The Senior Patient Experience Representative in the Department of Dentistry plays a critical role in ensuring timely access to care and an exceptional patient and family experience. This position manages incoming referrals, supports accurate specialty triage, and oversees hygiene scheduling to optimize clinic flow and access.
Key Responsibilities

* Collaborate with interdisciplinary care teams (ICT)-including LTSC, GSSC, primary care providers, specialists, and behavioral health treatment facilities-to support program enhancements, process improvements, care coordination, discharge planning, and aftercare.
* Participate actively in ICT meetings, contributing insights on behavioral health concerns, treatment options, care strategies, and engaging in collaborative decision-making regarding enrollees' care.
* Receive and triage behavioral health and substance use referrals, coordinating with the ICT and primary care providers to refer enrollees to appropriate inpatient or outpatient services.
* Develop, update, and implement enrollee-centered care plans in collaboration with enrollees and care teams, promoting self-care, shared decision-making, and addressing identified needs through routine engagement and proactive outreach.
* Conduct outreach, assessments, and home visits via telephonic, electronic, or in-person contact for identified enrollees, including routine evaluations and post-transition care.
* Monitor clinical status to identify early signs of deterioration, intervening proactively with the ICT to prevent unnecessary hospitalizations and escalate behavioral health needs during weekly case conferences.
* Establish consistent communication and reporting schedules with providers and enrollees to review enrollee status, progress toward goals, care needs, utilization plans, and follow-up.
* Ensure compliance with DSNP regulations and quality assurance standards by documenting, monitoring, and evaluating enrollee interventions and care plans accurately within electronic medical record systems.

We are seeking a part time, 20-hour weekday Scheduler. The hours can be either 3:30pm - 7:30pm or 4pm - 8pm. This is a remote position but must live in Massachusetts.
Job Summary
Under direction, receives incoming communication and online requests from both patients and referring providers interested in accessing care. Performs patient registration, and scheduling processes. Ensures that patients, referring providers and other customers receive timely, efficient and high-quality service. This is a telephone/computer-based position; there is no face-to-face interaction with patients or other customers.
Essential Functions
Functions as the face of enterprise to initiate and coordinate registration and scheduling for new patient appointments.
* Documents all patient demographic, medical, financial, and referral information into the computer in strict compliance with established department procedures for the purpose of getting a hospital registration number and scheduling of an appointment.
* Answers telephone, triages calls, takes accurate and detailed messages using professional and courteous customer service techniques.
* Provides basic information and instructions to patients regarding the practice and Hospitals across the enterprise.
* Obtains all patient information required by the providers prior to an appointment. Ensures that the medical records are available to all health care providers.
* Completely and accurately transcribes call, referral and scheduling information in multiple systems.
* Provides excellent customer service to both internal and external customers of the Mass General Brigham Hospitals and ambulatory practices; supports ongoing communication and provides service recovery when needed with patients and referring physicians.
* Maintains confidentiality and privacy consistent with HIPAA guidelines.

Under the general direction of the Director of Risk Capture, the Pre-Visit Clinical Review Specialist (CRS) facilitates the accurate and appropriate identification of patient medical conditions through comprehensive chart review combined with review of coding output data sources (internal and external claims) that results in improvement in the overall quality, completeness and accuracy of problem lists, visit documentation and disease registry assignments. The CRS utilizes both clinical and coding knowledge of Hierarchical Condition Categories (HCCs) to inform accurate and appropriate diagnosis considerations for suspect condition identification and recapture opportunities. This role serves to educate providers and the clinical care team on all aspects of risk capture and linkages with quality.

