Key Responsibilities:
* Providing clinicalsocial work services to assist patients and families with personal, family, medical and environmental difficulties.
* Performing psychosocial and risk assessments, assigning diagnoses and developing and implementing interventions and treatment plans.
* Participating in planning and implementation of discharge and outpatient service plans and collaborating with health care team and community agencies to plan, implementing and following up on treatment and provision of needed services.
Minimum Qualifications
Education:
* Master's degree in social work
Experience:
* Strong clinical skills and experience with various therapeutic modalities.

Job Summary

The Massachusetts General Hospital seeks a dynamic Licensed ClinicalSocialWorker (LCSW)/Licensed Independent ClinicalSocialWorker (LICSW) to serve on the Special Operations Team. In this role, the LCSW/LICSW will provide and oversee the provision of psychiatric, psychosocial, and overall mental health services and referrals for military Special Operators into the Home Base program. They will also be dedicated to supporting the Special Operations Forces (SOF) program and will provide coverage to other clinical operations as needed. The LCSW/LICSW may help cover approximately 2-3 weekend shifts per year.

* Provides psychosocial assessments of patients and families with social, emotional, interpersonal, and/or environmental issues. Formulates biopsychosocial assessment, disposition, and treatment plans.
* Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
* Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
* Coordinate care for Special Operations Forces (SOF) veterans and military-connected family members seeking care at Home Base with the appropriate internal or external clinicians in a multi-disciplinary team, coordinated care model.
* Serve as a contact point for accepting and triaging SOF referrals from internal clinicians, outside clinicians and self-referred patients.
* Serve as primary case manager for patients in SOF admissions process in collaboration with Admissions Coordinator, including, but not limited to, the following responsibilities: conduct pre-screening to assess treatment and evaluation needs for SOF patients referred; coordinate signing of Release of Information forms, acquire relevant medical records, and coordinate with SOCOM where applicable; serve as point of contact for ongoing questions from the patient as they arise.
* Monitor patient's status in admissions process using appropriate documentation.
* May be asked to collaborate with SOF/ComBHaT team, patients, clinical team, and Resource Specialist to identify clinical and psychosocial needs in patients' home communities and identify appropriate resources.
* May provide direct clinical services to Veterans and/or family members, which may include group or individual therapy.
* May provide psychoeducation regarding PTSD, TBI, and related conditions to Veteran patients and family members as clinically appropriate within the context of the Intake and triage duties.
* Collaborate with an inter-disciplinary team of clinical staff (socialworkers, psychologists, nurses, psychiatrists, physiatrists, physical therapists, etc.) to coordinate patient care and plan for discharge/aftercare when needed.
* Consult with team to facilitate mandated assessments when abuse is suspected (child, disabled adult, elder), and safety assessment when violence is reported. Collaborates with MGH resources (HAVEN and Child Protection Team).
* May assist with crisis intervention and management.
* Documents timely and relevant information in patient electronic medical record and Home Base database.
* Provide coverage for social work responsibilities in Outpatient Clinic as needed.
* For LICSWs: provide clinical supervision to LC-level and/or MSW students as needed.
* Additional responsibilities as assigned.

The role involves providing psychoeducation, mental health support, crisis intervention, counseling, group facilitation, and resource connection to address the wide range of challenges faced by AYA patients.
The ideal candidate will have experience working with adolescents and young adults in an oncology or related medical setting and be skilled in crisis intervention, short-term counseling, bereavement support, trauma, abuse, neglect, and substance use concerns. This is a multifaceted role supporting holistic patient care by integrating psychosocial services throughout the treatment journey.

Job Summary
The Massachusetts General Hospital seeks a full-time, licensed socialworker (LCSW or LICSW) for a one-year training program in Addiction Medicine starting in June 2026. The Integrated Substance Use Disorder Training Program (ISTP) is a yearlong fellowship that aims to train nurse practitioners, psychologists, and socialworkers in team-based addiction care, especially in community-based settings.
The SocialWorker Fellow will work with other ISTP trainees to provide inter-disciplinary care for people with substance use disorders in a community-based health center setting. The training program will provide clinical experience in a variety of placements including an inpatient addiction consult service, a low-threshold walk-in clinic, and outpatient dual-diagnosis programs for adolescents and adults. Training and mentorship will be provided by experienced addiction practitioners who are faculty of this program.

