Iyah comes to Cityblock from Commonwealth Care Alliance (CCA) in Massachusetts where, as Chief Transformation Officer, he drove the design of an organization-wide effort to strengthen and scale CCA’s care delivery and health plan operations and strategy. Previously, as the founding Policy Director for Care Delivery Innovation and Investment at the Health Policy Commission, he led the building of a $160 million statewide healthcare transformation investment program. There, and in his earlier role at the state Department of Public Health, Iyah contributed significantly to Massachusetts’ healthcare reform efforts, in particular the advancement of payment system and care delivery redesign. He has served as a member of the National Advisory Committee on Transforming Complex Care, was honored as one of Modern Healthcare’s Up & Comers in 2016, and was affiliate faculty at Dr. Atul Gawande’s Ariadne Labs. Iyah studied Biology at Brandeis University and medicine at Boston University. He is usually found with his wife and two kids, either chasing garbage trucks in Brooklyn or sailing.

Prior to Cityblock, Toyin served as Chief Medical Officer of Commonwealth Care Alliance. In this role, she led all clinical operations; spearheaded care delivery innovations; and oversaw multi-disciplinary teams of more than 500 clinicians, community health workers, and administrators, serving more than 20,000 beneficiaries across the state of Massachusetts. Toyin received her undergraduate degree from Stanford University; an MPhil from the University of Cambridge; and her medical degree, with Distinction in Clinical Practice, from King’s College London School of Medicine. Having completed her residency training at Boston Medical Center, Toyin is a board-certified family physician and continues to practice primary care and hospital medicine focused on patients with chronic, complex, and end-of-life needs. Internationally recognized as a leader in care for populations with complex needs, Toyin advises organizations such as the Center for Health Care Strategies, the Commonwealth Fund, and Institute for Healthcare Improvement.

Bay Gross is a Co-founder, Head of Product at Cityblock Health and Co-Founder, Chief Product Officer & VP Strategy at Cityblock Health and is based in New York City, New York. He has worked as Product & Incubation at Sidewalk Labs; Rails Developer at Yale University Careers; and Dev, founding team at Amicus HQ. Bay works or has worked as Co-founder at Yale University Careers. He studied at St Mary's High School in Medford Oregon and Yale University between 2013 and 2013.

Andrea Blankmeyer is a CFO at Cityblock Health and is based in New York City, New York. She has worked as VP Finance at SoFi, CFO at Transfix, and Global Markets Summer Analyst at Deutsche Bank. Andrea studied at Harvard University between 2005 and 2009 and Harvard Business School between 2014 and 2015.

Melanie has spent her career focused on transforming care for complex and costly populations in the Medicaid and Medicare programs. Before Cityblock, Melanie served as the first Director of the Medicare-Medicaid Coordination Office at the Centers for Medicare and Medicaid Services (CMS). Under her leadership, CMS designed and launched demonstrations across the country to test new delivery system and payment models for Medicare-Medicaid enrollees in both community and institutional settings, caring for more than 400,000 lives. Prior to that, she was the Medicaid director for the state of Indiana, and then the senior vice president for policy and operations at the Center for Health Care Strategies. There, she led the organization’s efforts to integrate care for complex populations and directed a unique leadership training institute. Melanie earned a master’s degree in business administration from Harvard Business School, and a bachelor's degree from DePauw University.
