Responsibilities
Perform daily cleaning tasks, including sweeping, mopping, dusting, vacuuming, and disinfecting common areas, resident rooms, and bathrooms.
Ensure the cleanliness and tidiness of dining areas, hallways, offices, and other facility spaces.
Manage cleaning supplies and report any maintenance issues to the supervisor.
Follow all safety and health protocols to maintain a safe and sanitary environment.
Provide exceptional service to residents and staff while maintaining respect for their privacy and comfort.
Assist with laundry services, including washing, drying, folding, and storing linens and personal items.

Qualifications

We are seeking a Physical Therapist to provide skilled physical therapy services to patients across Senior Living Communities. In this role, you will be instrumental in helping seniors recover, enhance mobility, and maintain their independence through tailored physical therapy treatment plans.

Responsibilities
Evaluate physician's referral and patient's medical record.
Assessing and evaluating therapeutic/rehabilitative/functional status using approved evaluation techniques, tests and observations, and participation in the development of the total plan of care.
Evaluates the home environment.
Determines priority needs for physical therapy.
Directs physical therapy treatment.
Reports to physician, patient's response to treatment and changes in condition.
Identifies patient and family/caregiver needs for other home health services and refers as necessary.
May train patients in the use of prosthetic device.
Administer a prescribed physical therapy program to restore and maintain maximum
function and prevent disability using a range of therapeutic modalities.
Maintain appropriate documentation in the patient chart, and re‐evaluate patient
progress and revise treatment programs when necessary.
Communicates physical therapy philosophy and treatment approaches and goals to patients, families and other members of the rehabilitation team.
Supervise Physical Therapy Assistants according to Agency policy and state regulations.
Qualifications
Valid Physical Therapist license in Arizona, in good standing.
Two years of Home Health experience as a Physical Therapist preferred.
Possesses and maintains current CPR certification.
Licensed driver with reliable vehicle and current vehicle insurance.
Experience in geriatric physical therapy or working with seniors is preferred.
Ability to work independently and manage time efficiently.
Excellent observation, verbal and written communication skills, problem solving skills, and organizational skills.
Excellent communication and interpersonal skills to work effectively with residents, families, and the healthcare team.

Now Hiring: Full-Time RN - Senior Living Communities (West/Central Valley)
We are currently seeking a dedicated full-time Registered Nurse to provide services at Senior Living Communities in the West and Central Valley regions.
Responsibilities
Case Management: Develop and implement individualized care plans for patients, coordinating with physicians, family members, and other healthcare providers to ensure optimal care.
Patient Assessment: Conduct thorough assessments of patients' physical, emotional, and social needs and update care plans as required.
Care Coordination: Ensure the timely delivery of care services, including scheduling visits and coordinating with other healthcare professionals.
Medication Management: Oversee medication administration, monitor for side effects, and educate patients and families on proper medication use.
Patient Education: Provide education on health management, disease prevention, and improving quality of life for patients and their families.
Documentation: Maintain accurate and up-to-date records for all patient visits, including care plans, progress notes, and communication with the care team.
Advocacy: Act as an advocate for the patient's needs, ensuring that care is delivered in accordance with the patient's preferences and goals.
Collaboration: Work closely with physicians, therapists, and other care team members to ensure a coordinated approach to patient care.
Qualifications
Graduate of an accredited School of Nursing
Active Registered Nurse (RN) licensed in Arizona with current valid credentials.
Minimum two (2) years' Registered Nursing experience in Home Health required.
Must pass background check and meet all health and safety requirements.
Valid driver's license and reliable transportation required.
Strong assessment and critical thinking skills.
Excellent communication and interpersonal skills.
Ability to work independently, manage time effectively, and provide compassionate care to seniors.
Ability to travel to designated service areas, including West Valley to Central Phoenix or Central Phoenix to East Valley to provide care to patients within their homes or Senior Living Communities.
Compensation information: Our RNs are compensated on a per-visit basis, with rates varying depending on the type of service provided. We offer seven distinct pay rates aligned with specific visit types, ensuring fair and competitive compensation for each service delivered.
