RESPONSIBILITIES

General Position Description:
The hospice Spiritual Counselor/Chaplain provides spiritual care in the home to terminally ill patients and their families. Provides an assessment of the patient's and family's spiritual needs and provides counseling to meet those needs. Makes reasonable efforts to facilitate visits by local clergy, pastoral counselors or other individuals who can support the patient's spiritual needs short term and long term. Role Expectations:

The Hospice Chaplain is a professional spiritual care provider and a full member of the healthcare interdisciplinary team who provides for the hospice, pastoral and spiritual needs of patients, their families, and associates. Enhabit Home Health & Hospice is searching for a Chaplain with hospice experience to join our team. The Chaplain is a member of the interdisciplinary team and reports to the Hospice Branch Director.
* Providing direct spiritual support and/or counsel to patients/families in keeping with patients/families' beliefs.
* Working with staff, clergy and community groups to enhance their sensitivity to the spiritual needs of patients/families and reporting on services as indicated.
* Providing consultation and education for patients/families and interdisciplinary team members.
* Providing bereavement follow-up services as defined by Hospice.

General Position Description:
The hospice Spiritual Counselor/Chaplain provides spiritual care in the home to terminally ill patients and their families. Provides an assessment of the patient's and family's spiritual needs and provides counseling to meet those needs. Meets mandatory continuing education requirements of the Hospice.
Demonstrate commitment, professional growth, and competency by maintaining a working knowledge of public and private eligibility standards and requirements.
Promotes the Hospice's philosophy and administrative policies to ensure quality of care.
Demonstrates understanding of coordination and continuity of care by collaborating with the IDG/IDT.

Physical and Mental Effort: Prolonged standing and walking required, with the ability to lift up to 50lbs and move patients. It requires working under some stressful conditions to meet deadlines and patient needs, make quick decisions and resource acquisition, and meet patient/family individualized psychosocial needs. Requires eye-hand coordination and manual dexterity.
Ability to communicate with patients, families, physicians, co-workers, and visitors to exchange accurate information regarding patient condition and health status. Ability to exchange and express information utilizing language and communicate information effectively.
Ability to hear the nature of sounds. Ability to give full attention to what other people are saying, take time to understand the points being made, ask questions as appropriate, and not interrupt at inappropriate times.

What You'll Do as a Hospice Chaplain:

Serve as an integral member of the interdisciplinary team, participating in patient care conferences and collaborating with nurses, social workers, aides, and medical staff.

About You
The candidate must meet one of the following educational requirements:
Bachelor's degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)
A minimum of one unit of ACPE-accredited Clinical Pastoral Education (CPE)
Note: Degrees from accredited institutions are preferred. If a degree is from a non-accredited college or university, AVPO approval is required prior to hire.
