Chestnut Health Systemsâ„¢ is seeking a compassionate and detail-oriented CaseManager (Housing) to join our team in Bloomington, Illinois. This position plays a vital role in supporting individuals on their path to recovery and stability by providing housing-focused casemanagement services, fostering independence, and promoting wellness. The CaseManager will deliver client-centered support using trauma-informed and recovery-oriented care principles.

Responsibilities
Complete all required documentation and data entry in accordance with agency and program standards, including but not limited to LOCUS, IM-CANS, HRA/PHA, and progress notes.
- Ensure 100% of IM-CANS are completed prior to due dates.
- Submit progress notes and other required paperwork within 48 hours of service delivery.
Provide services identified in the IM-CANS to meet consumer needs and program requirements, including:
- Maintaining required service levels and timely completion of authorizations.
- Delivering clinical services such as community support, casemanagement, therapy, and medication training.
- Supporting a clean, safe, and healthy living environment for residents.
- Providing services in the most effective setting, including consumers' apartments or community locations.
- Linking consumers to additional clinical and community resources as needed.
- Completing clinical screenings of new tenant housing referrals when assigned.
Promote the recovery model of treatment by assisting consumers in achieving self-directed recovery and incorporating trauma-informed care principles such as hope, empowerment, and respect.
Maintain positive relationships within Chestnut and with community partners including hospitals, courts, medical offices, and social service agencies to ensure coordinated, high-quality care.
Complete and maintain accurate documentation to meet housing funder requirements, including attendance and census tracking.
Provide referrals, advocacy, and linkage to essential community resources such as healthcare, food, financial aid, and housing supports.
Participate in meetings, case staffings, and program development activities. Provide backup and support to other team members as needed.
- Attend 100% of required supervision (Rule 132) if not an LPHA.
- Complete all mandatory Chestnut trainings and continuing education requirements through Relias.
Utilize evidence-based practices focused on skill-building, therapy, and recovery support for individuals, families, and groups.
Take a leadership role on assigned projects and initiatives.
Uphold Chestnut's culture of customer service excellence by adhering to organizational behavioral standards.
Protect the confidentiality of all client and organizational information at all times.
Qualifications
Education and Experience (one of the following):
- Bachelor's degree in a behavioral health field; or
- Bachelor's degree in an unrelated field with 2+ years of behavioral health experience; or
- High school diploma or equivalent with 5 years of social service or peer experience; or
- High school diploma or equivalent with CRSS or CPRS certification (in good standing with the State of Illinois).
Proficient in basic computer applications including Microsoft Word and Electronic Medical Records (EMR).
Valid driver's license, private automobile insurance, and insurability required.
Strong written and verbal communication skills.
Ability to work collaboratively with a multidisciplinary team.
Demonstrated understanding of trauma-informed and recovery-oriented practices.

**Responsibilities**
Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge.
+ Create plan for care across the continuum, integrating patient/family preferences and values.
+ Serve as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs.
+ Advocate for resources and removal of barriers.
+ Maintain ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan.

+ CaseManager credentialed from the Commission for CaseManager Certification (CCMC) preferred.

Education:
+ Bachelor's degree in Social Work required.
Work Experience:
+ 3 years of experience preferred.

Chestnut Health Systems is a leading provider of behavioral and primary health care services. Over four decades, Chestnut has grown to almost 800 committed, compassionate employees providing substance use, mental health, and primary care treatment to patients in Illinois and Missouri. We provide fully integrated care for all our patients by combining behavioral health care services with our community-based primary care health centers. Chestnut also conducts research and training throughout the U.S.A.
Responsibilities
* Engage clients in behavioral health services through outreach and assist in overcoming barriers to needed services.
* Conduct screenings, referrals, linkages, advocacy, and discharge planning to connect clients with recommended services and community resources.
* Coordinate care by sharing assessment and treatment information with other care providers, families, schools, and professionals, ensuring interventions are client-centered, trauma-informed, recovery-oriented, and strength-based.
* Maintain knowledge of community resources and provide appropriate referrals.
* Offer ongoing support and maintain communication with clients and referral sources throughout treatment.
* Develop and foster professional relationships with referral sources and other behavioral health agencies.
* Operate within a multi-disciplinary team, providing clinical guidance to less experienced staff and interns, and modeling Chestnut Health Systems' Mission, Vision, and Values.
* Facilitate transportation for clients and families to clinical appointments as needed.
* Document client services in compliance with agency policies, procedures, and grant requirements, ensuring timely completion of paperwork.
* Promote the recovery model of treatment, supporting clients' self-directed recovery and using recovery-oriented language in documentation.
* Support team members by providing backup when necessary and uphold Chestnut's customer service standards.
* Maintain confidentiality and security of all organizational information, including financial and client data.
* Perform other duties as assigned.
Qualifications
* High school diploma or equivalent with 5 years of supervised behavioral health experience, OR;
* High school diploma or equivalent and be a Certified Recovery Support Specialist (CRSS) or Certified Peer Recovery Specialist (CPRS) in good standing with the State of Illinois, OR;
* Bachelor's degree in counseling, social work, or related human services field OR;
* Bachelor's degree in any field with two years supervised clinical behavioral health experience.
* Basic computer skills, including Microsoft Office Suite, and Adobe Acrobat
* Electronic Medical Record (EMR) systems (preferred)
* Valid driver's license, private automobile insurance, and must be insurable

The anticipated starting pay for new hires for this position is between $19.00 - $20.00 an hour. There are several factors taken into consideration in determining base pay, including but not limited to: job-related qualifications, skills, education, experience, local market conditions, and internal equity.
