Master's Degree from accredited school of Social Work required; Valid license/registration of permit to practice in state(s) of agency operation if required

Experience or education in grief counseling preferred.

Responsibilities:
As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Provide supervision of interns and/or social work fellows (SWF), as needed. If providing supervision of SWFs, must meet state regulations as a qualified supervisor.
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

Professional opportunities and experiences:
Students will provide comprehensive audiological testing to include otoscopy, cerumenectomy, diagnostic audiometric and immittance testing, and speech in noise testing. With a thorough case history and detailed evaluation, our goal is to provide patient-centered care for each individual. Cochlear implant evaluation, activation, and follow up services are available in our practice.
We provide services for patients of all ages; however, our primary focus is the adult population. Treatment may include hearing aids, cochlear implants, assistive listening devices and accessories, comprehensive counseling, and regular follow-up care. The Hearing Center of Chestertown is committed to ensuring that our patients are well-educated consumers and well-cared for in terms of their hearing loss and treatment plan. Students will use assistive devices including television and telephone connectivity, remote microphones, remote controls, and any other device to help patients connect to their everyday listening environments.
The Hearing Center of Chestertown also provides in-home care to patients who cannot easily come to our office, particularly those at Heron Point, an ACTS retirement community located a few miles from our office. We are a Community Care Center for veterans and collaborate with the VA to provide premium devices and services to our veteran population.
Important details:
The Hearing Center of Chestertown offers a basic monthly stipend to our final year externship doctoral students. We offer parking for students who drive to the office and can assist in locating housing in Chestertown if interested.
The Hearing Center of Chestertown offers an 11-month program of training starting May/June 2026 - April/May 2027. The student is expected to work approximately 32-40 hours per week.
We provide students with the opportunity to attend local continuing education opportunities and we engage our students with ongoing projects that support the overall work of the practice which enhances their professional development.
