Minimum Qualifications
Education:

Experience:

* Ability to work in collaboration with orthopedic surgeons, nurses, Advanced Practice Clinicians (APC) and ambulatory staff is essential.
* Analytical skills to resolve highly complex problems requiring the application of clinical, scientific or technical principles, theories and concepts and in-depth, cross-functional experience-based knowledge.

Reporting to the Clinical Manager APP Cardiac Surgery Clinical Inpatient as well as the Chief of Cardiovascular Surgery, this position is responsible for participating in all aspects of pre and postoperative care of the cardiac surgical patient. The mission is to facilitate such care in conjunction with the surgical house staff, and under the supervision of the staff surgeon. This position requires a license as a Nurse Practitioner or Physician's Assistant in the Commonwealth of Massachusetts.
Scope: The primary responsibilities of the NP/PA are in relation to the care of patients in the inpatient setting. The responsibilities include
The NP/PA works under the direction of the surgeon, and duties consist of pre and postoperative care of the surgical patient
The NP/PA is expected to perform clinical tasks on the patient unit: Placement and removal of Central lines, chest tubes, sutures, and temporary pacing wires as directed by the surgeons, and chief resident.
The NP/PA will discharge patients (complete discharge paperwork, discharge summary, communicate with cardiologist/PCP when appropriate).
The NP/PA will organize patient information and participate in patient case conferences both pre and post-operatively.
Job Description:
Job Summary: Participates in all aspects of pre and postoperative care of the cardiac surgical patient. This position will interface with various medical staff attending in providing care/coverage of the patients.
Essential Responsibilities:
Performs and documents preoperative evaluation (history and physical examinations). Orders and interprets diagnostic tests.
Examines and evaluates pre and postoperative patients in both inpatient and outpatient settings. Writes orders for medications and treatments. Performs invasive procedures such as: placement of arterial and central venous lines, insertion of chest tubes, intubations of patients, removal of intra-aortic balloon pump catheters.
Collaborates with staff surgeon and house staff in educating the patients and their families in medical care. Works with staff in providing patient care in the following locations: the intensive care units (CCU, CVICU, MICU), Cardiac Surgery Stepdown Units, PCU, Cardiology Units, and staff cardiac surgeon's clinical practice area.
Inserts and removes of chest tubes (thoracostomy), central line placements, arterial lines and Swan ganz floats. Troubleshoots intra-aortic balloon pump catheters. Removes intra-aortic balloon pump catheters and temporary pacing wires. Initiates blood products for hemodynamically unstable patients.
Required Qualifications:
Master's degree required.
License Nurse Practitioner License required., or Registration PhysicianAssistant License required., and Certificate 1 Basic Life Support required., and Certificate 2 Advanced Cardiac Life Support required.
3-5 years related work experience required.
NP/PA licensed in the Commonwealth of Massachusetts.
Three to five years experience as a NP/PA with part of that time dealing with highly acute patients on a surgical service.

Responsibilities include three full days of outpatient clinic focused on pre- and post-transplant care, as well as one and a half days dedicated to administrative responsibilities, care coordination, and interdisciplinary collaboration.
The PA plays a central role in coordinating care across the transplant continuum, addressing the medical, psychological, and social needs of patients and their families in the outpatient setting. Strong interpersonal, organizational, and communication skills are essential, as the role involves patient education, multidisciplinary coordination, and communication with referring providers. The APP is expected to maintain current knowledge in transplant nephrology through ongoing review of the literature and continued professional development, and to practice in alignment with the mission, vision, and core values of Brigham and Women's Hospital as a collaborative member of the Nephrology Division and the broader APP community.
As a member of the BWH Nephrology Division, the PA will be responsible for the following duties:
Clinical Care
* Provide outpatient clinical care during 3 full days clinic sessions per week in the kidney transplant clinics.
* Conduct comprehensive evaluations of patients with CKD and ESRD referred for kidney transplant evaluation, including medical history review, physical examination, diagnostic data interpretation, and patient counseling.
* Manage longitudinal outpatient follow-up of kidney transplant recipients including routine visits and problem-focused assessments.
* Collaborate with transplant nephrology attendings in the development and implementation of individualized care plans.
Transplant-Specific Management
* Monitor allograft function, laboratory values, and clinical trends in post-transplant patients.
* Assist in the management and adjustment of immunosuppressive medications according to established protocols and under physician supervision.
* Identify, evaluate, and help manage transplant-related complications, including infection, rejection, hypertension, metabolic abnormalities, and medication toxicities.
Care Coordination
* Coordinate care with transplant surgeons, transplant coordinators, pharmacists, nursing staff, infusion services, and other subspecialists.
* Arrange and facilitate infusion center treatments for kidney transplant recipients when clinically indicated.
* Ensure timely follow-up of test results, referrals, and treatment plans.
Patient Communication and Education

1) Perioperatively
Prior to the operation, the PA serves to assist with a wide variety of preparations including but not limited to:
a. Assist with placing appropriate perioperative order sets and required pre-op checklist.
b. Verify correct patient information, operative procedure and surgical site and check the patient's chart for any abnormalities
c. Verify that the correct equipment and instrumentation is prepared and ready to use.
d. Carry out the surgeon's request for exact positioning for surgical procedure.
e. Assist with patient positioning, including helping transfer of patients from stretcher/bed to OR table, ensuring that the bed is locked and secure during transfer, and application of patient safety measures, i.e., safety strap application, protective padding and arm board placement
f. Assists circulator and anesthesia provider in preparation of patient to include but not limited to foley catheter placement, tourniquet application, placement of IVs, cricoid pressure application, sterile prep and drape, and other procedures as needed.

