SUMMARY
Position Type: Non-exempt, full-time or part-time, hourly role.
Scope of Role:
Responsible for providing direct support within a single program or unit within a program, typically supporting 4 - 6 individuals in residential settings, or up to 10 individuals in periodic programs.
Key Responsibilities:
Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, skill/behavioral development.
Assist with daily activities: Personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
ESSENTIAL JOB FUNCTIONS
To perform this job successfully, an individual must be able to satisfactorily perform each essential function listed below.
Services and Supports:
Direct Support: Provides direct support or assistance in accordance with individual service or program plans; assists with socialization and behavioral development, personal care, housekeeping, recreational activities, transportation, community orientation, shopping, financial management, citizenship, and other activities of daily living.
Records: Accurately documents progress and activity; reviews records and logs to stay abreast of changes in service plans; maintains confidentiality.
Management of Individuals' Assets: Makes cost-effective and appropriate purchases within the budget of the individual served and according to their choice / preference
Relationships: Maintains healthy and professional relationships with individuals, friends, families, guardians and case managers; greets family members and other visitors; implements Sevita Customer Service Standards.
Reporting: Reports any instance of alleged abuse or neglect according to internal and external standards; reports medical, behavioral and other incidents in accordance with Sevita policy and external requirements.
Rights: Maintains confidentiality, respects the rights of persons being served, according to applicable the bill of rights; practices universal precautions; assists individuals in exercising their rights.
Health Care:
Appointments: As needed, may accompany individuals to medical appointments; relays orders and information to and from medical providers as required.
Medications: If assigned, accurately administers or supports self-administration of medication and documents delivery of medications and treatments; promptly reports administration errors; maintains appropriate security of controlled medications and other medications and supplies.
General Health Care: Monitors individual's health; documents health concerns; communicates with nurse or supervisor as appropriate; calls 911 in the event of emergency; follows individual health care directives.
Medical Supplies and Equipment: Correctly follows procedures in utilizing medical equipment; reports malfunction or disrepair.
Meal Preparation: Prepares or supports individuals to prepare food in accordance with planned menus and individual choice; knows special diets and prepares individual meals accordingly; monitors mealtime and correctly implements any dining plans.
Employment Responsibilities:
Training: Attends orientation and on-going training as directed; participates in monthly staff meetings.
Employee Scheduling: Works scheduled hours; arrives for work on time; stays on shift until replacement arrives; notifies supervisor in advance when unable to work as scheduled or as soon as possible after the designated start time; may fill in for other shifts where required without causing unapproved overtime, or in the event of emergency.
Teamwork: Works together with others, values others contributions; is courteous; communicates openly; listens; and shows respect to others.
Workplace Safety: Every employee plays a role in developing and maintaining a safe workplace; complies with all established safety policies, procedures, and rules; reports unsafe hazards to their supervisor and participates in safety related training or activities.
Maintenance:
Vehicles: May transport individuals into the community; drives safely and according to local laws; ensures proper use of safety equipment including seat belts, lifts and wheelchair ties; reports accidents to appropriate authorities immediately; reports accidents and safety concerns immediately to supervisor or maintenance personnel.
Housekeeping: Performs daily and seasonal housekeeping as directed; maintains neat, clean and safe environment; reports hazards or safety concerns.
Maintenance and Repair: Monitors environmental safety and may make minor repairs, change light bulbs, etc.; may perform yard maintenance including keeping walkways safe and snow removal; reports equipment or facilities requiring repairs to supervisor or maintenance personnel as required.
Safety: Checks water temperature as required when assisting with bathing; participates in safety drills and protects persons being served in the event of emergency.
Other:
Performs other duties and activities as required.

Minimum Knowledge and Skills required for the Job
The requirements listed below are representative of the knowledge, skill, and/or abilities required to perform the job.
Education and Experience:
High school diploma or equivalent preferred
Six months of experience in human services preferred.
Must be 18 years of age.
Working knowledge of computers
Certificates, Licenses, and Registrations:
Valid driver's license in good standing.
Car registration and vehicle insurance if providing transportation for individuals receiving services.
All state-required training(s) and certification(s) completed in mandated timeframes.
Maintain valid driver's license, meet all insurance requirements, and be approved by management to be a driver for the company. If an employee is determined to have lost their driving privileges by failing the company MVR driving record requirements per company policy, the employee will be required to sign a Non-Driver form and restricted from driving on behalf of the company until their MVR report satisfies the company requirements for a good driving record.
Work Environment:
Works at the program location and may accompany individuals into the community.
Physical Requirements:
Heavy Work.Exerting up to 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.

