Job Description We provide compassionate and mission minded hospice care to patients and families NWA and NE Oklahoma. This position will work onsite in addition to providing nursing care in patient home settings. This role requires use of personal vehicle, a valid driver's license and current auto insurance. General Description: The RN is an integral part of the hospice interdisciplinary care team and as such is responsible for providing patient care visits, assessing medical needs, liaising with physicians, attending deaths, triage calls, and assisting with the admission process. The RN Case Manager assumes patient care responsibilities for individual patients including: Completing comprehensive physical assessment, pain assessment and management, symptom control management as it relates to terminal illness, coordination of Circle of Life services to the patient/family, completes required documentation (i.e. assessments, care plans, clinical notes, agency tracking forms, etc.) In a timely manner in accordance with COL policies, establishes a therapeutic nursing relationship with the Hospice patient/family/caregivers, provides weekend coverage and may assist during times of excessive nursing absenteeism.
