Job Description Job Responsibilities:

* Identifies which patients in the specialty care practice have ongoing care coordination needs for their specialty condition.

* Outlines the nature and duration of involvement needed by the specialty care team and specialty care coordinator then identifies the primary care team involved.

* Utilizes assessment skills and risk assessment tools to identify patients with actual or potential care needs that would require care coordination.

* Conducts targeted outreach to a defined panel of high risk patients (chronic illness, lack of social support, readmissions, ED visits, surgical episodes, etc.) to ensure timely and efficient care delivery across the continuum of care.

* Utilizes technological tools (registries, patient lists, care team tab, etc.) to manage populations.

* Conducts comprehensive clinical assessments that include disease-specific, age-specific, medical, behavioral pharmacy, social and end of life needs of each patient.

* Informs the patient and family regarding coordination of their care and shares this information with the healthcare team.

* Works collaboratively with interdisciplinary team to develop goals and plan interventions to maximize patient outcomes.

* Monitors patient compliance with plan of care.

* Performs reassessments regarding patient progress toward goals and updates plan of care as appropriate.

* Ensures care gaps are closed around specialty disease/chronic disease/surgical episodes.

* Serves as primary patient contact for team related to condition/surgical episode and facilitates access to services.

* Coordinates members of the patient care team.

* Serves as the liaison between patients, families, and physicians, clinical staff by advocating for patient and families then responding to and facilitates resolution of patient/family questions and concerns.

* Assists in managing transitions of care across care setting, ensuring optimal communication and planning. Identifies barriers to receiving care and facilitates solutions.

* Partners with other care coordinator teams such as primary and transitional care social work, rehabilitation, pharmacy, palliative care and others.

* Defines and ensures compliance with disease-specific care paths for specialty care or chronic disease.

* Works with the patient and family to assess current knowledge, health literacy, and readiness to change, utilizing teach back to assess level of knowledge.

* Coaches patient and family on self-management support; including setting long and short term goals.

* Educates about managing a specialty or surgical condition (inclusive of preoperative, perioperative, postoperative and recovery) inclusive of prevention and health maintenance tasks. Educates and connects to other care providers and community resources to enhance care.

* Works with practices on quality and process improvement initiatives.

Education:

* Graduate from an accredited school of professional nursing.

BSN preferred

Certifications:

* Current state licensure as a Registered Nurse (RN).

* Specialty certification preferred.

Basic Cardiac Life Support (BLS) required

Senior Investment Analyst - Stakeholder Engagement

The Senior Investment Analyst will work with the Chief Investment Officer (CIO) and Managing Directors to monitor and shape investments in multiple asset classes, as well as make recommendations on and oversee a small portfolio of managers or a grouping of managers. Working in a fast-paced environment, supports the team in financial modeling, trading and rebalancing the portfolios, performing economic analysis, and other projects. Senior Investment Analyst - Stakeholder Engagement

The Senior Investment Analyst - Stakeholder Engagement position is an integral part of the Cleveland Clinic Investment Office (CCIO) and acts at the directive of the Chief Investment Officer and Principal of Stakeholder Engagement & Strategic Investments. The Senior Investment Analyst works as a cross-functional investment team member responsible for developing and executing stakeholder engagement strategies tailored to the Investment Subcommittee, Finance Committee, Executive Leadership, donors and various other internal and external stakeholders. The role also works to form actionable investments in multiple asset classes as part of CCIO's generalist model.
Key Job Responsibilities:

* Assists in developing and executing the engagement strategy of the Investment Subcommittee, Finance Committee and other internal and external Cleveland Clinic Foundation (CCF) stakeholders through deep understanding of CCF's various investment portfolios and investment managers, relevant capital markets, investment trends and governance priorities.
* Develops thoughtful, high-quality, stakeholder-centric presentation materials with engaging narratives that maximize stakeholder understanding, appreciation and ownership of CCF's investment management practices.
* Orchestrates Investment Subcommittee onboarding, educational opportunities and strategic off sites.
* Orchestrates CCIO's semiannual offsite, working collaboratively across CCIO to conduct a holistic investment portfolio review and to identify primary goals and objectives for future evolution of the investment portfolios and CCIO.
* Forms actionable investments in multiple asset classes as part of CCIO's generalist model.
* Serves as a respected ambassador for CCF and CCIO at manager meetings, conferences and industry events.

Responsibilities:

* Participate in idea generation, market research and analysis activities that create actionable investments in multiple asset classes.
* Build financial models to adequately assess and support the underwriting of investment themes and individual managers from both a risk and return perspective.
* Make recommendations to add or remove investment managers after significant due diligence, as well as maintain relationship for the manager once added to the portfolio.
* Compile market mapping, overviews and assessments of potential equity, fixed income, hedge fund, private capital and real asset managers, as well as economic and market analyses.
* Maintain quantitative and qualitative manager performance activity and holdings within the Clinic's internal database.
* Collaborate with the risk management team at the inception of a new investment to develop elevated governance and oversight and agreed to boundaries and exit strategies for individual investments and/or investment themes. Evaluate risk on an ongoing basis for existing investments.
* Prepare internal monthly and quarterly manager and/or portfolio reports detailing performance, exposures, concentrations and other portfolio characteristics.
* Maintenance of presentations summarizing changes in the securities markets across asset classes
* Collection and preparation of data related to current portfolio positioning and asset class valuations in support of monthly portfolio rebalancing discussions
* Monitoring of asset class valuations and data for further research into potential new investment opportunities
* Support Research/Strategy Team to develop, shape and monitor investment themes
* Preparation of quarterly reports and presentations for CCF executive team and other CCF stakeholders
* Support annual detailed analysis of portfolio changes and investment decisions
* Participate in idea generation, market research and analysis activities that create actionable investments in multiple asset classes as a part of the Investment Office generalist model
* Other duties as assigned.

Education:

* Bachelor of Arts or Bachelor of Science in Economics, Social Sciences, Mathematical Sciences, Engineering, Computer Science, Business or other related field required.
* Masters of Business Administration or Master of Science in Business, Finance, Economics or Mathematical Sciences, Data Analytics or other related filed required.
* Strong record of academic achievement preferred.

Certifications:

* Chartered Financial Analyst (CFA) or Chartered Alternative Investment Analyst (CAIA) preferred.

Complexity of Work:

* Within scope of job, requires critical thinking skills, decisive judgment and the ability to work with minimal supervision.
* Must be able to work in a stressful environment.
* Proficiency in Excel, PowerPoint, Word, Adobe Acrobat, Factset, Bloomberg.

Work Experience:

* Minimum 5 years of relevant professional experience in Investment Banking, Management Consulting, or Asset Management.

Preferred Languages:
