Interpreter - Medical and Dental
Job Summary: The Medical and Dental Interpreter will assist non-English speaking patients and families as they receive services within Choptank Community Health System (CCHS). They will be available to answer the phone and convey messages related to the other office staff. Assists Referral Specialist in communicating medical appointments outside of CCHS. Reinforces basic health teaching. This is a nonexempt, full-time position in pay grade 4 with the pay range of $21.08 - $28.45. The Interpreter reports directly to the Practice Manager.
Required Skills/Abilities:
Working knowledge of automated patient data systems.
Must be proficient in computer skills.
Must possess excellent interpersonal skills and communicate effectively.
Fluent in secondary language and English to provide interpretive services for bilingual clinical visits.
Ability to effectively communicate in writing in both English and Spanish.
Excellent communication and interpersonal skills to greet patients and visits in person or over the phone
Eligible for Certificated Medical Interpreter Course
Must demonstrate the ability to use office equipment such as computers, printers, telephones, fax machines and copier machines.
Education and Experience:
High School Diploma or equivalent.
Preferred experience in medical interpretation services
Certified Medical Interpreter required, if not certified, must complete and pass certification exam within one year from date of hire.
Minimum six months experience preferred.

Duties/Responsibilities:
Non -English interpretation and translation services

Assists with sliding fee applications and appropriate and accurate income documentation
Facilitates services for non-English speaking patients with outside agencies such as Department of Social Services etc. as needed.
Informs patients of medical results and provider messages.
Daily works electronic health record messages.
Receives calls from patients and provides customer service.
Consistently screens calls, documents phone calls in the electronic health record. Messages are accurate, legible, and complete.
Voice mail messaging service is answered timely.
Always follows established procedures for handling patient calls and visits.
Works with the medical staff and clinical team to coordinate patient flow
Practices AIDET with all patient interactions.
Addresses patient concerns with Site Director in timely and responsive manner.

Verifies insurance eligibility per established procedure for all Medicaid patients.
Ensures all registration forms are completed during first appointment and verified at every visit and signatures are obtained annually

Scheduling
Schedules non- English-speaking patients appropriately in practice management system following template guidelines.

Reschedules non- English-speaking patients when necessary.
Manages same day requests in coordination with clinical team
Reschedules appointments when needed.
Troubleshoots scheduling problems.

The Clinical Administrator is a member of MedStar Health entity-based Division of Nursing and leadership team. This is accomplished by direct action delegation coaching and support of the Division of Nursing's associates and shared/collaborative governance activities. The Clinical Administrator is recognized as a professional role model and clinical expert who promotes a professional practice and care environment that supports continuous learning.The Clinical Administrator assumes leadership authority and responsibility during a specific shift (e.g. day evening night and/or weekend) either for a designated number of patient care units/departments or the hospital at large. Collaborates with the members of the Nursing Leadership team and other administrators/leaders within the hospital in maintaining adherence to regulatory and accreditation standards and MedStar Health's policies and procedures. Primary Duties and Responsibilities
Assumes clinical and administrative supervision of nursing and patient care services for multiple units/departments or hospital/entity. Consults advises or informs nursing providers and other leaders concerning issues that arise and their resolution as appropriate unit needs and/or problems during the shift worked. Escalates concerns applying the chain of command.
Consults and communicates with nursing and hospital administration as well as senior medical staff and other Nurse Leaders concerning any operational issues or matters pertaining to staffing and the management of assigned areas. Functions as liaison between patient care administration and medical staff.
Creates a professional atmosphere and an environment of coaching and development that supports shared/collaborative governance. Ensures effective utilization of the interdisciplinary model of care (IMOC) and associated standards of excellence. Rounds on patients during the shift for the purpose of engagement satisfaction and improvement in the care experience. Investigates and follows through on patient complaints and safety events.
Assists Nurse Leaders in selecting training and orienting department personnel. Contributes to the development of standards of performance evaluating performance and conducting performance management planning for associates as appropriate. Initiates or makes recommendations for associate counseling and corrective action. Maintains ongoing communication with leaders to review programs provide feedback discuss new developments and exchange information.
Manages materials equipment and supply activities assuring adequate availability and utilization. Communicates material and supply needs to departmental leaders. Participates in product selection and evaluation as appropriate.
Coordinates with the Capacity Management and/or Admissions Departments to strategize for the daily flow of patients into and out of beds. Rounds routinely on each nursing unit and other departments to identify empty beds actual/potential discharges and transfers. Communicates this information back to the Admissions Department. Evaluates the effectiveness of bed turnover and communicates with Environmental Services as necessary.
Monitors high risk patients and complicated conditions and institutes appropriate actions. Ensures compliance with unit divisional and hospital policies procedures as well as regulatory and accreditation standards. Prepares written documentation as required. Maintains required reports records statistics and notes.
Receives and reviews preceding shift reports and notes patients' conditions when rounding on units. Coordinates department wide staffing patterns to meet safe and efficient staffing levels for each unit. Adjusts staffing to meet patient care needs and to provide adequate and fiscally responsible coverage of all units. Assists Nurse Leaders with unit/departmental staffing through coordination of float pool agency and per diem personnel.
Supervises coordinates and evaluates patient care services interventions during assigned shift. Intervenes where necessary to assure optimal patient care outcomes by utilizing approaches and strategies that support and optimize outcomes. Takes action to support the National Patient Safety Goals nursing sensitive indicators regulatory requirements and other identified quality metrics.
Maintains and demonstrates a professional patient-first atmosphere and an environment of coaching and development that supports shared decision making interdisciplinary collaboration and a high level of patient and associate satisfaction as evidenced by outcomes data. Provides shift-based clinical leadership through role modeling professional practice behaviors proper delegation of activities and management of the nursing team by maintaining staff accountability. Evaluates the need for and contributes to improvement in the professional practice environment.
Participates in hospital/entity committees task forces interdisciplinary forums and projects (e.g. serious safety event reviews and performance improvement teams) at the request of Nurse Leaders. Attends departmental meetings and contributes to the achievement of Magnet Recognition or Pathways to Excellence program. Promotes a public image of professional nursing excellence and represents the hospital in community outreach efforts as appropriate.
Maintains knowledge of current trends and developments in the fields of nursing and health care through a variety of professional activities including but not limited to reading the appropriate literature attending related seminars and conferences and maintaining membership in professional nursing associations. Use of a range of therapeutic and other interventions, including supportive counseling, group work, and education to address issues through cancer continuum.
The Social Worker will monitor patient care across the continuum through follow-up with patients/families/caregivers and community services.

