The Surgical Home Hospital Advanced Practice Provider will support the work of the Surgical Home Hospital by evaluating (either in person or virtually) and managing patients in their home as a direct Surgical Home Hospital provider, supported by a multidisciplinary team that includes an Attending Surgeon (who oversees the direction of care for patients enrolled), a Home Hospital nurse, a paramedic (Mobile Integrated Health), an administrative coordinator, and other allied staff as needed (e.g. home health aide, physical therapist). The Surgical Home Hospital APP will work with the inpatient surgery teams at all five participating MGB Hospitals (MGH, BWH, Salem, Faulkner, NWH) to identify patients (both operative and non-operative) admitted to the hospital-based Surgery Service or in the Emergency Department who can be admitted to the Surgical Home Hospital service. The Surgical HH APP will screen for appropriate patients, communicate with the hospital-based Surgery teams about appropriate patients, consent patients for admission, and subsequently manage patients on the Surgical Home Hospital Service. These patients may be in the Emergency Department and deemed appropriate for direct admission to Surgical Home Hospital, admitted to the Surgery inpatient service and deemed appropriate for transfer to the Surgical HH Service &/or be elective surgery patients who in the pre-operative setting were identified as appropriate for transfer to HH in the postop setting. Upon admission to the Surgical Home Hospital service, the Surgical HH APP will see the patient either in the brick & mortar hospital, in person in the patient's home or virtually. The Surgical HH APP will be responsible for placing admission or transfer orders to the Surgical HH Service. Daily Surgery HH rounds will consist of at least one daily patient encounter (in-person or virtual depending on patient needs and clinical course), with medical decision-making and delivery of needed care as appropriate (see specific clinical duties below). The Surgical HH APP will facilitate continuity betweencare settings, ensure home-based and acute care protocols are seamlessly integrated in the home settings, and function as the responding clinician in the home environment. The ideal candidate for this Surgical HH APP role will be a patient-centered, proactive, flexible, experienced, and confident clinician comfortable with autonomy. Must collaborate well with multidisciplinary teams and able to make complex clinical decisions as well as be comfortable with level-of-care triaging, and telemedicine. The role offers the potential to broaden clinical duties as the Surgical HH Service expands. This may include involvement with the research & development team creating surgical HH pathways and protocols. As well as daily collaboration w/ the Attending Surgeon of record to ensure closed loop communication about the plan of care. · Communicates with the patient and family about Surgery HH, incorporating patient and family values and goals in the plan of care. · Maintains a safe care environment for HH patients and leads in the practice of and reinforcement of a strong safety culture among all clinical partners involved. · Attends relevant team and other meetings related to Home Hospital and related work. · Participates with quality measurement and/or quality improvement initiatives, as appropriate, to advance the work of Home Hospital and other alternative care pathways, including advancements or innovations within the electronic health record. Communicates regularly with teacher, lead teacher, and/or supervisor and shares all pertinent information as quickly as possible. Observes and records children's behavior for the purpose of assessing skills and planning appropriately to meet individual and group needs. Recognizes areas of concern, assists in appropriate intervention or needed referrals and participates in conferences. Assesses classroom needs for equipment, materials and supplies and reports to lead teacher and/or supervisor. Maintains the space, equipment, materials and supplies in a safe, healthy, clean and orderly fashion. Assists in providing accurate and timely reports, record keeping and other documentation required for assigned classroom. Maintains strict adherence to health and safety policies and procedures. Attends all required staff meetings, workshops, trainings, committee meetings and parent functions. Mandated Reporter (51A) training is mandatory for all staff. Responsible for providing positive and supportive relationships by maintaining contact with families by phone calls, conferences, home visits, arrivals & departures, and communicating family volunteer opportunities. Actively supervises children at all times. Engage all clients by understanding and addressing their needs whether within or outside the scope of work. Attend & participate in engagement team meetings as requested and communicate effectively with families and staff in other areas. Maintain confidentiality of client, employee and agency information in accordance with federal and state laws and funder requirements. Ensure compliance with program/department, agency and/or funder requirements, as well as, SMOC policies & procedures. We do our very best to be consistent with classrooms and sites however, the needs of the children and families may require us to make changes as needed to provide the best quality to our program. Other duties as assigned. Knowledge and Skill Requirements: EEC PS Teacher certificate preferred Clear Communication skills Degree in ECE or related field preferred. Must be comfortable working with diverse backgrounds and able to effectively communicate with children, families and co-workers. Bi-lingual skills a plus. Priority given to past or present Head Start parents for positions for which they are qualified in the Head Start program. Organizational Relationship: Directly reports to Site Director. Indirectly reports to Lead Teacher, Asst, Directors. Direct reports of this position are Teacher Aides. Physical Requirements: Must be able to handle the physical aspects of the position including the lifting of a child, and moving quickly in the event of an emergency. Recent physical exam determining good health and meeting all EEC requirements. COVID19 Vaccines are strongly encouraged for all staff. Working Conditions: As part of the responsibilities of this position, the Lead Teacher will have direct or incidental contact with clients served by SMOC in various programs funded or administered through the Executive Office of Health and Human Services.
