Position: State Aid- Adult Care Manager Compensation: $52,000; non-exempt Location: Riverhead, NY and travel primarily throughout Suffolk County Hours: 37.5 hours per week; flexibility for evening and weekends State Aid Care Managers establish supportive, helping relationships with people who do not have Medicaid, or transition in and out of Medicaid eligibility. State Aid Care Managers assist in achieving improved overall health and social functioning. Work is done primarily in the community and position entails travel throughout Suffolk County. Providing supportive, recovery and wellness-directed coaching and counseling in-person and via telephone communication to a caseload of approximately 30-40 clients. Development and implementation of outreach strategies to engage State Aid target communities/clients ensuring alignment with agency and program goals. Forging relationships and establishing provider & community contacts to build the State Aid roster. Qualifications: A Bachelor's degree in a human service/health discipline and two years' experience is preferred. Candidates with Master's degrees are strongly encouraged to apply. The Assistant Teacher will support the Lead Teacher to ensure that families receive quality services which provide for growth in academics, social and emotional development, and that the program meets the educational, physical and developmental needs of the program participants. The Assistant Teacher will assist the Lead Teacher in planning, scheduling and implementing the daily program, and making daily observations of the children.
