Purpose Statement The Casting Associate is responsible for filling ice cream cake molds in an assembly line process, blending crunch, peanuts and other deco ingredients as assigned. Also maximizing product quality and achieving individual outputs to support team in meeting its production goals.     The Family Support Provider (FSP) for Stronger Families, Stronger Futures (SF2) provides support and education in the home environment to parents and their children (pre-natal to age 5) who are at risk for child abuse and neglect. The FSP works in collaboration with service providers to reduce abuse or neglect. SF2 also focuses on the child development of the child(ren) through various assessments and the Parents as Teachers curriculum. We focus on the three areas of emphasis; Family Well-being, Parent-Child Interaction, Development-Centered Parenting. GENERAL DUTIES AND RESPONSIBILITIES * Provides home visits to assigned families where children may have been identified through program screening criteria as being "at risk" for abuse and/or neglect. * Provides home visits to assigned families who are identified as needing services including but not limited to parenting and child development education, advocacy and support and assistance with accessing community resources. * Makes home visits based on the needs of families. * Provides families with information that includes but is not limited to child development, parenting, safety, immunizations and community resources. * Utilizes program-approved curriculum to improve parenting skills and increase the understanding of normal child development. * Develops strength-based service plans to address areas identified as challenges. * Assists client in seeking additional services from community resources as needed. * Maintains collaborative relationship with primary care provider and community services. * Accepts new assignments as determined by the Supervisor. * Actively participates in supervision. * Immediately notifies supervisory staff of situations in which children may be at risk (i.e., physical abuse, sexual abuse, chronic neglect, unsafe home, etc) and/or any other sentinel event. * Maintains accurate record of all client contact (and collateral work) including service hours, number of home and office visits, contact with collateral agencies, travel time, office time and training hours. * Completes client contact log, all required paperwork and statistics per agency standard and requirements. * Testifies in court as needed. * Makes referrals to appropriate community resources. * Maintains the confidentiality of the client records and information at all times. * Attends and actively participates in unit and program staff meetings. * Promotes agency's services, programs, image and philosophy. * FSP who meet the credentialing requirements for the senior home visitor position may, in addition to their regular duties, be assigned leadership responsibilities within the home visiting program. * PQI Functions * Ensure the consistent accurate collection of the data * Identify patterns and trends for program * Use results of data to inform supervisor of trends that may impact services * Performs other duties as assigned by the supervisor. General Responsibilities Facilitate, apply, practice, and demonstrate Cognitive Behavioral Theory (CBT) using Roca's Intervention Model. Assist and role model in the process of obtaining and sustaining employment through job search and overall workforce professionalism. Founded just outside of Boston in 1988, Roca operates a nationally acclaimed Intervention Model with five sites in Massachusetts and one in Baltimore, Maryland, and Hartford, CT. Over 37 years, Roca has learned that to impact urban violence, we must work directly with the young people and systems at the center of it. Roca's Intervention Model engages young people at the center of urban violence in an intensive four-year program to address trauma, teach life-saving cognitive-behavioral skills, and break the cycle of incarceration and poverty that traditional youth programs alone can't break. And, because it takes all of us, Roca also helps institutions like police and state agencies change the way they work with young people who are traumatized and living at the intersection of gangs, drugs, poverty, violence, immigration challenges, and involvement with multiple public systems. Position Overview Roca's work is based on the theory that young people, when re-engaged through positive and intensive relationships, can change their behaviors and develop life, education, and employment skills to disrupt the cycles of poverty and incarceration. Roca's Intervention Model engages the highest-risk 16-24 year olds in a long-term process of behavior change and skill building opportunities. Understanding that meaningful change doesn't happen overnight, we deliver an Intervention Model that is four years long and allows for relapse during the process. We know that lasting change requires a safe space to grow and to flex newly acquired decision-making skills. If a young person is seeking change or can make it through our model without relapsing, then they are not a candidate for Roca and can likely be served by a less intense program. The Youth Worker will report to the Director of Roca Hartford and have the primary responsibility of engaging 20-25 very high-risk young mothers through relationships and programming as well as working with community partners (DCF, Probation, and other community-based providers) within the City of Hartford. The successful Youth worker will work closely with team members, supervisors, and others within the organization to ensure that each young person is driving to outcomes of economic independence and living out of harm's way. We strive to meet short and intermediate term outcomes for Young Mothers including but not limited to: Increased Gains in Emotional Regulation, Increased Utilization of Early Childhood Services, Reduced Negative Systems Involvement, and in the long term: No Re-Incarcerations and Retained Employment. Responsibilities Maintain a caseload of young people and build transformation relationships in alignment with Roca's intervention model. Demonstrate the ability to provide additional types of educational/vocational/life skills programming, including but not limited to educational supports, financial literacy, workforce readiness, parenting and child development, healthy habits, driver's education, and/or help with job placement. Strategically plan workweek schedules- including outreach lists for contact, programming needs, identifying if YP needs additional assistance and CBT coaching and constructive use of Track Books. Facilitate, apply, practice, and demonstrate Cognitive Behavioral Theory (CBT) using Roca's Intervention Model. Consistently exercise and application of Cognitive Behavioral Theory (CBT). Assist and role model in the process of obtaining and sustaining employment through job search and overall workforce professionalism. Engage with other aspects of work, such as but not limited to, family and criminal court engagement, work behind the wall/in residential programs, peacemaking circles, work with the community, productive and professional participation in meetings, etc. Ensure accurate and timely entry of case notes into ETO (Efforts to Outcome system) daily. Analyze and interpret ETO data as it relates to young people and understanding how data relates to a young person's progress. Adapt outreach efforts in accordance of data collected in ETO. Excellent at organizing, managing and completing multiple complex projects and tasks simultaneously with thoroughness, accuracy, timeliness and good humor. Self-motivation, initiative, sound judgment, and commitment to ongoing learning are essential. Ability to work as a part of a team. Bilingual in Spanish preferred.