Principle Duties:
Drive Clinical Delivery
* Performs accurate and timely pre-visit review of selected ambulatory encounters to identify opportunities to recapture medical conditions that meet criteria as HCC diagnoses and to capture new, suspected HCC conditions.
* Accurately interprets clinical information in the medical record, evaluating clinical indicators to identify potential diagnoses
* Presents clear HCC Consideration Communication to provider and educates providers to obtain greatest possible diagnostic specificity to accurately reflect the patient's condition(s)
Identify Education Opportunities
* Identifies themes through chart review that might present education opportunities for individual or groups of providers
* Gathers feedback from periodic post-visit chart reviews and incorporates these learnings into educational opportunities with providers
* Identifies opportunities for Process Improvement and Quality Improvement, as needed
Foster collaborative relationships across the enterprise

Qualifications:

* 2 years' experience in Primary Care, medical coding, risk adjustment or CDI preferred

Job Summary
PRINCIPAL DUTIES AND RESPONSIBILITIES:
All duties will be performed under general supervision by the Clinical Research Manager.
The following clinical duties will be performed:
* Interfacing in a professional manner with a diverse team of physicians, clinical research assistants, research nurses, pathologists and patients
* Coordinating the collection and processing of tissue samples and blood samples from patients
* Consent patients to translational research studies (minimal risk specimen collection studies)
* Explain study protocol to patients and obtain informed consent
* Screen and follow patients relevant to specific projects/studies
* Assist clinical team in screening potential patients for study participation
* Schedule all protocol required tests and procedures
* Consent patients to specimen collection protocols

* Enroll patients as required by the study sponsor and internal enrollment monitor team
* Follow patients for the duration of study participation via electronic medical record review to ensure protocol compliance

* Maintain research charts and/or electronic files for all enrolled patients
* Ensure adequate source documentation is in place for all data reported
* Resolve data queries issued by the sponsor

Job Summary
The MGBMG Pharmacy Department aims to expand clinical pharmacy services across the Mass General Brigham Medical Group (MGBMG) locations in a way that enhances the patient experience, health outcomes, and quality. One of the key goals is ensuring that our patients with chronic diseases are supported in achieving clinical outcomes known to reduce morbidity and mortality from these conditions. Pharmacists can play a key role in this goal by offering higher touch services (medication prescribing, monitoring, education) to patients that are struggling with chronic disease management. Working with primary care providers, population health coordinators, high-risk nursing programs, and community health workers, the pharmacist has the potential to positively impact patients through virtual, in-person appointments, and telemedicine.
In collaboration with the MGBMG team, the Ambulatory Clinical Pharmacist will monitor the medication use process in accordance with the laws and regulations governing credentialed pharmacists and the practice of pharmacy by the appropriate state and federal agencies. They will work relatively independently, and within the limits of established pharmacy/ambulatory care practice site policies and procedures, as a member of the patient care team. The Ambulatory Clinical Pharmacist will report to the Clinical Pharmacy Manager at MGBMG and be responsible for working at the top of their licensure in collaborative drug therapy management (CDTM) work, as well as in-service presentations, clinical consult work, collaboration with our pharmacy team, and development of work to assist in the expansion of our department.

Opening / closing shift as needed. Hours between 6:30 am - 3:00 pm and 8:30 am - 5:00 pm. Hybrid role, working remote 1 - 2 times per month Supervises and leads daily administrative operations of a clinical service.
Key Responsibilities:
* Supervises support staff and oversees clinical administrative operations. Plans, prioritizes, and delegates work; monitors performance and provides feedback. Implements and maintains office systems, policies, and procedures. Trains new staff and communicates updates to hospital and departmental policies.
* Monitors clinic activity to support a positive patient experience. Assists staff with customer service and scheduling issues. Addresses patient concerns and escalated service needs.
* Manages electronic and paper records, including sensitive administrative, financial, and employee data. Oversees data entry, ensures accuracy, and generates reports. Recommends and implements system improvements. Serves as clinical floor Super User and participates in Joint Commission/Environment of Care rounds.
* Collects and analyzes data; prepares spreadsheets, charts, and summaries for supervisor review. Performs related administrative tasks as needed.
* Oversees daily department operations, including payroll and compliance reporting. Tracks personnel actions and initiates required forms and documentation. Routes and monitors administrative forms, including purchase orders and payments.
* Manages supply inventory and computer equipment. Purchases standard items and coordinates repairs within budget. Recommends non-routine purchases and liaises with support departments to ensure service needs are met.
Minimum Qualifications
Education:
* Associate's degree or equivalent experience required.
* Bachelor's degree preferred.
Experience:
* Two years of related work experience required.
* Additional years of related work experience may substitute for education requirement.
The posted pay range is Boston Children's reasonable and good-faith expectation for this pay at the time of posting.
