Purpose Statement:
Help patients by planning and providing individual and group support services. This includes working with people who are dealing with mental health challenges, trauma, substance use, and physical or emotional abuse, and helping them build skills to cope and heal.
ESSENTIAL FUNCTIONS:
Conduct psychosocial assessment of patient and develop an effective treatment plan that applies therapeutic principles appropriately while ensuring patient safety at all times.
Through patient observation and patient/family interview obtaining information on the patient's needs, skills, interests and limitations for the development and implementation of the treatment plan.
Provide crisis intervention and assist families in understanding the implications and complexities of a life situation and its impact on lifestyle.
Participate in multidisciplinary treatment team meeting to discuss psychosocial assessment, treatment planning and ongoing discharge planning.
Identify, assess and coordinate patient cases and anticipate their needs related to illness, hospitalization, finances, education and home/community services.
Facilitate education, process groups, support groups and referrals as requested.
May conduct intakes and assist in coordinating placements within facility programs.
Promote recovery, symptom reduction and the achievement of the highest level of social functioning.
Communicate or aid family members in understanding and supporting the patient.
Manage a caseload and assume responsibility for patient chart documentation.
May facilitate education, process groups, support groups and referrals as requested.
Manage a caseload and maintain accurate and timely documentation to assure patient records comply with facility and accreditation standards.
Demonstrate a positive, empathetic and professional attitude towards customers always. When patient needs are not met, acknowledge and work to resolve complaints. Recognize that patient safety is a top priority.
OTHER FUNCTIONS:

EDUCATION/EXPERIENCE/SKILL REQUIREMENTS:
* Master's degree in Social Work from an accredited college or university or a new graduate of accredited master's degree social work program with successful completion of social work field education, internship or similar practicum through master's degree program required.
* Two or more years' experience working with patient population preferred.
LICENSES/DESIGNATIONS/CERTIFICATIONS:
State licensure, as required by the state in which the facility operates; which could include: Licensed Master SocialWorker (LMSW), Licensed Clinical SocialWorker (LCSW), Licensed Practical Counselor (LPC)
CPR and de-escalation/restraint certification required (training available upon hire and offered by facility).
First aid may be required based on state or facility.

Position Summary
The Overnight Case Manager will join our dedicated team in a licensed Transitional Residential Treatment program. This role will involve providing comprehensive mental health support services to individuals with behavioral health needs. The incumbent will play a crucial role in facilitating rehabilitation, recovery, and community integration for our clients.
Primary Responsibilities
Conduct comprehensive assessments of clients' mental health needs, strengths, and goals.
Develop individualized rehabilitation plans in collaboration with clients, incorporating person-centered approaches and evidence-based practices.
Provide individual and group counseling sessions to address mental health symptoms, coping skills, and life skills development.
Implement behavioral interventions and therapeutic techniques to support clients in achieving their recovery goals.
Monitor and evaluate clients' progress regularly, adjusting treatment plans as needed to ensure effectiveness.
Coordinate with interdisciplinary team members, including psychiatrists, nurses, case managers, and community partners, to ensure holistic care delivery.
Facilitate psychoeducational workshops on topics such as stress management, conflict resolution, and relapse prevention.
Advocate for clients' rights and access to resources within the community, including housing, employment, and healthcare services.
Maintain accurate and timely documentation of client interactions, treatment plans, and progress notes in accordance with regulatory standards.
Participate in staff meetings, case conferences, and training sessions to enhance professional development and program effectiveness.
Ensure safety and establish and follow rules for overnight safety and emergency procedures.
Perform other duties as assigned.
Qualifications and Requirements
Bachelor's degree in Psychology, Social Work, Counseling, or related field preferred.
Minimum of 2 years of experience working in a mental health or behavioral healthcare setting, preferably with individuals experiencing severe mental illness.
Must be able to meet and receive a criminal records clearance, as required by California Code, Welfare and Institutions Code - WIC § 5405, federal contractor requirements and/or agency practices.
Must pass post-offer, pre-employment physical and TB clearance as required by state community care liecensing standards.
A valid driver's license, reliable personal vehicle, personal auto insurance as required by law, and an MVR sufficient to obtain and reasonably maintain insurability under agency auto liability policies are essential job requirements.
Local travel as required to fulfill primary job responsibilities and duties.

Job Responsibilities:

Plan, oversee, facilitate and document patient's recovery.
Co-facilitate assigned group or family sessions as needed.

Prepare individual treatment plans for each assigned patient.
Initial assessments as well as follow up assessments.

Required Education, Skills, and Qualifications:
HS graduate or equivalent required;

* Familiarity with Medication-Assisted Treatment (MAT), including methadone, buprenorphine, and naltrexone.

What You'll Do:
Performs the psychosocial assessment of patients and families/caregivers in the hospice program in a timely manner and ensures that identified needs are addressed in the patient's plan of care. Provides an assessment in the patient's identified residence and assistance when this is not safe, and another plan is required
Demonstrates exceptional service to patients, families, referral sources, co-workers, and other customers. Assist team members as needed to ensure that this level of service is consistently provided
Counsels patient and family/ caregivers and staff as needed in relationship to stress, and other identified coping difficulties
Provides accurate and timely documentation of patient services to reflect the plan of care
Provides assistance with applications for Supplemental Security Income, Hospice, Medicaid, and Medicaid Waiver. Obtains resources from community social, health, pharmaceutical and welfare agencies to assist in meeting patient/family needs
Serves as liaison between patients and families/caregivers and community agencies
Qualifications:
Social Work Degree
New Mexico: Master's Degree in Social Work required.
Current State Licensure in Social Work

