An innovative-evidenced based program that enhances social communication skills while improving problem-solving and emotional regulation for improved flexibility and motivation in all settings is the focal point of our work. Schedule: 11-7 Monday-Thursday, 9-5 Friday (schedule may vary) Some responsibilities include: Reviews behavior plans, and any other documentation as a method of understanding the child and/or adolescent and presenting problem. Engages with client identifies individual, family and community strengths and resources. Provides specific therapeutic support, as prescribed in the client's treatment plan, including crisis intervention techniques, immediate behavioral reinforcements, emotional support, time-structuring activities, time-out strategies and psychosocial, rehabilitative activities. Provide administrative support for clinical programming. Supports creating and updating curriculum and lesson plans. Supports room set up for program daily. Monitors and supports client's daily progress on goals and objectives, as prescribed in the treatment plan. Supports program outcome data collection and analysis. Provides consistent communication and feedback to the family and other team members on client's progress. Is able to intervene and provide immediate assistance in crisis situations. Completes all documentation requirements within determined timeframes; completes all forms, consistent with CGRC corporate compliance procedures. Assist in interviewing families and obtaining social histories. Provide supportive back up to other Family Based team members. Act as a liaison linkage to inter agency and intra-agency personnel. Assist in coordinating referrals to other appropriate service providers. Assist the family problem solving to focus on their strengths and collateral needs and service plan to address their identified strengths and needs. Participate in screening, treatment planning and discharge process. Rotate 24-hour emergency service seven days per week. Attend and participate in appropriate conferences and training sessions as determined by the Program Supervisor and mandatory PCGC trainings. Participate in staffing with the Mental Health Professional regarding the progress of the treatment plan. Participate as co-therapist in all family therapy sessions with the Mental Health Professional. Establish a close, extended family like relationship with identified families as a unit. CREATE Therapist Conducts initial assessments utilizing program principals in order to assess client and family dynamics. Provides group therapy in accordance with evidence based curriculums with clients diagnosed with autism spectrum disorder. Facilitates family therapy to transfer skills to the home setting in order to address family system needs in the form of interventions. Providing a safe and structured environment that allows engagement in therapy. Develops effective and timely treatment goals with measurable objectives. Consults with all other professionals involved in service to the child, adolescent and family. Provides information/referral services to clients and families to ensure receipt of quality and appropriate services/resources. Utilizes treatment plans to develop, promote and track the course of therapy. Collect and track data for treatment goals. Meets program expectations regarding chart documentation and timeliness for documentation procedures. Handles all administrative duties such as chart documentation, billing requirements and other necessary paperwork consistent with licensure, accreditation, funding sources and CGRC technology reporting requirements. Completes all training and actively participates in required supervision in accordance with program and agency guidelines. Discharge planning for clients transitioning to other services. Provides therapy services to promote exploration and understanding of personal and interpersonal dynamics. Develops effective and timely treatment goals with measurable objectives. Facilitates agreement on goals and tasks of therapy; defines the role of each participant Consults with all other professionals involved in service to the child, adolescent and family. Provides information/referral services to clients and families to ensure receipt of quality and appropriate services/resources. Utilizes treatment plans and outcome measures to track and adjust the course of therapy. Meets program expectations regarding productivity. Handles all administrative duties such as chart documentation, billing requirements and other necessary paperwork consistent with licensure, accreditation, funding sources and CGRC technology reporting requirements. Completes all training and actively participates in required supervision in accordance with program and agency guidelines.
