JOB SUMMARY The Triage Worker Provides screening and immediate support and referral options to consumers who call Prestera's access and after-hours numbers in search of assistance. DUTIES AND RESPONSIBILITIES Engages and communicates effectively with consumers and families and team members. Collaborates effectively with internal and external customers. Completes screenings to identify needs of the consumer as needed. Schedules appointments and assures all parties have relevant consents and information needed. Arranges transportation for consumers if needed. Follows all referrals through completion of services. Provides hand off communication to staff when an open consumer accesses the crisis line for support. Completes shift reports and hands off any paperwork to the oncoming shift. Assures documentation is completed by the end of shift. Document all activities electronically within 24 hours of service. Utilizes technology to the fullest degree possible to expedite crisis service processes for the consumer and other collaborating partners. Attends all departmental and center meetings/trainings as assigned. Adheres to confidentiality, HIPAA, and risk management policies and procedures including but not limited to completing Incident Reports. Participates in other functional work by participating in related projects, as applicable and directed by Center leadership SUPERVISORY RESPONSIBILITIES No direct supervisory responsibilities are required. Qualifications QUALIFICATIONS AND REQUIREMENTS EXPERIENCE, KNOWLEDGE, AND SKILL REQUIREMENTS: Valid Driver's License Must have a high level of interpersonal skills to handle sensitive and confidential situations. Possess strong written and verbal communication skills in conjunction with strong time management and organizational skills with the ability to complete multiple tasks and high volume of work with deadlines. Must be able to lift up to 15lbs. EDUCATION AND TRAINING REQUIREMENTS: High School Diploma/GED, Associate's Degree preferred. Minimum of 1 year experience in the field.

Additional Essential Functions and Areas of Accountability: Know and follow infection control policies including proper hand washing techniques. Report any observed or reported changes in the client's condition and/or needs to the Registered Nurse. Document care provided and complete all forms required for the client's records. Complete the appropriate records to document cares given and pertinent observations. Promotes personal safety and a safe environment for clients by observing infection control practices, following agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager. Demonstrate safe practice in the use of equipment, avoiding equipment use until orientation has been provided. Notifying supervisor(s) of educational needs immediately. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested and through approved channels in a very timely manner.
