Clinical Assessment & Treatment:

What Will I Do?

You will provide mobile and face-to-face therapy for individuals, couples, families and groups in a competent, professional and confidential manner and according to the agreed upon treatment plan along with helping clients develop and implrement aftercare ministrations.
You will meet with the Clinical Supervisor for supervision as assigned and participate in scheduled staff and in-service meetings.
You will work in conjunction with CONCERN Case Managers to implement treatment plans and individual program descriptions.
You will maintain a prositive, professional relationship with all linkage and referral agencies.

This position requires some flexibility with scheduling around client needs. Ability to work days, evenings, and/or weekends is preferred but not necessarily required.

What Will I Do?

You will work in conjunction with CONCERN Case Managers to implement treatments and individual program descriptions, schedule assigned clients according to CONCERN delivery standards, fulfills all case management duties in a timely fashion and professional manner maintain high standards of confidentiality, and consistently reaches assigned billable expectations weekly. You will handle reports and progress notes of assigned clients following required deadlines and ensure all aspects of files are complete, current, and in accordance with auditing, payer standards, and regulations.
You will help clients develop and implement aftercare plans.
You will take part in scheduled staff meetings, in-service gatherings, Quality Assurance audits, and supervision with the program psychiatrist and Clinical Supervisor as directed.
You will maintain a positive, professional relationship with all linkage and referral agencies.

Position Summary:

Responsibilities:
Responsible for providing clinical services as outlined by the policies and procedures of Better Morning.
Carry a caseload of up to 6 CBI cases and will be required to meet and engage them 5 times a week.
Provides services to youth and families in a culturally competent respectful manner
Provides individual and family counseling.
Initiate CBI services within 24 hours of receiving the referral notification from supervisor
Initiates Family Team Meeting within 7-14 days of referral
Develops safety plan with the input of youth, family, and all stake holders
Educates the family about CBI services and role of CBI clinicians and CBI team
Explains the benefit of a team working with a family
Provides 24- hour availability for youth and families
Explains in detail to youth and family how crisis calls are handled 24/7 and the role of the CBI team
Provides CBI services in home, school, and community
Assures that intensity of service meets the needs of the youth and family
Participates in CBI required training curriculum
Remains flexible to schedule sessions around youth and family's availability
Complete MUI report in case of unusual incidents
Facilitate discharge team meeting including all stake holders
Participate in individual and group supervision as scheduled

Key Responsibilities
Clinical Assessment & Treatment Planning
Conduct comprehensive clinical assessments, including ASAM criteria, biopsychosocial evaluations, and other required diagnostic tools within established timeframes.
Develop, implement, and regularly update individualized, measurable treatment plans in collaboration with patients and the multidisciplinary treatment team.
Present assessment findings and treatment progress clearly during treatment team meetings and case reviews.
Complete required ASAM documentation for admission, continued stay, transfers, and discharge, as applicable.
Therapeutic Services & Patient Care
Provide individual, group, and family therapy services in accordance with program standards, including regular individual sessions and daily process groups.
Facilitate therapeutic, psychoeducational, and experiential groups that promote insight, skill-building, accountability, and recovery-focused progress.
Assist patients in identifying strengths, challenges, and barriers to recovery, adapting therapeutic approaches to meet diverse needs.
Apply clinical judgment to support patient movement through appropriate levels of care and prepare patients for post-discharge recovery.
Family Engagement, Care Coordination & Continuity
Maintain regular communication with families and support systems, including family sessions and collateral contacts, as clinically indicated.
Maintain appropriate communication with referral sources and external providers to support continuity of care.
Collaborate with case management and aftercare services to develop comprehensive discharge and continuing care plans addressing legal, financial, vocational, and psychosocial needs.
Crisis Intervention, Safety & Ethics
Apply sound clinical judgment in crisis situations, including suicide risk assessments, safety planning, and de-escalation interventions.
Maintain professional boundaries and enforce program rules in a firm, fair, and respectful manner.
Demonstrate adherence to ethical standards, patient confidentiality laws, and organizational policies.
Documentation, Compliance & Quality
Complete clinical documentation accurately and within required timeframes in accordance with organizational policies, licensure requirements, and accrediting body standards.
Maintain secure clinical records and ensure documentation clearly reflects patient progress toward treatment goals.
Participate in supervision, quality improvement activities, and ongoing professional development.
Qualifications
Master's degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or another related human services field that qualifies the individual to pursue professional clinical licensure.
Minimum of two (2) years of clinical experience providing therapy in behavioral health and/or substance use treatment settings, strongly preferred.
Active clinical license in the applicable state (LCSW, LMFT, LMHC, or equivalent), strongly preferred.
Experience working in residential, inpatient, or detoxification levels of care, strongly preferred.
Knowledge of state and federal confidentiality regulations.
Familiarity with Joint Commission standards and clinical documentation requirements.
Strong clinical assessment, documentation, communication, and problem-solving skills.
Ability to manage a clinical caseload and work effectively within a multidisciplinary treatment team.