Purpose Statement/Position Summary: The Social Services Caseworker supports the delivery of therapeutic day treatment services for preschool-aged children with emotional and behavioral challenges. Under the supervision of clinical and program leadership, this role assists in implementing individualized behavior plans, facilitating therapeutic activities, and ensuring a safe, structured, and supportive environment. The position emphasizes direct care, milieu-based interventions, collaboration with families and multidisciplinary staff, and timely documentation to promote children's social, emotional, and developmental growth.
Minimum Qualifications/Work Experience:
Required:
* 1+ years of social work/case management experience in a pediatric medical setting or an equivalent combination of related education and work experience
* Lived experience as a parent or caregiver of a child who was hospitalized during infancy and/or received early intervention services during that period
* Must be willing and able to work fully onsite
Preferred:
* Bilingual skills/fluency in Spanish
* Head Start, Pre-School, or ABA Experience
Education/Licensure/Certifications: Bachelor's degree in Social Work or an equivalent combination of related education and work experience required. BLS certification from the American Heart Association.
Pay Scale Information
$51,979.00-$85,394.00

Job Summary
Odyssey House Louisiana, Inc. is seeking a full-time SocialWorker for the Community Health Center. The schedule is Monday-Friday 8am-5pm. The SocialWorker will provide comprehensive assessment and diagnosis of behavioral health clients. The position will also assist primary care providers and Program Managers with implementing disease management programs; participate in the development, review, and evaluation of the family and/or client's plan of care; utilize information from various sources to identify patients who would benefit from disease management and case management services; educate and engage patients in their own care management; identify social and/or medical barriers that impede health outcomes; link patients to appropriate levels of care/appropriate and supportive services; work collaboratively with a multidisciplinary inter-agency team to facilitate achievement of desired treatment outcomes, monitor patient ED and hospitalization utilization rates; and utilize an Internet based care management system to track patient progress and health outcomes.
Responsibilities and Duties
It is the duty and responsibility of the SocialWorker to:
coordinate/facilitate patient care;
provide direct care to patients within the Scope of Practice for which the provider is licensed;
provide direct care to clients/patients to support the agency's department;
manage patient care under Best Practices, UDS recommendations, and as prescribed to maintain accreditation;
conduct thorough documentation of visits and medical orders to support billing charges;
complete billing components of visit to optimize revenue within dictated time frames;
work collaboratively and maintains communication with patient care team (providers, nurses, etc.) to provide effective, timely, and appropriate patient care management;
assist patients and providers to adhere to evidence-based treatment protocols for specific disease states;
educate patients and helps to engage them in their own care management, provide asthma and diabetes education;
monitor patient care by tracking patient charts, counsel patients during physician visits, follow up through telephone calls and home visits as needed, and utilize Internet based case management tracking system;
meet directly with patient/family to assess needs and develop an individualized care plan as necessary;
ensure/maintain plan consensus from patient/family, physician, and health care team;
collaborate with relevant community-based social services and health care organizations (socialworkers, home health care, school nurses, hospice providers, etc.) to address patient needs/barriers to improved health outcomes and to secure necessary care and equipment;
address/resolve system problems impeding progress; proactively identify and resolve delays and obstacles to expedite care;
utilize advanced conflict resolution skills as necessary to ensure timely resolution of issues;
collaborate and communicate with multidisciplinary team at all phases of planning process, including initial patient assessment, planning, implementation, interdisciplinary collaboration, and teaching and ongoing evaluation;
identify patients with need for Patient Assistance Program submission to pharmaceutical companies in order to offset the cost to patient (This includes assisting patient in filling out application, identifying needed documents to support income, assure provider licenses are submitted, application is signed by provider, manage the course of application, notify patient of arrival of medicine, and when need for refills occurs; log of applicants will be kept and submission of application and arrival of meds will be documented in log);
perform utilization management and quality screening for assigned patients;
identify at-risk populations using approved screening tool(s) and follows established care and reporting protocols;
monitor medical resource use on an ongoing basis and take action to achieve continuous improvement;
refer cases and issues to PCP and follows up as indicated; follow up on referrals from PCP as appropriate;
actively participate in clinical performance improvement activities;
participate in development, implementation, evaluation and revision of disease management tools as a member of the clinical change team;
attend local meetings as necessary;
participate in training as necessary to successfully complete job responsibilities, including training in disease management protocols, care management strategies, use of network software programs, and data analysis skills;
use data to drive decisions and plan/implement performance improvement strategies related to care management for assigned patients, including fiscal, clinical, and patient satisfaction data;
collect special study data for specific performance and/or outcome indicators as determined by program;
participate in discussions of key variances and outcomes that relate to areas of direct responsibility;
use data in collaboration with other disciplines to ensure effective patient management;
provide effective treatment planning and assist clients in successfully achieving goals;
evaluate crisis situations and apply appropriate interventions;
actively participate in meetings that support the integrated health care model to provide comprehensive care for clients;
assist in the detection of “at risk” patients and development of plans to prevent further psychological or physical deterioration;
assist the primary care team in developing care management processes including guidelines, disease management techniques, case management, and patient education to improve self-management of chronic disease;
provide assessment, consultation, and brief intervention for psychological/psychiatric problems and/or disorders;
teach patients, families, and staff, care, prevention, and treatment enhancement techniques
monitor the site's behavioral health program to identify and make improvement recommendations for problems related to patient service; and perform other duties as assigned.
Qualifications and Skills
Required:
Minimum qualifications include the following:
Master's in Social Work
LCSW unencumbered licensure
Greater than 1 year Clinical/UM/Discharge Planning/Home Care/Social Work/Ambulatory experience
Current CPR certification
