Job Summary
AmeriPharma is seeking a dedicated RegisteredNurse (RN) with expertise in home infusion therapy to join our team. This role combines infusion therapy and patient education, providing both in-home care and valuable support to patients undergoing treatments such as TPN, IVIG, and IV biologics.
The RN will be responsible for providing telephone and in-person advice and education to patients. This role requires offering interventions, counseling, and support services that require significant nursing knowledge, judgment, and skills in psychological, biological, physical, and social sciences. Under the supervision of the Director of Nursing, the RN will make decisions based on their education, nursing experience, and clinical guidelines, ensuring optimal compliance and persistence in home infusion programs.

Duties and Responsibilities
Conduct in-home infusion therapy visits with patients according to the plan of care.
Prepare clinical/nursing notes and document patient interventions in both clinical and non-clinical records consistently.
Act as a clinical resource, working collaboratively with other pharmacy departments.
Maintain knowledge of drug side effects and interactions.
Offer telephone and in-person patient education on therapy protocols, drug side effects, and home infusion procedures.
Ensure patients understand their therapy plans, medications, and the importance of adherence.
During initial and ongoing consultations, serve as a patient liaison between the patient, their family, and the pharmacy to ensure a seamless therapy process.
Perform comprehensive, initial, and periodic head-to-toe assessments for patients and initiate preventative and rehabilitative care.
Use health assessment data, and input from the agency team, the physician, the patient, and the family to determine patient needs.
Manage patient and family expectations regarding services, outcomes, and discharge goals.
Establish primary and secondary diagnoses based on patient assessment and home health care focus.
Develop care plans with skilled interventions, medical supplies, and necessary services to meet goals.
Regularly evaluate patient progress, in collaboration with team members, and revise care plans as needed.
Perform other duties as assigned.
Qualifications
Graduate of an accredited nursing school with a current RN state license.
5+ years of experience in home infusion and infusion centers.
3+ Experience with IV Immunoglobulins and IV Biologics. CRNI certification(Certified RegisteredNurse Infusion) is preferred.
Experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients is preferred
Effective interpersonal, time management, and organizational skills.
Strong written and verbal communication skills.
Organizational ability to maintain consistently accurate records.
Ability to direct information to the appropriate healthcare provider.
Proficient in computer skills, including word processing, and efficient use of the internet and email.
Current CPR and BLS (Red Cross and AHA) certification, valid driver's license, reliable transportation, and automobile insurance.

Initiates, develops, and implements basic nursing plan of care treatments, evaluating patient progress towards goals. Participates in the provision of direct patient care, performs treatments, administers medications, and educates patients and families/caregivers. Modifies plan of treatment in response to changing patient status based on physician orders to achieve established or revised patient care goals.
Assesses patient needs and physical status at each visit through health data access and patient interview. Re-evaluates patient needs through physical reassessment, response to therapy, and supplemental physician orders. Obtains and clarifies physician orders for plan of treatment revisions, informs physician promptly of significant changes in patient's condition, and provides written summary to physician within supplemental order.
Provides effective and safe teaching using patient-centered care approach for patient and family to achieve independence with prescribed therapy and care needs through active participation per plan of treatment goals.
Effectively and timely communicates with Naven Health's Clinical team, medical providers, patients, and families/caregivers to facilitate continuity of care. Prepares clinical documentation in real-time during visits and submits to the nursing department to comply with established timelines for billing optimization. Completes all documentation legibly and applies approved abbreviations and documentation error correction practices per Naven Health's policy.
Under close supervision, coordinates discharge planning and prepares discharge summaries with patient instructions and thoroughly reports patient care needs, progress and goals when transferring care.
Demonstrates compliance with agency operations, Naven Health's policies and procedures, professional standards, local, state, federal regulations/guidelines, and accreditation standards.
Maximizes work efficiency through the use of computers and other automation technologies to validate plan of treatment orders, communicates patient care provided, and follows assigned visit schedule.

The Clinical Coordinator assists the Manager and/or Director in providing leadership and management within the assigned department(s). JOB SUMMARY: Provides quality patient care according to established standards of nursing practice and Rimrock's policies. Assures adherence to state statutes and rules, CARF standards, and federal regulations including HIPAA and 42 CFR Part 2. Positively represents the organization and promotes Rimrock's Mission to the community.
ESSENTIAL FUNCTIONS:
Completes nursing assessments on all new patients, formulates nursing care plans, provides report on patient to provider, and charts in electronic health record.
Administers medications as ordered.
Makes frequent rounds for purposes of observing, assessing and meeting patient needs and conditions in the Medical Unit.
Review all LPN assessments.
Charts the progress note on patients' complaints, nursing observations and condition changes, including incident reports and behavior forms.
Reports to the Director of Patient Care any pertinent nursing or medical information regarding patients.
Reports the condition and status of all patients to the on-coming nurse.
Counts controlled drugs at the beginning and end of shift.
Participates in continuing education and in-service education programs for the improvement of patient care and own professional growth and development.
Administers TB tests, CIWA scales and BAL's to all applicable patients.

EDUCATION/TRAINING: Graduated from an accredited nursing program and received nursing diploma. Currently licensed as LPN or RN in the State of Montana. Current CPR training and First Aid.
SKILLS: Knowledge and understanding of current nursing theory and practice and ability to perform accordingly under the supervision of the Director of Patient Care and/or Medical Director; understanding of Rimrock's policies related to patient care; knowledge of limitations in nursing practice and ability to request assistance of supervisor when needed; ability to establish appropriate rapport with chemically dependent patients; understanding of chemical dependency therapy process and ability to function as a member of an interdisciplinary treatment team. Computer skills and proficiency with Microsoft software including Word, Excel and Outlook.
PHYSICAL DEMANDS: Work is indoors in an office environment with moderate noise. Intermittent physical effort involving lifting of up to 25 pounds, walking and stooping is required. A typical workday involves sitting, frequent use of a keyboard, reaching with hands and arms, and talking and hearing, approximately 70% of the time. Approximately 30% or less of the time is spent standing. Normal vision abilities required, including close vision and ability to adjust focus.
WORK ENVIRONMENT/JOB LOCATIONS: Involves everyday risk or discomfort requiring normal safety precautions in an office setting, meeting room and patient care areas. Work may involve mental and emotional stress. Some driving required. Will work primarily at Main Facility.
