergency calls from participants and family members and tries to assist with available resources.