2) Intraoperatively:
During the operation, at certain points of the procedure the PA may be scrubbed at the bedside with the attending surgeon. During much of the procedure, the PA may be scrubbed alone at the bedside as the attending surgeon performs the procedure on the robotic console. Roles include the following: a. Make incisions and place ports at discretion of attending surgeon
b. Visualization of the operative site by digitally manipulating tissue, suctioning, irrigating, or placing sponges
c. Manipulating suture materials
d. Placement of surgical clips and/or utilization of the stapler as directed by the surgeon
e. Change robotic instruments (remove and reintroduce instruments)
f. Administering subcutaneous postoperative injections of local anesthetics when directed by the surgeon
3) Postoperatively
a. Closing skin incisions, secure drains, and apply wound dressings
b. Handles all surgical specimens according to procedure, assist with specimen reconciliation as needed
c. Transfer patient to stretcher
d. Accompany patient to recovery room to assist with report to staff in post-anesthesia care unit (PACU)
e. Ensures the appropriate post-op order sets are in place, communicates post-op plan with team and ensures the ambulatory follow-up is scheduled correctly.
4.) Professional Development:
This individual must become familiar with mechanics of the DaVinci system including details of port placement, camera targeting, robotic docking, emergency undocking and troubleshooting procedures
In addition:
a. Works with Lead Surgeons, other PAs, and OR leadership to help grow and develop the Robotic Surgery and Robotic PA programs
b. Trains bedside personnel (residents, fellows, and other PAs) in robotic procedures
c. Keeps current regarding developments in technical instrumentation, trends and techniques through internal and external programs, particularly regarding the DaVinci Surgical System.

The Diabetes PA is an advanced practice practitioner who has a specialized knowledge and skills in diabetes through completion of a nurse practitioner program, completion of a diabetes certificate program and/or work experience in the care of patients with diabetes. The practitioner has the credentials to function in an expanded clinical role and is responsible for the daily management of diabetes patients. Job Summary
The physicianassistant (PA) is a licensed provider who has acquired a broad range of knowledge and skills through the successful completion of a physicianassistant program. Boston Children's Hospital is seeking a Nurse Practitioner (NP) or PhysicianAssistant (PA) to play a crucial role in both our Neonatal Consult Service and Surgical Service within the NICU. The NP both provides advanced clinical care to critically ill neonates across the hospital and contributes significantly to our 30-bed tertiary care Level III/IV NICU.
This role encompasses expert consultation to infants throughout multiple intensive care units and pediatric specialty services. Collaborating with neonatologists, pediatric surgeons, and multidisciplinary teams in the NICU, the NP manages complex pre-operative and post-operative surgical patients and medically fragile neonates from extreme prematurity through six months corrected age and beyond. In addition, the NP is pivotal in the Neonatal Consult Service, offering comprehensive assessments, diagnoses, and management of acute and chronic neonatal conditions, including cardio-respiratory failure, nutritional support, pain and sedation management, gastrointestinal disorders, and congenital or genetic anomalies. They serve as a key consultant across the institution, advising on individualized care plans for infants with varied diagnoses and treatment needs.
Key Responsibilities:
* Facilitating the Neonatal Consult Service: Acting as a primary consultant within the hospital, guiding care plans and treatment strategies for infants across multiple ICUs and specialty units.
* Managing Care in NICU's Surgical Service: Overseeing critically ill neonates on the surgical service, including pre-operative and post-operative management in collaboration with medical and surgical teams.
* Providing Specialized ICU Care: Delivering expert care including pain and sedation management, cardio-respiratory failure treatment, nutritional support, and management of complex medical and surgical diagnoses in infants ranging from premature to 6 months after their due date.
* Institution-wide Consulting: Extending consultation services to infants in other ICUs (Cardiac, Medical-Surgical, and Medical) and pediatric specialty and sub-specialty units. Collaborating with the Neonatology Consultation Team on comprehensive management across diverse diagnoses and treatment modalities.
* Participating in Multidisciplinary Activities: Engaging in interdisciplinary rounds, care planning, quality improvement initiatives, and education of trainees and staff.
* Working 40-hour workweek with start times as early as 6:00am with rotating responsibilities within the NICU's surgical service and throughout the hospital with the consultation service, demanding a high level of clinical expertise, critical thinking, and collaborative practice within a fast-paced academic medical center.
Minimum Qualifications
Education:
* Master of Science in Nursing or Masters in PhysicianAssistant Studies required.
Experience:
* Minimum 1 year RN, NP, or PA experience in a Level III NICU or in a critical care setting.
* 2-3 years of experience in NICU, Cardiology, or consultation preferred.
* Surgical or cardiology advanced knowledge and experience preferred
Licensure/ Certifications:
* A current Massachusetts NP or PA license required
* Neonatal NP or Certified Acute Care Pediatric NP strongly preferred
* Sign-on bonus of $5,000 (for people who have not worked at BCH in the past 2 years.)

The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting.
