Job Summary As a member of the Suicide Prevention team, the care coordinator will work with individuals who have been hospitalized for suicidal attempts or suicidal ideation and assess individuals needs, coordinate a plan for rapid follow-up after discharge from emergency departments and inpatient psychiatric facilities. Establish follow-up and care transition protocols to help ensure patient safety, especially among high risk adults in health or behavioral health care settings who have attempted suicide or experienced a suicidal crisis, including those with serious mental illnesses and/or substance use disorder(s). Responsibilities Completes initial assessments and provides ongoing assessments to include suicide assessments, substance abuse, living situation, support system, mental status, history, strengths and barriers, needs and resources, medical status, and medications. Outreaches to crisis stabilization units, inpatient psychiatric facilities, domestic violence shelters, and emergency departments to identify, engage, and enroll eligible recipients. Develops in conjunction with the individual served, family members, service providers and significant others, a care plan and/or treatment plan that utilizes individual strengths and addresses identified needs. Conducts suicide risks screenings on referred individuals. Provides comprehensive risk assessments and biopsychosocial assessments on eligible participants day of screenings. In partnership with individuals enrolled, creates a safety plan. Provides lethal means counseling to enrolled participants. Provides assertive contacts with enrolled participants based on suicide risks. Provide face to face contacts with recipients within 24 hours of missed appointments. Coordinate a transition of care plan including a process for a warm hand off for individuals who are ready to exit program. Ensure engagement in follow-up care. Facilitates stabilization of mental health symptoms through care coordination, assessment, and outreach. Monitors and documents progress or lack of progress for the individual. In collaboration with the individual, completes formal treatment plan reviews and/or care plan reviews as indicated. Advocates for acquisition of the services and resources necessary to implement the care plan and/or treatment plan. Completes referrals to community services and resources as needed. Advocates for persons served to ensure acquisition of necessary social services to include activities of daily living, including medical, dental, legal, housing, and entitlements and documents the effectiveness of the service and the person's satisfaction. Works collaboratively with individuals served to identify and support linkages to community resources and trainings: educational, vocational, social, cultural, mutual self- help groups, etc., that supports the person's chosen goals and interests. Provides side-by-side assistance to persons served in all aspects of daily living (i.e., assist with housekeeping, budgeting, grocery shopping, etc.). Provides recovery support services to increase an individual's social and interpersonal activities (i.e. supportive coaching, social skills development, side-by-side coaching, planning social/leisure time activities, trauma informed care). Assist individuals served to develop problem-solving skills so that they can respond to challenges that may arise in their recovery process. Promote the development of self-efficacy through role-modeling strength-based recovery. Qualifications In order to be considered candidates must have a high school diploma or equivalent and must have previous personal experience as a recipient of mental health services is required. Bachelor's degree preferred. Experience working with persons with severe and persistent mental illness preferred. Obtain, or work towards obtaining, certification as a peer specialist within one year of employment. Directs and coordinates the admission process and the treatment, rehabilitation, and support services of the program in coordination with the psychiatrist. Assigns 3- 5 staff for each Individual Treatment Team (ITT) and a primary case manager. Directs and coordinates the completion of the initial and comprehensive assessments and oversees the development of the individual's recovery plan. Provides clinical supervision of the development of the initial and comprehensive recovery plans and leads recovery planning meetings. Directs and coordinates the FACT team's services in coordination with the FACT Psychiatrist. Provides direct clinical treatment, rehabilitation, and support services to persons served by the program. Coordinates the daily organizational meetings to ensure the team's work of the day is scheduled and participants' identified needs are met and appropriate services are provided. Functions as a practicing clinician on the team. Participates in providing 24-hour on-call coverage for the FACT program. Provides clinical supervision to all FACT staff and provides clinical backup, crisis intervention, and consultation after regular business hours. Provides and ensures staff is trained in and carries out evidenced-based best practices (e.g. motivational interviewing, trauma-informed care, WRAP). Monitors for trauma, abuse, and neglect and uses appropriate reporting mechanisms. Reviews documentation to ensure the timely and appropriate treatment to the individuals served, which meets State requirements and MHRC Medical Records Standards. Qualifications In order to be considered candidates must have a Master's degree in Psychology, Counseling, Social Work, Psychiatric Rehabilitation, or human services related field. Three years of experience working with adults with severe and persistent mental illness (i.e. schizophrenia, bipolar disorder, borderline personality disorder) is required. One year of supervisory experience is required. Valid Florida License as LMHC, LMFT, or LCSW required. Experience with DBT therapy techniques preferred. Each member of the FACT team must meet MHRC/RBHS vehicle driver requirements, including a valid Florida Driver's License, automobile insurance coverage equal to or exceeding 50,000/100,000/50,000 split limits, and no more than eight points on their license for any combination of violations. The FACT Team Leader position requires individual and group interaction with persons who have a serious and persistent mental illness in their home environment, a closed office, hospital, nursing home, and/or locked unit. Most of a FACT Lite team member's day is spent in the community completing many of the above duties. Only about 25% of their time is spent in the office. Company vehicles are available, however members of the FACT Lite team also need to use their personal vehicles to provide services and are eligible for reimbursement for mileage. The FACT Lite team operates Monday through Friday from 8:00 am to 5:00pm. Team members rotate providing crisis on-call coverage after normal business hours, on the weekends and holidays. Job Summary The Activities Specialist helps to ensure quality programming to adults and children through the provision of structured recreational and socialization activities. The Activities Specialist designs groups, activities and events of an educational, therapeutic and social nature. The goal of the programming is to improve the skills of individuals receiving inpatient crisis stabilization services. The Weekend Activities Specialist completes documentation and shares information during shift report and team meetings. The crisis stabilization units provide emergency psychiatric services. Most patients are admitted involuntarily to the facility via the Baker Act, the mental health act of Florida. The normal length of stay on the unit for patients is typically 3-5 days. Due to the types of services provided, crisis stabilization units are a fast paced working environment and present employees with challenging and rewarding opportunities.
