Christiana Care Hospital in Newark, DE is looking for a (RN) Case Manager Supervisor to provide direct Care Management Services, to a defined patient population, while providing supervisory services to the Case Management & Social Work staff.
The Care Management Supervisor will provide on-going support to the department. The department Manager and Director will assist in the direction and education of all phases of the Care Management process. The Supervisor will be held accountable to work in a collegial manner with patients, physicians, staff, consultants and all ancillary departments. Will have the responsibility for the direct oversight/supervision of department-based teams; coordinates special departmental projects to include patient and staff satisfaction, length of stay reduction, and service delivery improvement.
SCOPE, PURPOSE, AND FREQUENCY OF CONTACTS:
Interacts daily with patients, nurses, physicians, and other ancillary and professional staff Daily contact with all Care Management staff at any Christiana Care Health System campus.
The Care Management Model:
Our Care Management Triad Team Model is a collaboration between the following:
RN Case Manager - Manages patient care, drives patient progression, and establishes a discharge plan.
Social Worker - Resolves psycho-social barriers and supports discharge needs.
Utilization Management - Reviews patient status for appropriateness and anticipated payer coverage.
EDUCATION AND EXPERIENCE REQUIREMENTS:
DE RN License or Compact RN License required.
CCM/ACM Certification preferred. If certification not in place, willing to complete within 18 months of hire.
Bachelor of Science in Nursing required, Master of Science in Nursing (Masters preferred)
Minimum 5-7-year experience in health care environment. Prefer at least 3 years case management experience with at least 2 years serving in a leadership role.

This exciting curriculum integrates:
Lecture and simulation education
Peer support
Up to 16 weeks (as determined by your individual transition to practice needs) of preceptor-led clinical experiences structured to foster critical thinking, evidence-based practice, interdisciplinary collaboration, effective communication, quality and safety, and professional development.
Our medical and surgical nursing units at Newark and Wilmington campuses serve an adolescent/adult/geriatric population with a wide variety of complex diagnoses and surgical procedures. Nurses having successfully completed the residency will have built a strong foundation for nursing practice in a fast-paced, challenging environment. The preceptor led program followed by continued residency meetings during months 8, 10, and 12 to further develop as professional nurses and integral members of our caregiver team.
Nurse residents are required to pass State Boards and be licensed as a RN in the State of Delaware (or compact state) prior to starting position. Upon hire, a commitment agreement to work at ChristianaCare will be signed. The agreement requires completion of two years within ChristianaCare and 15 months of those two years on the unit of hire. Individuals unable to fill this commitment will be charged $10,000.

ChristianaCare is recruiting for a Pharmacy Technician to work part time - 40 hours biweekly, day/evening shift rotation. Every other weekend required; holidays as required.
Principal Duties and Responsibilities:
Assist the pharmacist in providing timely, efficient, cost effective, and appropriate pharmaceutical care to the patient
Refill patient medication bins through robot cart fill and manual picks; update carts by including new orders, removing discontinued medications, and transferring medications. Update labeling of bins on admission, transfer, and discharge of patients
Deliver and exchange 24-hour cart fill to nursing units at scheduled times. Check medication storage areas for discontinued and discharged patient medications; ensure that returned medications are placed back into stock
Reorder medications in short supply and restock the picking station and other areas of the pharmacy
Send prepared medications via pneumatic tube system. Any medication that cannot be tubed is delivered to the appropriate location on the nursing unit
Manage automated dispensing cabinet inventory, including accurate selection and loading of medications
Ensure that medications are stored properly and that only in date medications are stored. Remove discontinued medication
Receive and check incoming supplier order(s)
Prepare IV medications using aseptic techniques and in following processes in full compliance with USP 795, 797, and 800 federal regulations
Use appropriate Personal Protective Equipment (PPE) and closed system transfer devices (as needed) for the preparation of chemotherapy and other hazardous medications
Education and Experience Requirements:
High School Diploma required
Pharmacy Technician Certification Board (PTCB) certification required unless enrolled in a PharmD program.
Applicants with Pharmacy Technician experience are encouraged to apply and will be required to obtain PTCB certification within 1 year of employment.
Applicants enrolled or graduate of a 4-year bachelor program in the following health science majors are encourage to apply and will be required PTCB certification within 1 year of employment: Applied Molecular Biology and Biotechnology Biology, Chemistry, Exercise Science, Health Behavior Science, Human Physiology, Medical Diagnostics, Medical Diagnostics Pre-PA, Medical Laboratory Science, Nutrition, Nutrition and Dietetics, Nutrition and Medical Sciences, Nursing, Occupational Therapy, Physical Therapy, Physician Assistant, Sports Health, or other health related degrees

Student Nurse Externs provide hygiene/comfort measures and selected direct and indirect patient care activities and ancillary duties for our patients. This position is for ALL campuses.
Principal Duties and Responsibilities:
The nurse extern will demonstrate skills and knowledge vital to deliver care appropriate to a diverse patient population, including knowledge of growth and development, the ability to acquire information to identify patient needs, and to provide the appropriate care needed.
The student nurse will perform crucial unit specific tasks such as:
Communicating to patients, families, employees, and physicians utilizing both verbal and written communication skills.
Responds to patient needs with promptness, initiates interventions within scope of responsibility and collaborates with RN.
Performs assigned work safely, adhering to established departmental safety rules and practices. Reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards, or safety violations that may cause injury to oneself, other employees, patients and visitors.
Knowledge of nursing care provided for specific patient population of unit, as well as knowledge of department and unit standards, policies, and selected procedures.
Ability to perform specific unit tasks as assigned by the RN, and recognize change in patient status and take appropriate action.
Ability to communicate optimally and utilize required technical skills as outlined in procedures for patients and to follow universal precautions.
Exercise judgement, tact and diplomacy.
Education and Experience Requirements:
High school diploma/GED required or International Education Evaluation from a ChristianaCare approved vendor
Current enrollment in an accredited registered nursing program which prepares the candidate to sit for the NCLEX-RN.
GPA of 3.0 or higher (preferred)
One completed “in-person/ hands-on clinical rotation by the start of the program
Availability to attend the mandatory classroom training portion; these dates will be outlined during the interview process.
