POSITION SUMMARY
The Board Certified Behavior Analyst (BCBA) is responsible for designing, implementing, and supervising individualized therapy programs for individuals receiving Applied Behavior Analysis (ABA) services. These programs aim to improve daily functioning, independence, and overall quality of life. The BCBA utilizes evidence-based ABA strategies-such as positive reinforcement, shaping, prompt fading, and task analysis-to create effective behavior intervention and skill acquisition plans tailored to each client. In addition to clinical responsibilities, the BCBA provides ongoing supervision of RBTs, ensuring high-quality service delivery, adherence to treatment plans, and professional growth. This role includes overseeing data collection systems, analyzing behavior data to monitor client progress, and making data-driven decisions in collaboration with families and care teams.
ESSENTIAL DUTIES AND RESPONSIBILITIES
* Able to lead cross functional teams in support of client's treatment plan
* Administer assessments (e.g., ABLS-RR, AFLS) and create goals based off the assessments.
* Assesses the individual's performance and implements treatment based on established goals, including but not limited to the FBA (i.e., Functional Behavior Assessment, Behavior Support Plans, Behavior Reduction Plans).
* Serves, communicates, and consults as a member of an interdisciplinary team.
* Provides training and education to staff, caregivers, and other stakeholders.
* Provides feedback and ongoing support to Registered Behavior Technicians
* Participates in ongoing treatment meetings, as well as staff meetings and professional development opportunities.
* Provide direct supervision to Registered Behavior Technicians (RBTs) in accordance with BACB standards, including observation, performance feedback, competency assessments, and guidance to ensure effective and ethical implementation of behavior-analytic services. Responsible for promoting a positive Safety Culture is expected and necessary to make CHS a safe place in which to work. Safe work practices and policies for this position shall be reviewed and strictly followed.

POSITION SUMMARY:

The Registered Behavior Technician, under the direct supervision of the BCBA, implements therapy to support individuals in service needing applied behavioral and related supports to improve their daily functioning and independence. POSITION SUMMARY
The Payroll Specialist supports the CHS mission by maintaining payroll records as required by regulatory agencies.

SUMMARY
Service coordinators help people meet their personal needs and goals in obtaining the greatest degree of independence and inclusion possible in everyday community life. They help people with developmental disabilities and their families identify and obtain needed services and supports. They also advocate for, monitor, and evaluate services along with the individuals, their families, or guardians.
* Manages a caseload of varying ages, disabilities, and case complexity to improve the quality of support and services by establishing strong, trusting relationships through regular contact and planning with the individual and their team to identify needs and creating an action plan to assist the person in meeting those needs.
* Develops community relationships and support systems and if appropriate, links resources by matching the unique support needs of individuals and families (identified in the plan) with resources in the community. Linking may involve researching existing resources, developing new resources, making referrals to collaborating agencies with information and follow-up support, preparing Utilization Review requests, service authorizations, and coordinating federal, state, county, and community programs to achieve necessary supports.
* Ensures quality enhancement by monitoring and assessing the quality, timeliness and effectiveness of services and support received through an ongoing review process. Monitoring occurs via phone or email contact as well as through in person meetings. In person meetings are completed Monthly, Quarterly, and Annually. These meetings can occur in a variety of settings including but not limited to individual's homes, Day Programs, community settings, an office setting, group homes, and/or residential settings. If there are findings from the monitoring, reporting processes will be followed including protocols for mandated reporting in the case of suspicion of abuse and neglect situations.
* Make certain documentation is completed via written case note documentation for all activities for each individual within three business days of TCM activity, maintaining a minimum of 120 TCM billable hours per month.

ESSENTIAL DUTIES AND RESPONSIBILITIES
As the Community RN, you will:
* Promotes the physical and mental health of individuals served by coordinating and monitoring medical services, including health and medication services, in accordance with the requirements of regulatory agencies and in response to individual needs; providing at least 1.25 hours of service to each individual supported each month; participating in the admission of new individuals to assigned programs; clarifying medical information and advocating for individuals in hospital admissions and/or discharges, including communicating with relevant healthcare providers or pharmacists to review individual health statuses and medication plans; conducting full health assessments (including skin assessments) of individuals within 7 days of discharge from hospitalization or ER visits; coordinating ongoing care with clinics, external healthcare providers, hospitals and pharmacies; coordinating and reviewing lab work as required; traveling as needed to provide medical related support to individuals as directed; reviewing and teaching health and nutrition per individual needs; conducting Professional Assessment and Monitoring services as authorized; promoting individual empowerment and self-care activities including self-medication; and participating in on-call rotations with other community nursing staff, as directed and required by staffing and support needs;

* Carries emergency phone, in a rotational basis, as directed by Assistant Director. During this time, the manager must be at least 20 minutes to no more than1-hour drive response time and not be cognitively impaired. Responds immediately to requests for assistance.

This position will primarily be scheduled to work evenings and weekends.
ESSENTIAL DUTIES AND RESPONSIBILITIES
As a Job Coach, you will:
* Support Individuals by assisting them in an employment setting (both as an employee as well as at a Community-Based Assessment site, etc.) by providing the support needed for the individual to achieve the highest level of independent success.
* Empower Individuals by seeking opportunities in various employment settings where they can demonstrate independence, complete assigned tasks without assistance (when possible), and advocate for his/herself when needed.
* Integrate Individuals in their employment settings by assisting in developing natural supports, coordination of transportation, and appropriate workplace interactions with co-workers and patrons of said employer.
* Ensure Regulatory Compliance by completing all required documentation to standard within the timeframes established by both the Center for Human Services, as well as our respective funding sources. Additionally, job coaches will actively participate in training and meetings, demonstrating professionalism, reliability, satisfactory attendance, and accountability.
* Strengthen Positive Work Culture by participating in effective, courteous, and professional communication; showing kindness and patience to individuals and co-workers, demonstrating respect and responsibility in daily work, and practicing advocacy and encouragement in all aspects of employment.

QUALIFICATIONS
REQUIRED EDUCATION AND EXPERIENCE
* High School diploma or equivalent
* Consistent work experience equal to or greater than one year.
* Experience working with individuals with disabilities preferred.
REQUIRED MISCELLANEOUS QUALIFICATIONS
* Valid driver's license
* Reliable, licensed, and insured vehicle
* Must have reliable form of communication
* Ability to satisfactorily complete mandatory (paid) trainings to include CPR/First Aid