Provides information about resources and options available in the community and coordinates service delivery.
Educates and addresses concerns with service delivery including service gaps and access issues, including but not limited to insurance and financial barriers to care.
Provide information based on current knowledge, to provide psychosocial support and assist the patient/family/caregiver in coping with their disease to improve their overall health care management.
Provide specific information on how to communicate with physicians and other members of care team.
Educates the patient/family/caregiver of psychosocial services and support within cancer program.

Empowers patients/family/caregiver through education and encouragement to self-advocate and communicate their needs.
Interprets patient/family/caregiver needs and provides information concerning the availability and limitations of resources.
Maintains pertinent and timely documentation in patient's medical charts and departmental records.

Professional Role
Attends regularly scheduled staff meetings, i.e., department, divisional, or system-wide.
Participates in professional activities to support the Cancer Program as representative of the Commission on Cancer for psychosocial support.
Supports role specific departmental educational and staff development initiatives.
Demonstrates skills and knowledge necessary to provide care appropriate to cancer population through Cancer Program, including knowledge of growth and development, the ability to obtain and interpret information to identify patient needs, and to provide the care needed.

HomeCare MD is seeking a compassionate and skilled PRN Speech Therapist to join our growing home care team. This role is ideal for an experienced Speech Language Pathologist who is passionate about providing exceptional patient-centered care in the comfort of a patient's home.
Position Summary
The Speech Therapist is responsible for conducting initial and ongoing patient assessments, delivering skilled Speech Language Pathology services, and acting as case manager for speech-therapy-only patients. This role will collaborate closely with physicians, caregivers, and interdisciplinary team members to support optimal patient outcomes.
Essential Functions
Develop individualized plans of care tailored to each patient's needs.
Recommend and train patients on augmentative and alternative communication methods (e.g., automated devices, sign language).
Teach patients techniques to improve speech production, voice quality, and oral/written language skills.
Educate and train patients in safe swallowing strategies or strengthening exercises to prevent aspiration.
Maintain accurate and timely documentation, including evaluations, progress notes, and reports for insurance justification.
Conduct research-based assessments and contribute to treatment methodologies.
Participate in interdisciplinary assessments and collaborate in creating and evaluating patient care plans.
Attend case conferences and required agency meetings.
Deliver services as prescribed by the patient's physician and in accordance with the plan of care.
Provide patient, caregiver, and family counseling and education.
Communicate effectively with physicians, healthcare practitioners, and the QAPI department.
Participate in agency-sponsored in-service training and work collaboratively with clinical leadership.
Education & Experience Requirements
Current Maryland Speech Language Pathology License.
Master's degree from an accredited college or university.
Current American Heart Association BLS for Healthcare Providers.
2-3 years acute care experience preferred.
2-3 years community health experience preferred.
Valid driver's license, automobile insurance, and ability to meet fleet safety requirements (travel required).
Strong knowledge of medical terminology.
Excellent interpersonal, communication, and people management skills.
Basic computer proficiency; Microsoft Office Suite preferred.
Experience with electronic medical records; Kinnser software experience preferred.
Basic knowledge of Medicare/Medicaid/OASIS/Commercial Insurance requirements.

