A Brief Overview
Provides professional social work services for patients and families in assigned areas of the hospital/clinics, and on an as needed basis in other areas of the hospital/clinics. Enhances the psychosocial functioning of patients/families while under provisional supervision. Develops and maintains a positive and long-term credible relationship with patients, physicians and nursing staff to ensure a team based approach and effective continuum of care for the patient.
Essential Functions
Under Provisional Supervision, conducts psychosocial assessments: • Screens patients in assigned areas daily focusing on new diagnosis of chronic illness, patients with complex medical needs or safety concerns. • Establishes professional working relationships with staff/families. • Collaborates with and facilitates multidisciplinary team discussions to understand the medical needs of the patients/families. • Appropriately documents assessment in chart. • Formulates patient goals and monitors the progression of the plan of care utilizing strength based interventions. • Provides appropriate patient education and documents in the patient's EMR. • Communicates psychosocial assessment and plan of care with patient care team. • Assesses high risk families and provide intervention focusing on the safety and mental health of the patient/family and collaborates with the healthcare team to ensure a safe environment.
Provides referrals for patient and family needs. • Gathers information from patients/families. • Shares appropriate resource materials and information with patients/families. • Maintains up to date knowledge of community resources specific to the patient's medical condition and social situation. • Has an awareness of population health management tools and how to utilize information to impact patients/families well-being • Assists with overcoming social determinants of health. • Advocates for access to mental health, financial resources and other community resources/services. • Assists with follow-up and follow-through to ensure that quality of care is obtained. • Documents interventions in chart.
Under Provisional Supervision, provides crisis/supportive counseling and coaching to patients/families • Supports self-management by assisting families with problem-solving, adjustment to illness, and management of long-term illness, utilizing their positive coping strategies. • Provides interventions which reflect knowledge of how disease impacts social well-being and is enhanced by disease specific knowledge. • Provides crisis intervention in a variety of situations. • Documents interventions appropriately in chart. Provides recommendations and warm hand-offs to health care team members. • Facilitates support groups as needed.
While under supervision, provides assessment and coordination of child abuse/neglect cases. • Assesses psychosocial dynamics. • Communicates with medical and nursing personnel regarding assessment and plan. • Facilitates referral and provides ongoing communication with CPS, law enforcement, and county attorney's office. • Documents pertinent information and completes all reports in a timely fashion. • Prepares and presents cases to the Children's Advocacy Team. Attends team meetings on an as needed basis.
Provides on-call services for Social Work Department as required as a provisionally licensed Social Worker. • Assists with all Trauma Cases in the Emergency Department as a crucial team member. • Responds to calls in a timely manner. • Performs on-call duties in a professional manner.
Covers for Social Work colleagues in their absence, and arranges for coverage of social work responsibilities when absent. • Communicates with co-workers regarding coverage needs.
Provides consultation/interventions regarding legal issues such as determining guardianship and assisting with adoption cases. • Consults with Compliance Officer/Legal Counsel as necessary • Provides consultation related to State Wards in determining who has legal authority to consent to treat and release information
Based on departmental/organizational need may be required to participate in Transplant program functions.
The in-patient or SPC social workers may be required to participate in Transplant program functions, Social Work role includes: • Pre and Post-transplant psychosocial assessment of patients and families • Present psychosocial information to the selection committee • Provide transplant program patients and families specific resource information • Confirm resources and discharge plans with team and patients/families
Education Qualifications
Master's Degree from an accredited college or university in Social Work Required
Experience Qualifications

GENERAL PURPOSE

The supervisor must maintain a current valid California License from the California Board of Behavioral Sciences.

Supervision will consist of individual, triadic and group formats.
The Board requires supervisors of associate marriage and family therapists (AMFT) or trainees (MFTT) to complete a minimum of six (6) hours of supervision training or coursework within two (2) years immediately preceding supervision (or within 60 days of commencement of supervision) and every renewal period thereafter.
The Board requires supervisors of ASW's to complete a minimum of 15 contact hours in supervision training prior to the commencement of supervision.
Both the 6-hour and 15-hour courses must be taken through an accredited or approved school, a state agency, or an approved Continuing Education Provider. A course taken through an on-line approved provider will also be accepted. Supervisors are to retain documentation verifying completion of this requirement and must provide it upon written request.
NOTE: ASWs are required to obtain 1,700 hours of the total 3,000 of post-master's degree experience under a LCSW to qualify for licensure.
NOTE: Out of the 52 weeks of individual supervision, ASWs are required to be supervised by an LCSW for 13 of those 52 weeks.
NOTE: A trainee is permitted to earn a maximum of 1,300 hours of experience overall prior to the degree being awarded. Up to 750 hours of counseling and supervision are permitted within the 1,300 hour maximum.
NOTE: All associates are required to have individual or triadic supervision for at least 52 weeks.
Schedule: Monday - Friday 8:30 am - 5:00 pm. From 8 to 20 hours per week.

