Citrus Health Network, Inc. was formerly known as Northwest Dade Center, Inc. and changed its name to Citrus Health Network, Inc. in September 1997. The company was founded in 1978 and is based in Hialeah, Florida. As of April 12, 2010, Citrus Health Network, Inc. operates as a subsidiary of Physicians Health Choice of Texas, LLC.
