In this position, you will:
Help residents achieve/exceed their personal goals

We require:
Clean, valid NYS Driver's license
High School diploma or equivalent
Some experience working with the intellectually and developmentally disabled population preferred; but will train. Cooking and light housekeeping may be required.
Must obtain and maintain AMAP (within six months of hire) and SCIP certifications
Prior experience as Caregiver, Care Assistant, Personal Assistant, Resident Assistant, Direct CareWorker, Nursing Assistant, Home Care Aide or Companion is a plus

The Direct Support Professional is responsible for safety and supervision of our consumers with Intellectual Developmental Disabilities, Behavioral health histories to facilitate healthy and socially acceptable behaviors while living in community-based sites. This position will coordinate with clinical staff, implement residentially based interventions and assist the clients in achieving their clinical assignments. The DSP is responsible for fulfilling the obligations as outlined in the client's program plan. Position: State Aid- Adult Care Manager
Compensation: $52,000; non-exempt

State Aid Care Managers establish supportive, helping relationships with people who do not have Medicaid, or transition in and out of Medicaid eligibility. These individuals have significant mental health needs and may have also experienced trauma and have physical health and/or substance abuse challenges. State Aid Care Managers assist in achieving improved overall health and social functioning. Work is done primarily in the community and position entails travel throughout Suffolk County. Agency vehicles can often be used but a safe, personal auto and a clean NYS Driver's License are required; mileage reimbursement provided. Qualifications:
A Bachelor's degree in a human service/health discipline and two years' experience is preferred. Candidates with Master's degrees are strongly encouraged to apply.
Excellent inter-personal communication including the ability to establish empathic, empowering relationships with people who have experienced psychiatric, substance abuse challenges, trauma as well as chronic physical health conditions;

Maintain positive community relationships
Attend planning meetings, seminars and treatment sessions.
Assist in scheduling and attending medical and dental appointments for the children
