Our Adult Day Service (ADS) program provides a professional care setting in which seniors and adults with disabilities are transported to our center to receive individualized care in a community-based group setting. Our Adult Day Service center is located in Bronzeville and provides daily onsite care for seniors and adults with disabilities. ESSENTIAL DUTIES & RESPONSIBILITIES: These include but are not limited to the following. (The company reserves the right to add to, delete, change or modify the essential duties and responsibilities at any time). Arrange for vehicle repair with the assistance of the ADS Manager. Follow established schedules; adjusts schedules to meet unanticipated changes; determines the safest and most efficient pick-up and delivery routes and other destinations according to schedule. Responsibilities: • Assist passengers on and off the vehicle safely and courteously. Assist clients who use medical equipment, and handle emergencies that may arise. • Provide support, assistance, and always interact with participants during field trips and activities in addition to pick up and drop off. • Provide support in the activity rooms as needed. • Ensure the van is clean and in optimal condition for the comfort and safety of the clients. • Follow the daily schedule assigned to transport clients to numerous areas within the city of Chicago. • Provide a high level of customer service to clients and families. • Note visible changes in the physical or mental health of clients regularly transported; communicate this information to appropriate personnel for follow-up, and document unusual behavior. • Secure wheelchairs with restraining devices to stabilize wheelchairs during transport. • Ensure that clients are received by the caregiver and/or authorized person. • Complete daily route report. • Report and complete incident reports as needed. • Conduct weekly vehicle inspections. Thoroughly inspect the vehicle and maintain a clean and safe condition. • Report the need for maintenance and/or service to the ADS Manager. • Maintain standard records such as logs of passengers, mileage, and gasoline purchases. • Operates radio or similar device to communicate with the base station, other drivers, caregivers or to report disruption of service. • Follow at all times the ADS Driver manual. • Always ensure safety of clients • Must operate the van in accordance with vehicle and transportation laws, ordinances, and regulations. • Other job duties are assigned. QUALIFICATION REQUIREMENTS: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Valid Illinois Driver's License. 3-5 years of experience working with seniors or adults with disabilities. 3-5 years of driving experience. EDUCATION: High School diploma/GED. CPR certification is required. WORKING CONDITIONS: There are no unusual environmental conditions other than inclement weather and inappropriate driving of other vehicles. PHYSICAL REQUIREMENTS: While performing the duties of this job, the employee is regularly required to speak English, specifically, to communicate both in person and over the telephone.     The Financial Coordinator - FH provides direct services to low-to-moderate income individuals who have expressed an interest in achieving a financial goal. This position is responsible for assisting clients in developing plans of action that are intended to help them reach their goals and achieve financial stability. The Financial Coordinator will provide services in a one-on-one training format, as well as hold group training sessions and workshops related to budgeting, credit building, banking products, and public benefits. This position will work with the Family Hub staff, family workers, and senior services staff to ensure that the client is getting assistance across these four major service areas. The Financial Coordinator will also engage in outreach and networking with clients, community members, business entities, local financial institutions, and Chicago Commons staff to serve families effectively. Education & Certification Requirements A bachelor's degree in finance, economics or related field PREFERRED. Two years of related field experience with a non-profit or community partnership organization. Experience & Training Requirements Bilingual - fluent in English/Spanish, preferred. A strong understanding of personal finances, particularly in the areas of budgeting, asset building, credit building, and public benefits services. Knowledge of entrepreneurship programs a plus. Effective communication skills, both written and oral. Must be comfortable presenting to small and medium-sized groups. Must be a self-starter and a team player. Have the ability to provide financial coaching and training services in a culturally sensitive manner. Proficient in Microsoft Word, Excel, Salesforce, PowerPoint. Complete a financial assessment intake form with all financial coaching clients. Assist the client in developing a budget and provide strategies for budget improvements. Access the clients' credit report/score and provide strategies for credit repair/building. Document the client's balance sheet and provide strategies for increasing net worth. Develop plans of action and provide tools, resources, and accountability to the client to help them meet their goals. Conduct or coordinate relevant periodic participant-informed group training sessions or workshops across all Chicago Common's service areas.
