The Contract Assessment Counselor will work up to 20 hours per week in the office and is paid per client. You will be responsible for providing comprehensive screenings and assessments to persons experiencing mental health and substance use issues. To determine the most appropriate level of care based on the information gathered on the screenings and assessments and to facilitate admission into the appropriate program. The Assessment Counselor may carry a small caseload contingent on the needs of the outpatient program and assessment scheduling needs.
Responsible for the conducting assessments, utilization reviews, urine screenings, completion of treatment planning processes, and clinical services to determine what problems need to be addressed.
Weekly Supervision meetings with Clinical Director
3.Completes day-to-day services documentation (same day) as required by funders, accreditation bodies, and Community for New Direction.
4. Act as client advocates to coordinate required services or to resolve emergency problems in crisis situations.
5. Work closely with community referral partners with individuals affected by issues of mental health, trauma and or abuse and emotional health issues to expedite communication and access to treatment.
6. Encourage clients to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships with others.
7. Develop therapeutic processes, work with families and help clients define goals, plan action and gain insight.
8. Refer clients to psychologists and other services if needed.
9. Evaluate the effectiveness of counseling programs and clients' progress in resolving identified problems and moving towards defined objectives.
10. Assist the Director in covering needed services for Outpatient Treatment Services.
11. Participate in staff meetings, committees, attend professional development conferences, trainings, webinars/seminars as needed to enhance your knowledge, broaden your horizon, and obtain continued education.
12. Performs other duties as assigned to promote the overall mission and vision of CND.
Education:
Master's degree in social work, Human Service, Psychology, Behavioral Science, or related field. With Three (3 years of experience in counseling and chemical dependency. Must be able to diagnose, counsel, and treat mental and emotional disorders is required. Must have an active (LISW, LSW, LPC, or LPCC) Licensure is required.
Additional Eligibility Qualifications
Knowledge assessing patients' mental status through observations and then developing treatment plans or making referrals to the appropriate services. Required to provide crisis intervention services to patients who are a danger to themselves or others. Monitor a patient's treatment progress and keep track of his or her mental status. Excellent interpersonal and organizational skills.

Position Responsibilities for SUD Counselor:
Completes Diagnostic Assessments and treatments plans on individuals seeking services for gambling, alcohol or other drugs.
Facilitates therapeutic cognitive behaviorally based groups for alcohol and other drug treatment.
Conducts individual and family counseling sessions to identify those problems the client has which may interfere with the recovery process and develop appropriate treatment for client.
Makes appropriate referrals for further counseling and services to improve client's quality of life and improve likelihood of recovery.
Provides case management, linkage and/or referrals to appropriate school, employment bureau, housing, health facility, or other community resource in order to enhance the client's quality of life and improve likelihood of recovery.
Position Requirements for SUD Counselor:
Ohio LPC or LSW required.
Experience working with individuals with SUD.

PRN SUD Counselor

GENERAL FUNCTION:The PRN SUD Counselor is a part-time counselor that fills in to assist with covering groups in the Intensive Outpatient Program and the Continuing Recovery program. This person also provides back-up case management and crisis intervention to the full-time Case Counselors, assist the Quality Assurance team with clinical record reviews, and assist resident monitors with rule enforcement and medication monitoring.
ESSENTIAL FUNCTIONS: Provide secondary case management and crisis intervention services to program participants. Facilitate Outpatient Group Counseling and Intensive Outpatient Group Counseling. Record all service activities by entering progress notes in the Y-Haven Electronic Medical Record. Participate in team meetings, quality assurance meetings, in-service trainings and Coordination meetings as needed. Coordinate with Clinical Director on all critical clinical issues. Responsible for following the child abuse and neglect reporting process including completion of Suspected Child Abuse/Neglect reporting form, calling children protective services (696-Kids) and notifying your immediate Supervisor, Branch Executive, Risk Management and Human Resources Department (s). All employees of the YMCA of Greater Cleveland are mandated reporters for any signs or suspicion of child abuse and neglect.
Administrative: Maintain accurate and thorough records for all clients seeking admission in the AWARDS electronic health record. Assist in supplying data for and creating Y-Haven service reports.
Program: Facilitate addiction therapy groups on a limited, as needed basis. Monitor medication self-administration for residents on a limited, as needed basis. Provide direct reimbursable clinical services 50% of each workday. Be visibly involved w/ clients, volunteers and staff in a positive, friendly manner that communicates and demonstrates the YMCA's core values of caring, honesty, respect and responsibility. Follow all established policies and procedures, including but not limited to, safety practices and personnel policies. Participate in and support association events and fund raising activities. Maintain a clean and orderly work environment. Secure required trainings and/or certifications necessary for the position.

QUALIFICATIONS:Education and Experience: Bachelor's Degree in Psychology, Social Work or related field required. Extensive knowledge of issues affecting the homeless population, chemical dependency and mental illness is necessary. Preferences: 3+ years' experience in social service field, successful experience in intake, screening and assessment.
Professional Certification(s): LCDCIII, LPC or LSW Required CPR, First Aid, AED certifications and Child Abuse Prevention training within 30 days of hire date.
WORK ENVIRONMENT & PHYSICAL DEMANDS: While performing the duties of this job, the employee is regularly required to talk and actively listen to clients, staff, and volunteers. The Counselor will provide mental health services to individuals incarcerated in the Delaware County Jail. Services will include but are not limited to mental health assessments, individual and group counseling, crisis intervention and case management. Participation in weekly group supervision and participation in other mandatory Maryhaven trainings will also be required.

Contract Assessment Counselor

The Contract Assessment Counselor will work up to 20 hours per week in the office and is paid per client. You will be responsible for providing comprehensive screenings and assessments to persons experiencing mental health and substance use issues. To determine the most appropriate level of care based on the information gathered on the screenings and assessments and to facilitate admission into the appropriate program. The Assessment Counselor may carry a small caseload contingent on the needs of the outpatient program and assessment scheduling needs.
Responsible for the conducting assessments, utilization reviews, urine screenings, completion of treatment planning processes, and clinical services to determine what problems need to be addressed.

3. Completes day-to-day services documentation (same day) as required by funders, accreditation bodies, and Community for New Direction.
4. Act as client advocates to coordinate required services or to resolve emergency problems in crisis situations.
5. Work closely with community referral partners with individuals affected by issues of mental health, trauma and or abuse and emotional health issues to expedite communication and access to treatment.
6. Encourage clients to express their feelings and discuss what is happening in their lives and help them to develop insight into themselves and their relationships with others.
7. Develop therapeutic processes, work with families and help clients define goals, plan action and gain insight.
8. Refer clients to psychologists and other services if needed.
9. Evaluate the effectiveness of counseling programs and clients' progress in resolving identified problems and moving towards defined objectives.
10. Assist the Director in covering needed services for Outpatient Treatment Services.
11. Participate in staff meetings, committees, attend professional development conferences, trainings, webinars/seminars as needed to enhance your knowledge, broaden your horizon, and obtain continued education.
12. Performs other duties as assigned to promote the overall mission and vision of CND.
Education:
Master's degree in social work, Human Service, Psychology, Behavioral Science, or related field. With Three (3 years of experience in counseling and chemical dependency. Must be able to diagnose, counsel, and treat mental and emotional disorders is required. Must have an active (LISW, LSW, LPC, or LPCC) Licensure is required.
Additional Eligibility Qualifications
Knowledge assessing patients' mental status through observations and then developing treatment plans or making referrals to the appropriate services. Required to provide crisis intervention services to patients who are a danger to themselves or others. Monitor a patient's treatment progress and keep track of his or her mental status. Excellent interpersonal and organizational skills.
