Interview patients to determine medical history, condition, desired results and appropriate modality.
Monitors patient's responses to treatment and adjusts, changes, or terminates treatment based on feedback.
Document patient information, interview responses, treatments, feedback, and results.
Maintain therapy area in a clean, safe, sterile and Organizational state.
Assist patients to safely perform activities related to the development of strength and endurance.
Minimum qualifications for the ideal future caregiver include:
High School Diploma/GED
Graduate of an accredited Massage Therapy Training Program
Current licensure as a Licensed Massage Therapist (LMT) by the state of Ohio
Current Basic Life Support (BLS) Certification through the American Heart Association (AHA) or American Red Cross
One year of experience providing massage services or an equivalent combination of relevant education and/or experience

Job Summary
The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position's qualifications, professional practices and ethical standards.
Licenses and Certifications
Must have and maintain a current, valid state PT licensure or certification
Current valid CPR certification, preferred
Qualifications
Education: Degree in Physical Therapy from an accredited program
Experience: 0-1 years
Roles and Responsibilities
* Conduct thorough assessments to evaluate patients' physical conditions, functional limitations, and rehabilitation needs.
* Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients' physical function.
* Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
* Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
* Collaborate with interdisciplinary team to ensure comprehensive patient care.
* Other duties as assigned.

Essential Responsibilities:
Perform comprehensive physical therapy evaluations and develop individualized plans of care
Provide skilled physical therapy services in accordance with physician orders and agency policies
Assess patient mobility, strength, balance, and functional limitations
Implement therapeutic exercises, gait training, and pain-management techniques
Educate patients and caregivers on treatment plans, safety, and home exercise programs
Document evaluations, progress notes, and discharge summaries accurately and timely
Communicate effectively with physicians, case managers, and interdisciplinary team members
Follow all state, federal, and Medicare home health regulations
Qualifications:
Current Physical Therapist license in the State of Ohio
Graduate of an accredited Physical Therapy program
Home health experience preferred, but not required
Strong clinical, communication, and organizational skills
Ability to work independently and manage a caseload
Valid driver's license, reliable transportation, and auto insurance
