The Physical Therapy Assistant Provides clinical physical therapy treatments for program participants on acute, restorative and maintenance levels as needed. Follows the treatment plan as per Registered Physical Therapist assessment and documented care plan.
QUALIFICATIONS:
A graduate from a physical therapist assistant curriculum approved by the American Physical Therapy Association.
Currently licensed as a Physical Therapist Assistant in the state of Colorado.
Minimum of one-year experience working with the frail or elderly population required.
Minimum of one year experience as a Physical Therapy Assistant in a skilled nursing facility is preferred.
Good working knowledge of physical, psychosocial and behavioral needs of the elderly population.
Able to understand medical terminology and how it applies to physical therapy, to communicate with participants, staff, IDT and physicians, and how to prepare meaningful reports.
Ability to communicate with the participants to establish favorable attitude to motivate the participants to desired cooperation and effort in his/her rehabilitation development.
Must have a valid driver's license, proof of insurance and have means of transportation.
ESSENTIAL FUNCTIONS:
Perform physical therapy treatments and related duties as may be delegated by the physical therapist.
Communicate Participant's changes and progress per treatment plan on a regular basis to Registered Physical Therapist.
Maintain proper documentation according to state, federal PACE regulations.
Maintain proper records according to PACE policy and procedures.
Perform and record, if required, as specified by the planned treatment program, non-evaluative on-going data: Measurements of vital signs, range of motion, strength and linear increments; Types of assistive mobility devices used, and the distances covered; Results of tests and evaluations performed by the registered physical therapist.
Respond to psych-social support needs of the participant and/or the participant's family.
Communicate appropriately and effectively with all personnel and to document according to professional standards.
Keep timely records, meet regularly with the Registered Physical Therapist and attend care conferences as requested.
Participate in patient care conferences and other rehabilitation related meetings as appropriate.
Communicate with Registered Physical Therapist and IDT members regarding patient progress, problems and plans.
Participate in in-service training programs for all staff in the facility.
Instruct participant and family members and nursing staff in restorative and therapeutic maintenance programs.
Abides by regulations pertaining to PACE as established by Federal and State Agencies and the Volunteers of America Health Facilities policies.
Attends required in-services & completes assigned online modules.
Perform other duties as assigned.

Observes drivers to assess their ability to safely operate and maneuver a WM vehicle and provides immediate, constructive feedback to ensure proper and consistent safety and performance duties are achieved.
Instructs and reinforces WM SAFETY defensive driving skills, WM Way processes, and OBU tablet usage to drivers.
Keeps accurate records of the driver's progress, including areas of strength and improvement, completed training tasks, and additional training and coaching recommendations.
Reports driver performance data to the Operations Manager and Onboarding Optimization Manager; highlighting both strengths and areas for improvement; recommends additional coaching and training if needed.
Mentors drivers by providing guidance and support as they transition into their roles, helping them develop skills and confidence.

Who We Are: Lutheran Hospital is a community-based, acute-care hospital in Wheat Ridge, Colorado. It includes Lutheran Hospice Center and Lutheran Spine Center at Denver West.
Lutheran Hospital began in 1905 as the Evangelical Lutheran Sanitarium, a tent colony for tuberculosis patients. As medical advances diminished the demand for tuberculosis treatment, community leaders decided the center should meet a new need. Community-based Lutheran Hospital, a not-for-profit, general medical facility located on a 100-acre campus, opened its doors in 1961.
Our new Lutheran Hospital campus at Clear Crossing in Wheat Ridge opened its doors on August 3rd. Construction on the six-story replacement hospital started in 2021 and provides a state-of-the-art care site in the West Denver suburbs.
Essential Functions:
Maintains up-to-date licensure and certifications required for position.
Provides procedural assistance as a circulating tech, scrub tech, or monitoring tech, according to Angio lab policies and procedures and Scope of Practice documents.
Ensures proper patient identification, order verification, and prepares the patient for the exam.
Adheres to radiation safety guidelines and maintains a safe working environment for patients and staff.
Develops specific knowledge about invasive, diagnostic, and interventional supplies, appropriate indications and applications, and proper use and deployment techniques.
Follows appropriate protocol for medical necessity, coding and charging, obtaining necessary consents, maintaining applicable QC (Quality Control) programs, and reporting any equipment failures or problems.
Provides appropriate patient education, ensures patient comfort, and address patient and/or family concerns.
Keeps accurate records of patient information, procedures performed, and any adverse reactions or incidents.
Coordinates patient care and communicates pertinent information to other patient care providers following procedure.
At the end of the procedure, suture the sheath in place, or remove the sheath and apply pressure to obtain hemostasis.
Performs advanced interventional procedures including ablation procedures, stents, thrombectomy, embolization.
Orient new angio staff, provide teaching, coaching, mentoring and provide direct supervision as a defined preceptor.
Proctors other staff including specialized equipment and procedural techniques.
Qualifications:
Work Experience
1-2 years Angio/Interventional experience preferred, or
1-2 years vascular or interventional experience.
Licenses and Certifications
Vascular/Interventional radiography certification Preferred
Relevant State Licensure and/or certifications Upon Hire Required
ARRT - American Registry Upon Hire Required
BCLS - Basic Life Support Upon Hire Required
ACLS - within 120 Days Preferred
PALS - Pediatric Advanced within 120 Days Required for pediatric hospitals

What You'll Do:
Plant, cultivate, irrigate, harvest, and put up hay and other feed crops.
Mix, utilize, and/or dispenses volatile compounds, including fuels, paint, fertilizer, and exterminating agents such as herbicides, insecticides, and fungicides.
Feed and care for livestock, including cows, calves, bulls, and stocker cattle.
Work with cattle on foot and horseback, including herding to pasture for grazing and transferring between pastures and working facilities.
Calve cows, wean calves, and prepare and administer medicines and vaccines as appropriate.
Operate and maintain vehicles, agricultural and construction machinery and equipment in accordance with verbal instructions and those provided in operator's manuals.
Build, repair, and maintain corral and pasture fences, pens, and other ranch facilities.
May be required to keep records and prepare reports regarding cattle, calving, and machinery maintenance.
What We're Looking For:
Required Education, Experience and/or Abilities
An associate's degree, preferably in an agriculture-related field, with two (2) years of direct agriculture-related experience; or an equivalent combination of education and/or experience may be considered.
Possession of a valid driver's license appropriate for the type of equipment to be operated.
Beef Quality Assurance (BQA) certificate or willingness to achieve certificate within three (3) months.
Additional Eligibility Qualifications
English language fluency, verbal and written.
Acceptable results of a pre-employment background check, credit check and drug/alcohol test.
