Responsibilities:

3. Oversight of plans of treatments, telephone orders, and discharge summaries
4. Perform admissions and recertification with accuracy, completeness, and compliance of federal and state guidelines.
5. Professional decision making on the service frequency and duration to provide for the requirements of the Plans of Treatments.
6. Obtain authorizations for Home Health services from insurance company.
7. Attend quarterly Utilization Review meetings as designated by the Director of Professional Services.
8. Assess patient clinical status in home 48 hours after hospital discharge and on a 60 days basis per company policy.
9. Review/approve plan of care and all related documents prior to physician review/signing.
10. Provide patient care as needed/requested by Administrator and/or Director of Nursing.
11. Schedule of in home Reassessments with families, Complete Supervisory reports on nursing staff upon visits.
Qualifications/Minimum Job Requirements:
Current state license as an RN, BSN preferred
Current CPR
At least 2 years experience as a Registered Nurse
At least 1 year experience as a Clinical Supervisor in Home Health Care preferred
At least 1 year of Pediatric experience
Current state driver's license and automobile insurance
Proficient in computers/EMR
Highly motivated, self-disciplined, have excellent communication skills and willing to travel to patient homes
Utilization Management Experience
Reliable transportation for patient visits
Reside in the Amarillo area

THE POSITION:
We are looking for a full time Supporting Documentation Specialist to join our exceptional customer service team who is committed to helping our patients thrive. THE QUALIFICATIONS:

We are seeking an experienced Clinical Supervisor in the Amarillo area. The Clinical Supervisor will be directly responsible for clinical activities of the office staff and will be part of a team that plans, implements and evaluates our patients' plans of care.
*25-30% of time will be spent doing home visits to patients. Candidate must reside in the Amarillo area.
Monday-Friday 8a-4p in Office
Responsibilities:
1. Consult with physicians and members of the multidisciplinary health team to coordinate patient care, when necessary.
2. Verify that documentation is consistent with federal and state guidelines and in compliance with physician's orders.
3. Oversight of plans of treatments, telephone orders, and discharge summaries
4. Perform admissions and recertification with accuracy, completeness, and compliance of federal and state guidelines.
5. Professional decision making on the service frequency and duration to provide for the requirements of the Plans of Treatments.
6. Obtain authorizations for Home Health services from insurance company.
7. Attend quarterly Utilization Review meetings as designated by the Director of Professional Services.
8. Assess patient clinical status in home 48 hours after hospital discharge and on a 60 days basis per company policy.
9. Review/approve plan of care and all related documents prior to physician review/signing.
10. Provide patient care as needed/requested by Administrator and/or Director of Nursing.
11. Schedule of in home Reassessments with families, Complete Supervisory reports on nursing staff upon visits.
Qualifications/Minimum Job Requirements:
Current state license as an RN, BSN preferred
Current CPR
At least 2 years experience as a Registered Nurse
At least 1 year experience as a Clinical Supervisor in Home Health Care preferred
At least 1 year of Pediatric experience
Current state driver's license and automobile insurance
Proficient in computers/EMR
Highly motivated, self-disciplined, have excellent communication skills and willing to travel to patient homes
Utilization Management Experience
Reliable transportation for patient visits
Reside in the Amarillo area

Job Description: A certified Bilingual Speech-Language Pathologist Assistants (SLPA) or Speech-Language Pathologist Clinical Fellow (SLP-CF) will implement treatment programs to assist pediatric patients with cognitive, speech, language, and/or social/emotional disabilities and delays by administering speech therapy services in the home and community. Requirements:
Texas State SLPA License
Current CPR certification
A minimum of 1 yr. of experience preferred
Responsibilities:
Provides high quality care and meets the needs of the patient and family by implementing speech therapy treatment plans in conjunction with the speech therapist supervisor.
Assists pediatric patients to develop or regain cognitive, speech, language, feeding/swallowing and/or social/emotional functioning and improves their level of independence by implementing skilled interventions to maximize the potential of each individual child.
Participates in educating, coaching, and empowering caregivers and families to develop skills to carry over therapeutic activities into the child's daily routine.
Assures continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs.
Documents patient care services and care coordination in an intuitive electronic medical record system.
Maintains patient confidence by keeping information confidential.

Perform clinical assessment and identify patient needs
Document, prepare, and complete patient Plan of Care. Track progress and revise as needed
Meet mandatory continuing education requirements
Plan and provide appropriate treatment for neuro-muscular, neurological, and orthopedic disorders based on medical referral and therapy assessment
Assist and instruct patients and families in transfer techniques appropriate to patient abilities. Instruct patients and families on the use of equipment, such as walkers, wheelchairs, canes, etc., in a safe manner. Utilize appropriate therapy equipment specific to patient diagnosis and needs
Promote company mission and administrative policies to ensure the quality of care
Requirements:
Bachelor of Science in Physical Therapy from an American Physical Therapy Association-approved program
Current Texas State License as a Physical Therapist
Current Texas Driver's License and reliable transportation
One year of experience as a Physical Therapist in an acute care or rehabilitation setting. Pediatric experience required
Effective interpersonal relations and communication skills
Knowledge of durable medical equipment
Ability to work a flexible schedule and travel locally
Able to work in a patient's home in various conditions
Excellent organizational and time management skills
Proficient in Microsoft Office
Able to maintain a high level of confidentiality
Proof of current CPR certification
Ability to do heavy lifting, bending, pulling, pushing, and standing. Prolonged standing and walking are required
Requires working under some stressful conditions to meet deadlines and patient needs and to meet patient/family individualized psychosocial needs