Employees must cooperate with the licensee and Department of Human Services staff in any inspection, inquiry, or investigation.
Must have a valid driver's license in good standing in The State of New Jersey; however, in the event the person lives in another state and is commuting, they must have a valid driver's license in good standing for the state in which they live.
At minimum, a High School Diploma/Equivalent is required

JOB SUMMARY
The Pet Care Assistant works with the veterinary medical team to provide and prepare for the best medical care of pets, and to ensure this is provided in a happy, welcoming, friendly and efficient manner, influencing clients to return and refer their friends and families.
ESSENTIAL RESPONSIBILITIES AND TASKS
Live and exemplify the Five Principles of Mars, Inc. within self and team.
Prepare the treatment room for examination of pets and all necessary medical item and supplies (e.g. vaccinations, fecal loops, slides, etc.) for the veterinarian and/or veterinary technician. Set up fecals, smears, and skin scrapings as allowed by state rules and regulations.
Maintain the flow of the Cycle of Service in accordance with position. Provide Client Education regarding preventive care, Optimum Wellness Plans, and basic status of hospitalized pets. Serve as support to the Client Services Coordinator staff on an as-needed basis.
Hold or restrain pets during examination, treatment, or inoculation.
Prepare routine in-house laboratory tests (e.g. canine heartworm, feline leukemia, FIV, and canine parvovirus) as allowed by state rules and regulations.
Monitor the comfort of hospitalized patients. Ensure the comfort of hospitalized patients by "walking", exercising, feeding, and cleaning cages and pet (when approved by the veterinarian or veterinary technician).
Bathe and dip pets when necessary to treat dermatological conditions (when advised by veterinarian or veterinary technician.
Clean the surgical suite and instruments.
Ensure cleanliness of the hospital including but not limited to housekeeping such as laundry, dishes, mopping, cage cleaning, trash collection and removal and instrument cleaning as outlined by hospital protocols

The Direct Support Professional will provide support, training, and supervision to people with intellectual disabilities in an Intermediate Care Facility (ICF) or Community Training Home (CTH).
ESSENTIAL RESPONSIBILITIES & DUTIES:
1. Behavior should be professional at all times and consistent with the Agency's, mission and policies and procedures, including treating individuals/consumers with dignity and respect
2. Exhibit appropriate work behavior with consumers, co-workers, families, consultants and other professionals in all settings.
3. Will remain on duty until relief staff arrive.
4. Assist consumers in maintaining safe and healthy lifestyles
5. Monitor and report changes in consumers' general health, appearance, behaviors and patterns of activities to include eating, sleeping and toileting activities.

24. Provide medications and treatments as ordered following Agency procedures, complete MAR, follow procedure to dispose expired medications and inform nurse of shortage of medications, med. errors and need to give over-the-counter medications when working in facilities not staffed by a nurse assigned specifically for consumer medication assistance.
25. Assist consumers with their finances according to Agency procedures to include paying bills, reconcile cash on hand with receipts and withdrawals/deposits, maintain checkbooks, be an authorized co-signer on consumer accounts, coordinate reviews with Finance Dept., etc.
26. Maintain thorough understanding of agency policies and procedures and conduct self accordingly to include: job attendance, job performance, safety protocol, etc.
27. Attend staff meetings and complete on-going training requirements within required timeframes and meeting required competency levels.
28. Maintain confidentiality of consumer information and personal passcodes
29. Use electronic equipment and Agency software (including messaging) in a professional and ethical manner and for business purposes only.

Lead DSP Duties:
1. Monitor Medication Administration Records (MAR) performed by staff to ensure it is completed timely and accurately in a CTH setting. Reports issues/errors/concerns to supervisor and records as a General Event Report (GER) in Therap.
2. Complete household Purchase Orders, grocery shopping, and work orders as required for consumer and residence needs and function dictates for operation.

TYPICAL PHYSICAL DEMANDS:
The Direct Support Professional may be required to perform lifting of objects weighing less than 50 pounds. The PCA is a non-skilled worker that may possess a certificate of training either formally (i.e., Certified Nurse Assistant (CNA) or in-house formal training; competency and assessment by an experienced RN, licensed by the state of South Carolina or an experienced LPN, with at least 3 years of experience). The training and supervisory will be under the direction of an RN. There will be subsequent supervisory assessments of PCA services 30 days after initiation of services; then at least every 90 days thereafter, one of which the PCA must be present in the home.
Primary Functions of Job:
To service elderly and/or disabled adults and children. Under no circumstances will any type skilled medical service be performed by a PCA.

The PCA must meet the following minimum qualifications:
*High School diploma or GED equivalent
*Able to read, write and communicate effectively with the client and supervisor
*Able to use the Care Call IVR system
*Capable of assisting with activities of daily living
*capable of following a care plan with minimal supervision
*Have valid driver's license/reliable means of transportation.
*At least 18 years of age
*Passed competency testing or successfully completed a competency training and evaluation program performed by an RN or LPN prior to providing services
*Other training, as required by SCDHHS and/or Medicaid Scope of Services for Personal Care II and HASCI Attendant Care, SCDDSN, and Veterans Affairs.
Abilities required:
Must be fully ambulatory and physically capable of assisting with the activities of daily living, as outline above.

