A robust transitional care and high risk outreach program provides physicians with additional information to support their patients outside of the clinic. Premier Medical, a part of NeueHealth, is seeking a Board-Certified Family or Internal Medicine Physician with strong experience in Value-Based Care to join our healthcare team. The ideal candidate will be dedicated to delivering comprehensive, patient-centered care, managing both acute and chronic conditions while focusing on preventive health and cost-effective outcomes. This role emphasizes collaboration, quality, and the use of evidence-based medicine to improve patient outcomes and reduce overall healthcare costs. Responsibilities: Provide high-quality, comprehensive primary care to a diverse patient population, with a focus on chronic disease management, preventive care, and health promotion. Conduct thorough evaluations, formulate care plans, and manage both acute and long-term medical needs. Practice in alignment with Value-Based Care principles, focusing on quality metrics, patient satisfaction, cost efficiency, and closing care gaps. Collaborate with care teams, specialists, and ancillary providers to ensure coordinated, integrated care. Utilize EMR systems effectively for accurate documentation, coding, and compliance with value-based programs (e.g., HEDIS, CMS Star Ratings, RAF/HCC coding). Engage patients in shared decision-making and educate them on preventive health measures, lifestyle modifications, and self-management strategies. Monitor outcomes and participate in quality improvement initiatives designed to enhance patient care and organizational performance. Stay current with advancements in Family and Internal Medicine, population health, and value-based models of care. Qualifications: A state license to practice medicine in Florida. Medical degree (MD or DO) from an accredited institution. Board Certification in Family Medicine or Internal Medicine (required). Minimum 3+ years of experience in primary care, with demonstrated Value-Based Care experience. Strong knowledge of chronic disease management, preventive care, and population health strategies. Familiarity with HCC/RAF coding, quality metrics (HEDIS, Stars), and risk-adjusted payment models preferred. Excellent interpersonal and communication skills with the ability to build long-term patient relationships and collaborate effectively within a multidisciplinary team.
