PURPOSE AND SCOPE: Functions as the hemodialysis team leader in the provision of chronic hemodialysis care and treatment. Provides day to day direction and supervision to assigned direct patient care staff. Communicates with the physician and other members of the healthcare team to interpret, adjust, and complete patient care plans. Provides safe, effective delivery of patient care in compliance with standards outlined in the facility procedure manual, as well as regulations set forth by the company, state, and federal agencies. PRINCIPAL DUTIES AND RESPONSIBILITIES: General and Staff Related: Provides day to day guidance, support and direction to direct patient care staff, providing informal feedback on an ongoing basis and formal feedback input for the annual performance evaluation. Participates in the department staffing and the appropriate hiring, firing and disciplinary actions. Recommends disciplinary action to Clinical Manager and initiate as appropriate. Ensures compliance with all company and facility approved procedures and policies as well as regulations set forth by state and federal agencies in clinics with more than 100 patients. Approves or disapproves time or personnel schedule changes in the absence on the Clinical Manager ensuring compliance with applicable regulations, policies and procedures for documenting time of work hours for staff members on assigned shift. Participates in patient care plan meetings. Maintains knowledge in the current practices related to the principles and techniques of dialysis by participating in all scheduled inservices. Trains and orients staff as necessary. Routinely observes and guides direct patient care staff for appropriate technique and adherence to facility policies and procedures. Promotes and assists with compliance to OSHA programs in order to maintain a safe and clean working environment. Maintains overall shift operation in a safe, efficient, and effective matter. Conducts staff meeting with Clinical Manager at least monthly or as needed to keep patient care staff informed of changes in patient care needs or operations to improve delivery of care. Meets routinely with the Clinical Manager to discuss personnel and patient care status, issues, and information. Supervise all documentation of patient information. Coordinates Charge Nurse duties with Staff RNs acting in the relief charge capacity. Patient Care: Assesses daily patient care needs and develop and distribute patient care assignments appropriately. Assesses patient needs, respond to dialysis treatments, and communicate concerns to rounding physician. Implement changes inpatient care/treatment as directed. Assumes primary responsibility in an emergency situation. Monitors and supervises all patient care activity during dialysis and assist as necessary. Collaborates with direct patient care team in making decisions to benefit patient care. Continuously monitors patient's condition with regards to problems and potential complications associated with dialysis. Administers medications to patients per physician's orders. Acts as the subject matter expert and as a resource for staff members. Technical: Supervises and participates in completion of short and long term care plans. Admits new patients according to facility procedure. Ensures educational needs of patients are met and educate the patient and family about End Stage Renal Disease, dialysis therapy, diet, and medication. Supervises the safe and effective use of all equipment involved in direct patient care. Operates all dialysis related and emergency equipment safely and efficiently when needed. Performs required testing and verification and initial the checklist for start-up and shut-down procedures as outlined in the Technical Services Manual. Completes Nurse's Technical Training Program/Water Quality Facility Training. Other: Assists with special projects or other duties as assigned by the Clinical Manager and the interviewing of potential direct patient care staff as requested. Promotes efficient use of medical supplies. Attends and participates in monthly Quality Assurance meetings. Performs other related duties as assigned. Reports to the UM Manager. This position is a position for a California Licensed Nurse whose primary responsibilities are to perform the prospective and retrospective review of the requests for service authorizations in order to monitor the continuity and coordination of care and to assist in the utilization of appropriate services. The Prior authorization Nurse conducts these reviews of authorization requests to verify accurate provider specialties, medical treatments and services, place of service and or facilities. Also oversees the Prior Authorization Coordinators to direct appropriate utilization and data. RESPONSIBILITIES & DUTIES: Determines compliance of pre-established medical necessity criteria applying Milliman, and Healthcare Management Guidelines. Redirects members for alternative treatment/service that are appropriate to the members condition after they have been reviewed with the UM Manager or Medical Director. Will identify cases requiring potential or actual use of medically in appropriate interventions and refers these to the UM Director & Medical Director for review. Should be able to process a minimum of 100-150 auths per day. Will interact with providers for medical information necessary to adjudicate authorizations. Will review authorizations and see if it's clinical history supports service/procedure that's being requested. Will review requested authorizations and verify it is requested to appropriate provider and service area. Will review potential denial of service with the UM Director & Medical Director. Must investigate and follow up on all requests for additional information by UM Manager and/or Medical Director. Will be the primary resource for the Prior Authorization Coordinators. Will educate providers as needed with the Authorization/Referral process. Will be primary point of contact with the providers on urgent authorization requests. Must identify and report to the QM Department of any potential quality of care issues and /or pre-established UM referral indicators. Must maintain confidentiality on all member medical/clinical information. Will perform other job duties as necessary to accommodate departmental change, workload and/or emergencies. Must ensure the privacy and security of PHI(Protected Health Information) as outlined in policies & procedures relating to HIPPA compliance. The Clinical Research Nurse, ensures that the health and safety of research participants and the integrity and quality of clinical trials, are maintained and conducted in accordance with federal, state, IRB, departmental, and institutional regulations, policies and procedures. The Clinical Research Nurse will primarily provide nursing expertise and care to research study patient management with primary focus on bone marrow transplantation and cellular therapy clinical trial protocols. They will ensure compliance to study protocols, may participate in feasibility evaluation, recruitment, consenting and enrollment activities, perform medical tests and procedures to facilitate study patient visits, provide investigational medication education and training, administer treatments, support side effect and adverse event monitoring, coordinate data collection, documentation and reporting, as applicable. In collaboration with research teams, they will support and provide direction in internal and external monitoring and auditing activities. They will serve as a liaison and work closely with investigators, department members, study sponsors, IRB and the institution, to support administration of compliance, financial, personnel and other related aspects of all ongoing clinical studies. Additional duties may be assigned to support incoming projects, department, and institutional needs. In collaboration with multidisciplinary researchers, they may drive research programs through nursing lens and perspective to integrate new advances in clinical trials management. The role requires excellent planning and organizational skills, as well as the ability to work in a changing, multiple-demand setting in order to prioritize a large volume of work and meet deadlines efficiently and accurately. Strong interprofessional collaboration, excellent written and verbal communication skills, and the ability to work with a diverse group of people, including physicians, patients and families, are essential. EXPERIENCE: Minimum 2 years' experience in Pediatric Bone Marrow Transplant/Hematology-Oncology nursing required Minimum 1 year research experience preferred Experience with clinical trials is strongly preferred Knowledge of Institutional Review Board (IRB) policies and procedures Proficiency in Microsoft Office EDUCATION/LICENSES/CERTIFICATIFICATIONS: Bachelor's degree in nursing required, Master's degree preferred Active California Registered Nurse (RN) license Certified Clinical Research Professional (CCRP) preferred
