SUMMARY: AFH Caregivers work directly with our residents to teach important life skills, including independent living skills, community integration, activities of daily living (ADLs), social and communication skills, and behavior modification. Work on Individualized goals as outlined in the residents' Individualized Service Plan (ISP) REQUIRED EDUCATION & EXPERIENCE: Must be at least 18 years old. Must demonstrate behavioral skills and judgment conducive to role modeling appropriate skills for residents. Experience working with individuals with developmental disabilities is preferred. GENERAL HOURS OF DUTY: Part Time, 9AM - 3PM, Monday through Thursday JOB DUTIES: Provide supervision of residents of group homes. Assist the residents with personal care, including, but not limited to, all or some of the following: bathing, grooming, fingernail/toenail care, and brushing teeth. Ensure residents take prescribed medications, including administering medications, as needed. Help each resident reach and maintain the highest level of performance and independence. Perform light housekeeping duties that clients can't complete on their own. Perform minimal maintenance duties as necessary (i.e.-plunging a toilet, changing a light bulb, etc.) Implement treatment as outlined in each resident's Individualized Service Plan (ISP). Transporting residents to activities as needed. Attend and participate in scheduled community outings during the scheduled shift Prepare meals for residents and assist in teaching residents how to prepare meals Complete daily resident documentation by the end of each shift.

Maintain Skills Trainer certification for the Short-Term Empowerment Program, including all required training. SUMMARY: Chileda's Adult Family home (AFH) consists of one home in the community where residents live and learn to become more independent in all facets of life. The Adult Family Home Program Director is responsible for the operations of the Adult Family Home and strategic initiatives. The Adult Family Home Program Director is responsible for ensuring that the AFH is in compliance with DHS 88 and certification regulations and works closely with Managed Care Organizations to ensure supports are in place to meet clients' needs. The AFH Program Director ensures Chileda's mission, vision, values, and lenses are implemented in all aspects of the adult family home through effective leadership and mentoring. REQUIRED EDUCATION & EXPERIENCE: Must be at least 18 years old with a High School Diploma or equivalent. A Bachelor's degree in a human services field is preferred. Three years of supervisory experience working with people with developmental disabilities in a licensed care setting, with proven experience in leading a diverse group of employees, giving feedback and guidance, and making operational decisions related to the adult family home. Must demonstrate good communication skills, both verbal and written. Must possess intermediate computer skills to learn and operate software programs. Skilled in problem-solving for staff and residents supervised, with the ability to demonstrate judgement conducive to role modeling appropriate skills. Collaborate with the Human Resources Department on employee-relations and performance management activities and staffing needs; interview, hire, and assist with onboarding staff for the Adult Family Home. Review all forms, reports, and required documentation to ensure they are completed on time and in compliance. Utilize visual supports, Picture Exchange Communication System (PECS), communication boards/books, and other communication devices with students who use these resources. Demonstrate the ability to consistently and competently implement PBSPs with all students. Complete all assigned trainings within the designated time frame and complete at least 24 hours of training annually. Use crisis management techniques taught in trainings including verbal de-escalation, creative redirection, topic changes, and allowing space and time for processing during a crisis. Recognize, take immediate action when necessary, and report unsafe conditions to the appropriate manager. Participate in emergency drills such as fire evacuation, severe weather, tornado, and disaster plan. Ensure a safe and clean environment by removing damaged items, ensuring classrooms and residential programming areas are clean, and reporting maintenance needs to Recreational Manager or Shift Manager. Responsible for completing floor care, including sweeping, vacuuming, mopping, and shampooing carpets. General cleaning of identified office space. Assist with maintaining the general appearance of Chileda's campus, including yard work or external repairs as directed by the supervisor. Communicate to the supervisor any proactive steps that can and should be implemented to impact student and staff safety. All other tasks as assigned by the Director of Buildings and Grounds. Physical Demands: The Maintenance Technician is a physically active, fast-paced position, requiring the ability to manage several tasks and complete projects timely. REPORTS TO: Recreation Manager JOB DUTIES: Facilitate recreational activities in assigned areas guiding staff and teaching students independent leisure skills. Prepare all planned recreational activities and have available for students and staff 15 minutes before programming begins. Assist Recreational Manager in planning and preparing for special events and other rec programming. Complete inventory of rec supplies when assigned. Work with other recreation assistants to ensure that all residential programming shifts are covered. Complete recreational tracking for assigned area(s) before the end of each shift. Complete all tasks assigned to the end of shift checklist. Attend all meetings as assigned by the Recreational Manager. Use a Trauma Informed Approach by communicating with and about students in a respectful, positive, and age-appropriate manner while consistently using a therapeutic tone even in high stress situations. Utilize visual supports, Picture Exchange Communication System (PECS), communication boards/books, and other communication devices with students who use these resources. Demonstrate the ability to consistently and competently implement PBSPs with all students. Complete all assigned trainings within the designated time frame and complete at least 24 hours of training annually. Use crisis management techniques taught in trainings including verbal de-escalation, creative redirection, topic changes, and allowing space and time for processing during a crisis. Recognize, take immediate action when necessary, and report unsafe conditions to the appropriate manager. Participate in emergency drills such as fire evacuation, severe weather, tornado, and disaster plan. Ensure a safe and clean environment by removing damaged items, ensuring classrooms and residential programming areas are clean, and reporting maintenance needs to Recreational Manager or Shift Manager. Leadership, analytical, problem-solving, organizational, and communication skills are required. Hours: Generally, 8:00 AM - 4:30 PM, Monday - Friday. Work collaboratively with all departments to ensure students are working on goals to move to less restrictive environments, ensuring that permanency goals are discussed at the onset of services and at least quarterly thereafter. Review contracts for services, ensure that contract requirements are being upheld, and funding sources are maintained throughout services. Complete necessary outcome reports as requested through contracts. Lead the External Human Rights Committee, working collaboratively with the Director of Behavior Services and community committee members to ensure best practices. Ensure a thorough enrollment process, including a thorough review process, ensuring proper paperwork is obtained, and funding is secured prior to and throughout enrollment. Work collaboratively with the Administrative Assistant to ensure quarterly file reviews ensuring all record requirements are met. Collect and Monitor clinical data including enrollment, discharge, permanency, and treatment plan data. Report on data at least quarterly in administrative team meetings, and lead initiatives for improvement. Ensure compliance with licensing requirements for out-of- state discharged students (ICPC). Serve as Lead Client's Rights Specialist, be available for calls related to client's rights or grievances outside of typical work hours. Complete internal investigations as necessary. Be an overall advocate for students ensuring that student's emotional and social needs are met while ensuring students have needed tools and resources to be successful beyond Chileda. Ensure yearly satisfaction surveys are sent to students, parents/guardians, and social service agencies. Aggregate the results and prepare for agency reviews. Through written communications, phone contacts, and community opportunities, introduce and actively promote the scope of Chileda's programs to potential referral sources, such as social service agencies, parent groups, advocacy groups, and school district personnel.
