Job Description
This position will serve patients and families in NWA and areas of NE Oklahoma. We have a variety of shift configurations including nights, days, and weekends.
Minimum Requirements: Graduate of an accredited school of nursing; Current LPN license without restriction; current satisfactory TB skin test; minimum 1 year nursing experience in a hospital or healthcare facility preferred; at least one year experience is preferred in the areas of home health, hospice, gerontology, geriatrics, acute care & care of the terminally ill. Ability to cope with human suffering, emergencies and other stresses. Should be caring, sympathetic, responsible and detail oriented. Must be able to correctly assess patients' conditions and determine when consultation is required. Reliable transportation and a current driver's license with liability insurance.
General Description: Provides nursing and supportive care in accordance w/an interdisciplinary plan of care. Treat patients, educate patients about medical conditions and provide advice and emotional support to patients' family members. Responsible for documentation, operation of medical equipment, and administration of medications. Follows plan of care and works closely with Case Manager, Physician, and other Interdisciplinary Team members to ensure exceptional care.

We provide compassionate and mission minded hospice care to patients and families NWA and NE Oklahoma. This position will work onsite in addition to providing nursing care in patient home settings. This role requires use of personal vehicle, a valid driver's license and current auto insurance.
General Description: The RN is an integral part of the hospice interdisciplinary care team and as such is responsible for providing patient care visits, assessing medical needs, liaising with physicians, attending deaths, triage calls, and assisting with the admission process. The RN must have an understanding that hospice care must be delivered according to accepted standards of practice, guided by the mission of the organization and the patient/family's needs and concerns, the RN blends all services of the hospice into an integrated program of support. We have a variety of shift configurations for Full and Part Time.
The RN Case Manager assumes patient care responsibilities for individual patients including: Completing comprehensive physical assessment, pain assessment and management, symptom control management as it relates to terminal illness, coordination of Circle of Life services to the patient/family, completes required documentation (i.e. assessments, care plans, clinical notes, agency tracking forms, etc.) In a timely manner in accordance with COL policies, establishes a therapeutic nursing relationship with the Hospice patient/family/caregivers, provides weekend coverage and may assist during times of excessive nursing absenteeism.
