The Child and Family Guidance Center is currently seeking a full time Behavioral Healthcare Quality Assurance Manager.
The Behavioral Healthcare Quality Assurance Manager will ensure compliance with internal and external program standards, including but not limited to state and federal contracts, Medicaid fee-for-service billing, and HIPAA privacy and security rules. The Quality Assurance Manager will play a critical role in monitoring fee-for-service billing by ensuring that electronic documentation is complete, accurate and meets compliance standards for reimbursement by CT DSS/Medicaid and all other funders. The Quality Assurance Manager will perform quality assurance reviews, produce valid and reliable data reports, and coordinate policy, procedure, and standardized form development.
The selected individual will be responsible for but not limited to the following Essential Functions:
Maintain government contracts and ensure compliance with licensing and credentialing requirements.
Maintain up-to-date knowledge of state and federal regulations, including CT DSS/Medicaid fee-for-service billing, and any changes that occur to ensure compliance with these rules and regulations.
Work with program staff, Fiscal Department, and the CEO to develop, implement and monitor effective systems for CT DSS/Medicaid fee-for-service billing; monitor billable services and ensure compliance with program standards.
Conduct internal quality assurance reviews addressing DSS/Medicaid and state licensing requirements. Work collaboratively with program staff and agency leadership to share findings from internal reviews and external audits and lead quality improvement initiatives.
Produce reliable and accurate outcome data from agency databases to support the agency's DEI initiatives.
Provide thorough analysis related to data reports and audits and develop PowerPoint and other types of presentations for internal and external use.
Implement, monitor, and report on HIPAA privacy and security practices.
Support development of new policies and procedures and develop and manage an annual review process for all policies, procedures and forms.
Support case record/chart review processes across programs.
Maintain comprehensive staff training and development requirements; coordinate, facilitate and assign staff training. Coordinate training and staff development in evidence-based practices.

Qualifications:
MSW, MBA, MPH, or related MA/MS degree preferred.
Knowledge and experience with Medicaid reimbursable services preferred
Two to three years of post-graduate experience in human services or mental health.
Prior compliance and/or quality improvement experience preferred
Knowledge and experience with Medicaid reimbursable services preferred
High level of skill with MS Office software

The Family Support Specialist works directly with the families in their homes. Family support specialist are responsible for providing comprehensive and intensive home visiting services to both parents and implementing the Healthy Families America model. Family support specialist should have experience working with vulnerable children and families.
The selected individual will be responsible for but not limited to the following Essential Functions:
Family Support Specialist's work directly with families, providing services in the family's home, at least once a week initially, using the Healthy Families America's evidence-based model to improve parenting skills, enhance the family's support system, improve parents' understanding of children developmental stages, and the family's ability to advocate for themselves and their children.
They will work closely and in a collaborative manner with the families to develop a strength-based “Action Plan” to address areas identified as challenges. The FSS will also be responsible for assisting the family in establishing goals and a plan for accomplishment of those goals, as well as the assessment of the normal growth and development of the target child.
Assist families with addressing barriers to attending important appointments.
Maintains accurate records of all client contacts and records utilizing multiple systems.
Assist and model for the parents how to access resources and utilize their natural supports.
Maintain collaborative relationships with other services and providers.

Qualifications:
Family support specialists must have a Bachelor's or Associate's degree in Early Childhood with training as a Parent Educator with a minimum two years' experience working with young children and families, prenatal to age six.
They must have previous experience working with vulnerable families.
Must have excellent analytical, interpersonal and communication skills (both verbal and written), computer proficiency, and the ability to communicate effectively with people from diverse social and ethnic backgrounds are also strongly recommended.
Knowledge of child abuse and neglect (mandated reporting) statutes preferred.
Must be fully vaccinated for COVID-19 virus, unless eligible for a medical or religious exemption.
Bilingual Spanish (preferred)

Smilow Ambulatory Multispecialty Department YSC (NP 1, NP4, NP8) This department is rooted in oncology nursing excellence and serves as a hub for patients undergoing cancer treatment and management for solid tumor cancers. The Bi-Lingual Outpatient Clinician is responsible for provide direct, quality clinical services to OPCC program youth, parents/guardians in the Bridgeport and designated satellite offices. Maintain timely and complete client records in accord with all department, licensing and regulatory requirements.
The Child & Family Guidance Center will provide a 5,000 sign on bonus to a verified bi-lingual candidate hired into this role.
The selected individual will be responsible for but not limited to the following Essential Functions:
Provide Intake assessments, Individual, Family and Group therapy.
Administer all applicable assessment tools at Intake, every 90 days and at discharge.
Provide Case and Crisis Management services to clients as needed.
Maintain all records in a timely manner (Intake, Treatment Plan, Treatment Plan Review, Discharge Summary, etc).
Maintain productivity required in contracts and grants, as indicated by all KPI standards.
Participate in weekly supervision, team meeting, and attend case conferences.
Participate in research activities of CFGC.
Provide emergency backup treatment (family, individual, and group therapy and case management) as needed.
Present at least two high risk cases per year to the CFGC Risk Management meeting.
Participate in various CFGC special interest committees/groups (ie. Trauma trained/informed care).
Provide Intakes for the Outpatient Department as needed.
Have a working knowledge of various EBPs (eg. TF-CBT) used by clinicians in the Outpatient Department.
Become certified in at least one EBP.
Demonstrate an increasing comprehension of the dynamics of human behavior and appropriate intervention techniques.
Collect and maintain program information to meet reporting and evaluation requirements.
Utilize and track Contingency Management rewards for clients to improve engagement

Qualifications:
The position requires a minimum of a Master's Degree in Counseling, Social Work, or Marriage & Family Therapy.

Good clinical skills to assess and treat child, adolescent and family mental health issues.

The Community Support Worker is responsible for partnering with families to identify strengths and needs of the child/family and advocate for appropriate support and services using the wraparound values and process. They will work with other providers to develop a Plan of Care that reflects the family's values and culture. This position is located within our Bridgeport office but may require working out of our Norwalk location as needed and traveling within lower Fairfield and New Haven County.
The selected individual will be responsible for but not limited to the following Essential Functions:
Provide services to about 11-13 families to identify strengths and needs.
Assists the family in developing and convening Child and Family Team Meetings (CFT) of traditional and non-traditional providers for the purpose of developing a Plan of Care (POC) that reflects the family's values and culture.
Works in conjunction with DCF to receive referrals and collaborates on transition plans for Community Support Services
Gather and submit data required by DCF in a timely manner
Maintains a Community Support Record on all families receiving Community Support Services in keeping with the wraparound process (family driven, strength-based, culturally responsive) and outcome focused documentation.
Provides case management services to families, coordinates child and family teaming activities assuring that families are connected to community resources.
Offers an array of services/supports to the families to enhance protective factors and collaborates with community resources and providers. That ensures a seamless and well coordinated system of care to support the family.
Develops a safety plan for each family served that guides the family to appropriate assistance. Ensures that service and safety plans are implemented appropriately and adapted by the CFT.
Assists in program evaluations such as baseline and outcome measures and client satisfaction surveys to families during service delivery or upon discharge from Community Support Services
Provides linkage and monitors coordination of supports and services, including follow-up and advocacy for children/youth and families.
Provides weekly home-based, in-person and/or phone contact with families as indicated.
Participates in monthly Community Support for Families staff meetings

Qualifications:
This position requires a Bachelor's Degree in Social Work, Sociology, Psychology or in the Human Service field.
A driver's license and reliable transportation are required.

Must be fully vaccinated for COVID-19 virus, unless eligible for a medical or religious exemption.
