General Description: The Triage LPN is an integral part of the hospice interdisciplinary care team. This position triages patient calls, assesses needs, dispatches nursing staff, and facilitates multiple aspects of patient care needs. This position must be able to demonstrate compassion and exceptional customer service over the phone and with visitors. This position will use multiple computer, cloud, and software programs. Must maintain regulatory compliance and ethical judgement while performing duties.
Requirements: Current unrestricted AR/OK LPN license, BLS certification, strong assessment skills with critical thinking and high level of problem solving within a healthcare facility. Strong documentation and communication with the ability to stay calm and cope with human suffering/emergencies. Prefer hospice experience, home health, or acute care nursing experience with computer charting experience.

The Medication LPN will work a part time schedule of Monday - Friday 8-4:30. This positon is onsite at our Lowell, AR facility and does not have the option for remotely working. The Medication LPN is responsible for processing patient medication refill requests by reviewing medical records, contacting providers for authorization, and accurately submitting refill information to pharmacies, ensuring timely access to necessary medications while adhering to established protocols and patient safety standards.
Requirements for Consideration: Current and valid LPN in the state of Arkansas. Prefer compact licensure. Needs knowledge of nursing functions, and basic anatomy and physiology. Critical thinking and independent problem-solving skills. Accuracy in maintaining medical records. Able to communicate effectively on a level that is easily understandable. Proficient computer skills are essential.
Specific Duties
Enter refill request information, route to appropriate physician, and make proper notifications to ensure accurate and timely prescription filling
Call/fax pharmacies with refill approval information and enter information in the EMR
Notify patients when prescriptions have been called in, set up for delivery, or are ready to be picked up.
Triage calls and participate in patient education regarding pharmaceutical procurement process
Work with infusion, DME, and other vendors to procure medications and/or supplies
Assist in training nurses on medication filling processes and assist with process improvements
Work with physicians and ensure timely notification of medication changes, availability, issues or anything that may affect the delivery of care