SUMMARY
The Direct Support Professional will provide support, training, and supervision to people with intellectual disabilities in an Intermediate Care Facility (ICF) or Community Training Home (CTH).
POSITION INFORMATION

POSITION TYPE: Non-Exempt
ESSENTIAL RESPONSIBILITIES & DUTIES:
Behavior should be professional at all times and consistent with the Agency's, mission and policies and procedures, including treating individuals/consumers with dignity and respect
Exhibit appropriate work behavior with consumers, co-workers, families, consultants and other professionals in all settings.
Will remain on duty until relief staff arrive.
Assist consumers in maintaining safe and healthy lifestyles
Monitor and report changes in consumers' general health, appearance, behaviors and patterns of activities to include eating, sleeping and toileting activities.
Provide training and assistance to all consumers in making healthy dietary choices and following any pre-established orders by medical professionals
Maintain individual safety and well-being, conduct safety drills/training, evacuations, etc.
Use adaptive supplies/equipment as prescribed
Provide the prescribed level of supervision and accountability throughout the individual's daily routine (including dining, bathing, awake and sleeping hours)
Assist, as needed using approved prompting techniques, with individual's bathing, toileting, grooming and other daily living and self-care activities.
Follow proper lifting techniques to ensure proper care of individuals who are non-ambulatory
Record and discuss individuals' health status with nursing staff
Provide well maintained living environment, promptly report all maintenance needs to supervisor and complete necessary housekeeping duties to include sweeping, mopping, laundry, dusting, taking out garbage, cleaning bathrooms and kitchen, etc.
Prepare meals and snacks according to menu and approved diets of individuals
Plan, support and perform skills training activities to individuals in all daily living life skill areas to include: chores, fitness activities, money management, recreation, etc.
Provide naturally occurring opportunities for individuals to learn
Keep individuals actively involved according to individuals' active treatment plan
Document data during formal training sections as specified in the individual's objectives
Reinforce informal training throughout daily routine of the individual
Inform supervisor of all individual injuries at time of occurrence and complete General Event Report (GER) in Therap.
Record understandable notes in various Therap modules and paper reports as required (includes T-Logs, SComms, ISP data, GER, supervision logs, staff duty sheets, etc.) as specified in the individual's plan.
Implement Behavior Support Programs, as written, to include documentation of all behavioral incidents. May require physical intervention and the use of MANDT techniques
Safely transport consumers to and from leisure/recreational activities, errands, work, and personal appointments and comply with the Agency's vehicle safety, maintenance and cell phone use policies. Operate wheelchair lifts and safety restraints as trained.
Provide medications and treatments as ordered following Agency procedures, complete MAR, follow procedure to dispose expired medications and inform nurse of shortage of medications, med. errors and need to give over-the-counter medications when working in facilities not staffed by a nurse assigned specifically for consumer medication assistance.
Assist consumers with their finances according to Agency procedures to include paying bills, reconcile cash on hand with receipts and withdrawals/deposits, maintain checkbooks, be an authorized co-signer on consumer accounts, coordinate reviews with Finance Dept., etc.
Maintain thorough understanding of agency policies and procedures and conduct self accordingly to include: job attendance, job performance, safety protocol, etc.
Attend staff meetings and complete on-going training requirements within required timeframes and meeting required competency levels.
Maintain confidentiality of consumer information and personal passcodes
Use electronic equipment and Agency software (including messaging) in a professional and ethical manner and for business purposes only.
Other duties as assigned.
Lead DSP Duties:
Monitor Medication Administration Records (MAR) performed by staff to ensure it is completed timely and accurately in a CTH setting. Reports issues/errors/concerns to supervisor and records as a General Event Report (GER) in Therap.
Complete household Purchase Orders, grocery shopping, and work orders as required for consumer and residence needs and function dictates for operation.
Complete daily census reports in DDSN software portal in an ICF setting.
TYPICAL PHYSICAL DEMANDS:
The Direct Support Professional may be required to perform lifting of objects weighing less than 50 pounds. The Direct Support Professional may be responsible for lifting adult individuals with assistance to provide aide with hygiene activities. Must be capable of effective verbal and written communication with individuals served, families, co-workers and visitors. Must be capable of moving intermittently throughout the day and performing such activities as climbing, balancing, kneeling, crouching, crawling, reaching, pushing, pulling, repetitive motions, standing, bending, walking, sitting and stooping.
TYPICAL WORKING CONDITIONS:
Direct Support Professional is involved on a regular basis with personnel, visitors, contracting businesses and people with disabilities receiving services. The Direct Support Professional may be required to work in other locations and program areas as needed; is subject to exposure to communicable diseases and physical assault. The Direct Support Professional is subject to a variety of work settings such as indoors, outdoors, varying temperature extremes, varying levels of noise, and may be required to wear a variety of safety/protective equipment such as eye protection, gloves, personal hearing protection and respiratory protection. The Direct Support Professional must be able to safely operate agency owned vehicles including driving during night hours.
PERFORMANCE REQUIREMENTS:
Knowledge, Skills, and Abilities: Ability to write the English language in a legible manner, ability to relate harmoniously with co-workers and families, ability to follow directions, ability to work independently, ability to complete quality work in a timely manner, ability to exercise independent judgment.
Education: High School Diploma or GED, minimum years of age is 18.
Experience: One-year experience working with people with developmental or related disabilities preferred.
Certificate/License: None required.
Other: Must possess a valid South Carolina Driver's License and maintain an acceptable driving record.