As a member of the multidisciplinary oncology team, and under moderate supervision, the ClinicalSocialWorker, LCSW Program Specialist, for the Universal Distress Screening Program independently provides patient and family centered care through clinical psychosocial assessment and intervention, including crisis response, individual, couples, family, and group interventions, and short-term bereavement support. The ClinicalSocialWorker, LCSW Program Specialist is responsible to innovate, develop, and implement projects, conferences, trainings, educational resources, groups, or other forums that address goals and expectations of the program stakeholders. The ClinicalSocialWorker, Program Specialist develops and implements project plans to address patient-level and systems-level psychosocial care gaps and has a leadership role in program meetings with commitment to continuous quality improvement. The ClinicalSocialWorker, LCSW Program Specialist also leads ongoing programmatic evaluation and refinement.
This is an excellent position for a socialworker with a background in program implementation and experience in working with adults with serious illness. This role sits within the Division of Social Work at the Dana-Farber Cancer Institute in the Department of Supportive Oncology. This role is an exciting opportunity to provide ambulatory clinical oncology social work services and lead the Institute-wide Universal Distress Screening Program (ambulatory and future inpatient settings) in collaboration with a multi-disciplinary screening team. The Screening Specialist is responsible for distress screening practice and operations, clinical integration, patient educational materials, referral and outreach workflows, data management, research collaboration, and program development to improve the patient and family experience throughout the process of cancer care. The socialworker will provide in-person and telehealth services to patients and families (i.e., phone/Zoom video). Modalities include individual, family, and group work. Candidate must be able to thrive in a fast paced, complex, academic medical environment where the socialworker is a key, valued member of the multidisciplinary medical team.
This is a full-time, 40 hour per week position. As a hybrid role the schedule is 2 days on-site coverage and 3 days remote at the DFCI Chestnut Hill location. DFCI guidelines state that employees must reside in New England: Connecticut, Maine, Massachusetts, New Hampshire, Rhode Island, or Vermont.

Primary Duties and Responsibilities:
1. Provides biopsychosocial assessment and intervention, crisis intervention, coordination of psychosocial services, and consultation, to reduce cancer-related distress and improve quality of life for patients, families and caregivers across the trajectory of cancer care; this includes those patients seen for screening for high risk of developing cancer and/or a diagnosis of cancer through to end of life and/or survivorship in collaboration with community mental health partners.
2. Assesses adaptation of families and caregivers to patient's illness and ensures maximum participation of patients/families/caregivers in the continuum of care process.
3. Provides clinical interventions that are focused on cancer-related distress, are evidence based, goal oriented, strengths based, and short-term and episodic in nature to ensure patient equity and accessibility to social work interventions; modalities include individual, family, and group work.
4. Identifies and addresses high-risk psychosocial factors as well as psychosocial strengths of patients/families/caregivers that impact health status, coping and adjustment (e.g., suicidal, or homicidal risk, domestic violence, disabled, child or elder abuse and neglect, physical, developmental, or intellectual disabilities, serious mental health conditions, etc.)
5. Responsible for facilitating complex serious illness and goals of care conversations to ensure shared treatment decision-making and effective patient-physician/health care team communication.
6. Responsible for identifying and communicating systemic inequities, their impact on patients, families, caregivers, and the community at large. and providing recommendations for systemic change.
7. Responsible for demonstrating commitment to diversity and equity efforts, including advocacy and self-education to advance knowledge base of the needs and concerns of underserved and marginalized populations.
8. Communicates clinical assessment and recommendations and provides consultation and support to interdisciplinary health care team.
9. Initiates collaboration with interdisciplinary team to manage psychosocial barriers that limit patients/families'/caregivers' ability to access and/or engage in recommended medical care.
10. Provides education/consultation regarding impact of cancer on patients and families/caregivers to community, and provides consultation, education, and program development (e.g., support groups, psychoeducational forums, etc.) within the disease-based or service-based team and in collaboration with the interdisciplinary team.
11. Co-facilitates evidence-based patient and/or family and/or caregiver support programs or groups.
12. Provides in-person and/or telehealth services to patients and families/caregivers; method of communication (i.e., in person/phone/Zoom video) is based on clinical assessment.
13. Provides clinical coverage in other socialworkers' absence(s).
Program Specialist: In collaboration with the Programmatic Manager:
1. Develops and implements program components (ex: projects, conferences, trainings, educational resources, groups, or other forums) that address goals and expectations of the program stakeholders.
2. Engages interdisciplinary stakeholders in program planning and development using the values, benefits, outcomes model of engagement.
3. Engages in ongoing programmatic evaluation and continuous quality improvement with internal and external participants and stakeholders.
4. Conducts exploratory reviews, benchmarking with peer institutions to support program development and implementation.
5. Develops project plans that includes micro-, mezzo-, and macro-level interventions.
6. Addresses and implements response to health care disparities in the program

The Behavioral Health Home Hospital (BHH) Research SocialWorker/Case Manager supports an innovative care delivery model offering specialized psychiatric-care services in patients' homes as an alternative to inpatient psychiatric hospital admission.
Provides quality clinical services to patients in various community settings, delivering hospital/inpatient-level treatment.
Collaborates with professionals to address psychosocial, environmental, age-specific, and cultural issues to maximize well-being and effective use of healthcare and community resources.
Utilizes clinicalsocial work techniques for consultation, assessment, treatment interventions, and advocacy.
Sees patients in person at their homes or remotely, managing up to 4 patients at a time.
Ideal for someone interested in a temporary position working on a research trial to broaden treatment opportunities for patients with active acute psychiatric conditions.

Must hold a professional license and/or certification as required for the position.

Must provide 2 recent references, including at least 1 supervisor or above in skillset.