Qualifications:

POSITION SUMMARY:
Holt International finds and supports permanent, loving families for children who are orphaned, abandoned or at serious risk of separation from their family. The master's level social worker represents Holt in their community and ensures successful adjustment of child and family by conducting an assessment of the family, and providing appropriate services throughout the adoption process. The master's level social worker works cooperatively with families, the Branch Office and Adoption Teams headquartered in Eugene, Oregon. Note: The Adoption Social Worker is primarily a home-based position. The Branch Social Worker position is required to travel (most often by car) to different locations within the assigned area to complete the essential functions of the position.
ESSENTIAL JOB FUNCTIONS (to be completed within timeframes outlined by the Branch Director):
1. Determines a family's ability and capacity to parent an adopted child by conducting pre-home study meetings as requested, reviewing and assessing agency-requested information, evaluating the family's knowledge and perceptions of adoption, teaching adoption education courses as needed, and conducting psycho-social assessments of family members.
2. Contributes to the delivery of high-quality adoption services by producing adoption home studies and post placement reports that comply with provided guidelines and meet current agency, state, country and USCIS requirements.
3. Provides support and case management of all assigned families by maintaining quarterly contacts throughout process, communicating information to the family, referring family to appropriate community support systems when necessary and documenting all correspondence in Holt's database.
4. Prepares family for travel and assists with post placement adjustment by providing training and identifying resources available to help meet the child's needs and aid in the child's adjustment to the family. Conduct home visits as required by state law and country program guidelines, assist in early identification of problematic issues, and make a determination regarding the stability and appropriateness of the placement.
5. Assures child's physical and emotional well-being and eases family's adjustment by providing counseling to the child and family and/or helping arrange for counseling with a clinician outside the agency if needed. In case of an adoption disruption or dissolution, maintain relationship with family to ensure transition plan is developed that meets the child's needs. Assist child and family with transition of child to new placement.
6. Contributes to public knowledge about Holt and supports Holt programs by participating in activities arranged by Holt including picnics or parent groups, representing Holt at adoption meetings, coalitions and organizations, and performing other outreach activities as appropriate.
7. Maintains professional knowledge by attending relevant seminars and course work, keeping informed about State regulations, legislation, adoption issues and child-related issues overseas; maintaining license, if required.
8. Contributes to a positive, productive work environment by meeting attendance and punctuality guidelines and pre-arranging time off with appropriate notice; treating all people with dignity and respect; and exhibiting good listening and comprehension skills including giving and welcoming feedback.

Duties and Responsibilities:
Conduct home visits to provide direct support, case management, and crisis intervention for individuals transitioning from homelessness to stable housing.
Develop and implement individualized service plans to help clients maintain housing, access healthcare, employment, and social services.
Assist clients in developing life skills, including financial management, healthcare navigation, and self -sufficiency strategies.
Provide clinical assessments and referrals for behavioral health, substance use, and medical services as needed.
Advocate for clients by coordinating with community agencies, social service providers, and healthcare professionals.
Maintain accurate and timely documentation of client progress, interactions, and service plans in compliance with program guidelines.
Facilitate group sessions and workshops focused on independent living, coping strategies, and mental health support.
Ensure adherence to trauma -informed care and person -centered approaches when engaging with clients.
Participate in multidisciplinary team meetings, case conferences, and training sessions to ensure coordinated care.

Qualifications, Knowledge, Skills, and Abilities: Must hold a valid Social Worker, LPC, LMSW, or LCSW license. Minimum of two (2) years of experience working with homeless populations, behavioral health, or social services. The ideal candidate must have experience working with homeless populations, and be comfortable conducting home visits.
Experience providing case management, crisis intervention, and behavioral health assessments.
Strong understanding of the homeless service system, housing programs, and community resources.
The ability to build trusting relationships with vulnerable individuals and navigate complex social service systems.
Excellent communication, interpersonal, and crisis management skills.
Ability to work independently and collaboratively in a fast -paced environment.

About the Role
The Social Work Intern Manager is a key member of the Neighbor Care Team at Community First! Village. This role is responsible for leading, mentoring, and supervising a team of social work interns who walk alongside our formerly homeless neighbors. The Social Work Intern Manager ensures interns are equipped to provide direct care, emotional support, and resource connections while creating a meaningful and values-aligned learning experience.
