The Custodian II performs moderately complex (journey-level) custodial work. Cleans, dusts, disinfects, and sanitizes to maintain a safe and healthy environment. Operates floor maintenance equipment such as strippers, buffers, vacuums, and extractors. Requests and restocks supplies as necessary. Assists in conducting inspections of buildings and equipment for cleanliness and needed repairs. Assists in performing minor building and equipment repairs and installations. Accountable for maintaining assigned custodial equipment in good working order. Maintains outside areas around the hospital, clears trash and debris. Works under general supervision, with moderate latitude for the use of initiative and independent judgment.
Performs other duties as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
This position may also be eligible to earn additional pay for work performed on maximum security locations.
Essential Job Functions (EJFs):
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
Performs custodial functions timely and with high quality including sweeping, mopping, dusting, and cleaning windows and light fixtures. Ensures proper sanitization and disinfection in accordance with training received.
Cleans and maintains floors, restrooms, nurses' station, and other areas in accordance with established procedures and safety measures.
Maintains and cleans the interior and exterior of assigned buildings, empties trash, reports needed repairs. Replenishes supplies as needed.
Coaches other custodians as needed.
Inspects assigned buildings and areas for cleanliness, completed work, and needed minor repairs, and performs minor maintenance.
Completes documentation in an accurate and timely manner. Protects confidential information. Maintains inventory and equipment records.
Helps patients maintain optimal health and provides a safe environment for self, persons served and others.

Initial Screening Criteria:
Six months work experience required.
Experience in custodial or environmental services work in a nursing home, hospital, residential facility, hotel or other similar residential equivalent preferred.

The Social Worker II performs and documents complex social services work including serving as a liaison between the individual, legally authorized representative, and others to assure ongoing care, treatment and support through the use of person-centered practices. This position gathers information to assess the individual's support systems and service needs, including participation in interdisciplinary team meetings, as needed. This position completes an initial social history, as well as maintains appropriate updates. This position may also support the assessment of the individual's rights and capacity to make decisions and provides individual and group training to promote the exercise of rights. Essential Job Functions (EJFs):

Interview examples include upon admission or return on an extended forensic commitment, prior to transition to a new home, upon a change in status and in preparation for the annual individual support plan meeting.
Oversees the communication and information exchange between the individual, legally authorized representative, actively involved person and agencies, and facility or treatment providers. Oversight is to ensure that assigned staff have performed liaison function regarding ongoing care, treatment, and support to include, maintaining open communication, providing required notification, participating in interdisciplinary team meetings as requested, obtaining necessary consents and accessing external resources as needed.
Provides assistance to individuals, legally authorized representatives, and actively involved persons regarding guardianship issues, annual living options discussions, end-of-life planning and rights in accordance with state and federal laws and regulations and state and center policies.
Oversees the coordination and completion of the rights and capacity assessment by assigned staff and reviews the documentation.
Assigns facility staff to perform admissions, transfers, transitions, discharges, and furloughs to assure access to services through the use of person-centered practices. Examples include: providing tours of the facility for individuals and families interested in admission, providing information to the local authority to assist in obtaining required documents for admission including certified birth certificates, ISD records and social security cards and coordinating with the receiving facility for individuals transferring within the SSLC system to include transportation and the transfer of resident information, personal possessions, medication, etc.
Coordinates the design and conducting of individual and group training to explore and promote exercise of rights, voting, citizenship, and development of decision-making, person centered thinking and self-advocacy skills. Examples include scheduling and facilitating a group meeting to provide information regarding candidates running for political office to support resident voting, gathering baseline information regarding resident knowledge of their rights related to privacy for visits with family and friends and telephone calls and gathering or developing training materials to provide group education to improve and enhance self-advocacy skills.
Participates in residence and family meetings and conducts formal presentations describing facility processes.
Performs other duties as assigned. Knowledge, Skills and Abilities (KSAs):

The Program Supervisor III (Director of Admissions) performs complex (journey-level) administrative and supervisory program work administering and coordinating admissions to the state hospitals. Responsibilities include providing court and facility liaison on all commitment cases; monitoring the validity of court commitment cases and renewal orders; scheduling and coordinating all admissions; ensuring the scheduling and availability of psychiatrists to admit patients in a timely and clinically appropriate manner; coordinating medication hearings; participating in the hospital's quality assessment performance improvement program; and serving on committees. Works under general supervision, with moderate latitude for the use of initiative and independent judgment.
Performs other duties as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
This position may also be eligible to earn additional pay for work performed on maximum security locations.
Essential Job Functions (EJFs):
Attends work on a regular basis and may be asked to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
Plans and coordinates admissions to the state hospital. Ensures timely and thorough admission processes of new patients. Maintains a daily check on the bed availability to facilitate more timely admissions. Assures the availability of psychiatrist to assess the patient on admission.
Supervises assigned staff. Responsibilities include, but are not limited to, reviewing applications, interviewing candidates, selecting and hiring potential employees, scheduling and assigning workload, establishing performance measures, evaluating work performance, providing feedback and guidance, recommending staff development and training, recommending personnel and disciplinary actions, scheduling and approving leave.
Confers with staff on admission processes and identified issues. Plans, coordinates, and implements new and revised hospital admission processes and procedures and monitors compliance.
Evaluates and analyzes operations, prepares reports and makes recommendations. Responds to requests for special project participation from executive leadership as requested. Develops and implements the admissions quality assessment and performance improvement activities that includes development of admission goals and objectives and performance measures for evaluating the admissions program. Attends and participates on various committees, subcommittees, and workgroups of the hospital quality assessment performance improvement program.
Provides training or technical assistance regarding the admissions program for internal and external stakeholders (e.g., patients, staff, and law enforcement, etc.).
Coordinates medication hearings and commitment renewals with the counties of residence of the patients, or where appropriate works with the County Court to ensure courtesy hearings and commitments are carried out on behalf of persons served. Assures proper documentation is prepared.
Assures transfers and discharges are coordinated with receiving facilities or aftercare resources (e.g., jails, other state hospitals, Local Mental Health Authorities, and Local Behavioral Health Authorities) as requested.
Assures the coordination of the Dangerousness Review Board (DRB) processes, as requested.
Coordinates and communicates with various courts, legal/law enforcement personnel, and other related individuals providing required legal documents.
Performs other duties as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Registrations, Licensure Requirements or Certifications:
Must possess a valid Texas driver license or obtain it no later than 90 days after hire date to drive a state vehicle.