Job Summary: The Certified Medical Assistant (CMA) is a member of the health care delivery team and is responsible for and accountable to the patients. The CMA performs delegated clinical duties within the supervising clinician's scope of practice and consistent with the CMA's education, training, and experience. This is a nonexempt, full-time position in pay grade 3 with the pay range of $19.14 - $24.88. The Certified Medical Assistant reports directly to the Medical Clinical Care Director.
Required Skills/Abilities:
Working knowledge of automated patient data systems.
Must demonstrate the ability to use medical equipment and materials safely.
Must be proficient in computer skills.

Duties/Responsibilities:
Initial Patient Assessment and Health Data Collection
Collects health status data relevant to the patient's presenting problem in a continuous, systemic, and complete manner.
Completes all components of a patient intake accurately, including but not limited to vital signs, chief complaint, medication reconciliation, allergies, surgical history, family history, quality measures, depression screening, alcohol/drug screening, and tobacco screening.
Reconciles past medical information into current EHR system per protocol.
Consistently documents in the Electronic Health Record according to established procedures.
Properly prepares patient for Clinician by following established intake standards for all visit types including face-to-face and virtual visits.
Assists in implementing the plan of care as requested by the Clinician including- medication reconciliation, venipuncture, injections, EKG's, nebulizer treatments, ear lavage, pain screening, stock medication administration, collection and preparation of laboratory specimens and performs CLIA Waived testing.
Assists clinician during patient procedures and/or examinations including pap smear, vaginal exams and swabs, colposcopy, incision and drainage, punch biopsies, ear lavage, nebulizers, Nexplanon insertion/removal, IUD insertion/removal, toenail evulsions, and mole/skin tag removals.
Properly documents and completes ICD-10 and CPT coding for services administered during the patient visit.
Assists patients with appointments and referrals as needed by researching chart to find answers to referral questions (when, where, why) and future appointments at CCHS.
Maintains a safe and hygienic environment for patients
Cleans and sterilizes exam room after each patient visit according to CCHS and CDC protocol.
Properly dons and doffs PPE as required for each type of patient encounter.
Removes and processes all patient specimens collected during visit.
At the end of each day, ensures proper disinfection of every exam room and work area including lab.
Adequately stocks exam rooms daily according to practice standards and provider requests.
Follows CCHS and CDC protocol for distinction of clean and dirty areas within the lab.
• Effectively Communicates within the Center
Communicates with members of the healthcare team to address patient's questions and/or concerns to the appropriate team members.
Consistently identifies patients at risk and communicates with providers and nurses on emergent patient needs.
Performs Other Duties as Required
Answers telephones, greets patients, schedules appointments, enters refill requests, and maintains medical records.
Completes nurse visits for pediatric, adult, and women's health patients. These may include blood pressure checks, A1c, INR, vaccines, medication administration via injection (B12, testosterone, Depo Provera, Depo Medrol), PPD placement, weight check, urine dipstick, urine pregnancy test.
Travels to other CCHS sites to cover shortages within the department.
Attends monthly site meetings, department meetings and full staff meetings as required.
Consistently displays professionalism and respect during interaction with CCHS patients, staff, clinicians, and guests.
Reviews open radiology and lab orders for all providers.
Review chart and call diagnostic centers to look for results, if no results, send missed lab/x-ray letter.
Close orders or reset due date alarms accordingly.
Pre-visit planning: coordinates with provider and updates patient chart alert prior to patient appointments per provider preferences and patient needs.
List specific reason for visit, last A1c if applicable, screenings needed (A1c, colonoscopy, mammogram, pap, etc.), and vaccines due.
Assists provider or nurse with procedures per provider or nurse request.
Regular, reliable attendance is a requirement of this job.
