As the Behavioral Health Provider (BHP), you are a part of the primary care treatment team that identifies, triages, and manages patients with medical and behavioral health needs within the primary care setting. In addition, the Behavioral Health Provider will provide skill training through psycho-education and patient education strategies and will develop specific behavioral change plans for patients and behavioral health protocols for target populations. Working with the care team in situations in which a patient may be experiencing a behavioral health crisis and take the lead to coordinate support and stabilization for the patient as well as continued monitoring of the patient's presenting behavior. The position requires the applicant to be either a Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor in the state of Oregon. As the Behavioral Healthcare Provider, you will: Provide brief (10-20 min), solution focused behavioral interventions with active development of Behavioral Change planning for patients referred to Behavioral Health via Medical Providers. Provide follow up appointments to deliver psychotherapeutic care related to the mind/body connection with medical problem presentations. Provide biopsychosocial assessment and evidenced-based psychotherapy interventions to patients across the lifespan with medical conditions to support their management of recovery from these conditions. Inquire of current and past events, preference of care plan delivery, partnering with staff at the client level for a team approach, document decisions and next steps in the care process. Provide mental health assessment, diagnosis, and brief evidenced-based psychotherapy usually lasting from 1-6 visits per episode of care to Primary Care patients with mental health and addictions problems across the lifespan. Work with clients to set up a care plan and how to continue the plan even as visits may or may not continue with the Behavioral Health Consultant. Complete all charting as needed and required by contacted funding sources: including assessments, progress notes, and billing Be immediately available to the health center staff during working hours in the event of a psychiatric emergency or behavioral health emergency and take the lead around stabilization and follow up regarding the patient when clinically appropriate and safe. Participate as a member of the health center staff in the event of any other kind of emergency. Provide behavioral health consultation and education to the Primary Care Provider (PCP) teams regarding psychosocial treatments. Partner with other providers and staff for each client and his/her/their needs to include family members and any other support systems for the client. Individuals, couples and/or family consultation with patients including: Functional and strength based assessment and diagnosis Psychoeducation for patients and their support. system Medication adherence counseling and disease self management counseling Motivational interviewing to develop behavioral strategies aimed at symptom reduction brief problem solving cognitive intervention aimed at modifying negative thinking and promoting self efficacy. Provide case management services to patients receiving behavioral health services, particularly in terms of coordinating care with behavioral health community partners. Maintain up to date notes and files per client event and interaction. Provide consultation to and coordinate care of patients with health center primary care staff. Identify, refer, and advocate for patients needing specialty behavioral health service, and other services as needed. ESSENTIAL DUTIES AND RESPONSIBILITIES Work as a member of the 16th and Burnside multi-disciplinary team to deliver comprehensive, client centered, and trauma informed integrated services. Actively participate in the integration of services and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines. Proactively identify opportunities for improvement within workflows and logistics. Participate in review of Key Performance Indicators; review and contribute to continuous improvement opportunities and plans. Build understanding of and appreciation for diversity among clients and support their connection to culturally responsive and culturally specific services as requested. Manage client flow, including but not limited to: Preparation for pre-visit huddle with clinical team, including chart review for immunizations, screenings, chronic conditions, mental health issues Preparing clients per standard workflow, including but not limited to: Collection, documentation and reporting of client's vital signs per medical protocols. Completion of preventive screening questions Medication reconciliation with clients Exam room stocking and setup pre-visit and clean-up post-visit Completing and documenting provider orders Assisting providers and care team in scheduling of medical referrals 2. Administer injectable medications per provider order, with appropriate documentation according to clinic protocols. Work either morning and mid-day or mid-day and evening medication pass with clients, as well as respond to PRN requests and maintain highly accurate records of client self-administration of medications in program MAR. Assist clients with following medication self-administration directions that they have received from medical providers and record accurate records of observations Report out to nurse and primary counselor any medication adherence concerns Do regular quality audits of the MAR in consult with Medical Staff Create new MAR and input new medications into the MAR throughout the month as indicated. Keep client medication bins organized and up to date. Coordinate as needed with medical staff on issues specific to methadone. Coordinate with providers to clarify medication orders Provide daily feedback to the medical team on observations of client medication concerns, MAR discrepancies, reported medication needs, etc. Facilitate medication self-administration in-room for clients on Isolation protocol Assist medical staff and clients in coordinating medical care for clients with providers outside16 x Burnside. Assist in securing appointments with outside providers, or contacting insurance plan, as needed. Assist medical staff in follow-up communication with outside prescribers regarding prescription issues. Assist in maintaining daily contact with clients who are temporarily out of 16 x Burnside due to a hospitalization and document care coordination in the EHR (EPIC) Assist in client education related to their medications and medication related issues Maintain medication room and supplies, under supervision of Nurse Supervisor Conduct weekly audit on controlled medications according to procedure and with another employee present and participating. Destroy unapproved/unused medication weekly according to procedure and record in destruction log. Conduct bimonthly inventory of medication room stock and restock as needed. Take point in all communications with pharmacy, including medication follow-up and reconciling faxes. Maintain and update as needed all program signage related to medications and medication pass Maintain medical/lab equipment and records in compliance with State and Federal regulatory requirements. Maintain and clean supply rooms, exam rooms and MA workstations. Receive, document and direct client telephone calls in a complete and timely manner according to protocols. 9. Participate in outreach to clients in order to encourage engagement in care and proactive health measures. Complete and document procedures per provider orders and clinic standards. Contact pharmacy with new prescriptions and refills. Maintain a clean, safe clinic environment, respecting scope of practice and clinic best-practice. Observe clients of the same gender as they provide urine drug samples, secure samples via chain of custody procedures. Maintain and complete appropriate data entry as it relates to urine drug screens, such as transcribing point of service results as well as lab results and LCMS or GCMS results. Ensure that urine drug screen procedure is trauma informed, and patient centered. Complete any care team duties delegated by the Nurse Supervisor. Attend all mandatory CCC trainings in a timely manner. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
