State Aid Care Managers establish supportive, helping relationships with people who do not have Medicaid, or transition in and out of Medicaid eligibility. State Aid Care Managers assist in achieving improved overall health and social functioning. Work is done primarily in the community and position entails travel throughout Suffolk County.
