Responsibilities:
Primary responsibility is to be available and flexible to float to various sites across the Legacy Health System and provide patient care as needed.
Functions as an integral part of the health care team, collaborating with and under the direction of a clinical manager or designee, in alignment with the Standards and Scope of Practice for RegisteredNurses.
The RN Resource Team is responsible for the assessment, analysis, planning, implementation, and evaluation of patient care, following Legacy Health's established patient care standards, including protocols for patient assessment and safe transfer.
This position is compensated at a flat rate, with additional differentials applied based on work conditions (e.g., shifts, weekends, holidays, etc.).
Qualifications:
Education:
As required by licensure.
BSN or degree in progress strongly preferred.
BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO.
Experience:
Three years registerednursing experience required.
Additional clinical experience requirements may vary dependent upon the specific department in which the position is utilized:
Critical Care: Three years critical care nursing experience required; the three-year requirement may be met for current Legacy employees who have a minimum of two years adult critical care experience plus one year emergency department, intensive care or pediatric critical care experience. ACLS required.
Knowledge/Skills:
Demonstrated ability of positive interpersonal and communication skills with colleagues, patients, families, physicians and the community.
Keyboard skills and ability to navigate electronic systems applicable to job functions.
Licensure:
Both Oregon and Washington state RN licensure required.
BLS for Healthcare Providers from the American Heart Association required.
ACLS from the American Heart Association required.

The Charge Nurse, under the direct and indirect supervision of the Nursing Unit Coordinator, Assistant Coordinator and/or Hospital Supervisor facilitates and assists with all levels of nursing care being provided to a group of patients within one nursing unit during the period of one working shift. In addition to these responsibilities, the Charge Nurse frequently assumes all or partial staff nurse responsibilities. The Charge Nurses are directly accountable to the Nursing Coordinator for the unit. The Charge Nurses assist the staff nurses with day to day problem solving, communication between shifts, and may serve as a link between staff nurse and the Nursing Coordinator.Essential Responsibilities:
The Charge Nurse will assist in the supervising of nursing personnel: Assist in the interviewing process of applicants for posted positions and makes recommendations to the Coordinator. Support organizational goals and objectives. Provide input into staff evaluations. Monitor performance of staff and provide documentation of performance problems to the Coordinator. Provide input into the development and maintenance of staffing patterns for the patient care unit and helps to monitor day-to-day staffing and utilization of nursing resources. Coordinate staff meetings under the direction of the Coordinator.
The Charge Nurse will contribute to overall functioning of the Unit by monitoring resources. Identify and communicate to the Coordinator systems problems that interfere with effective client care and convey risk situations to the Coordinator. Participate and give input into trend analysis for the operational budget. Assist with materials management, ensuring availability of supplies needed. Provide direction during emergency situations as delegated.
The Charge Nurse will actively participate in providing and/or facilitating client care. Participate in the development and implementation of standards of client care, policies and procedures and protocols. Coordinate and participate in the delivery of patient care on a specific unit. Utilizing the nursing process: 1. Assess need and priorities of patient. 2. Assist in the implementation of nursing care. 3. Evaluate effectiveness of nursing care. Participate in the development and ongoing monitoring of quality assurance activities. Provide input into and participate in development and achievement of departmental goals and objectives. Participate in multi-disciplinary patient care conferences and discharge planning activities as appropriate. Identify and assist in problem-solving both intrashift and intershift. Keep Coordinators Hospital Supervisors informed of problems/resolutions. Function within expected behavior standards of Staff Nurse as participating in role.
The Charge Nurse will function as an educator for other health care professional and clients. Assists in development and presentation of in-service programs. Participates in clinical orientation to the nursing unit. Participate in the planning and provision of education/teaching to patients and their families. Serve as a resource to other departments in area of specialty.
The Charge Nurse will participate in professional activities for self and others. Support departmental and organizational philosophy, goals, objectives, policies and procedures. Participate in meetings, special projects, and committees as designated. Support and/or participate in research programs approved by the Department of Nursing as designated. Promote interdepartmental and intradepartmental communications. Foster good public relations. Assume responsibility for own continuing education and development.
The Staff Nurse will apply knowledge of organizational expectations of personnel policies and procedures. Follow appropriate policies and procedures for use of time in work environment, including absenteeism and tardiness. Monitor and take corrective action to prevent recurring absences. Return promptly from breaks and schedule them in concert with co-workers to provide appropriate coverage of unit and client care activities. Consistently on time and ready to work at start of shift and has assignments completed at end of shift. Maintain confidentiality of all client and organizational records. Release information according to established policy/procedure. Demonstrate a positive attitude while at the work site. Does not conduct personal conversations when members are waiting for service. Does not carry on discussions regarding other staff members, the organization, or medical care in public areas. Restricts personal business to non-client care areas during bonafide break times. Identify self by wearing a name tag, and verbally introducing self by name, title and/or department. Address members with courtesy and respect. Provide assistance to members or connects member with appropriate person when needed. Apply same courtesy standards to members on the telephone as in person.

PURPOSE AND SCOPE:

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Provide day to day guidance, support and direction to direct patient care staff, providing informal feedback on an ongoing basis and formal feedback input for the annual performance evaluation. Participate in the department staffing and the appropriate hiring, firing and disciplinary actions. Recommend disciplinary action to Facility Administrator and initiate as appropriate.
Ensure compliance with all company and facility approved procedures and policies as well as regulations set forth by state and federal agencies.
Approve or disapprove time or personnel schedule changes in the absence on the Facility Administrator ensuring compliance with applicable regulations, policies and procedures for documenting time of work hours for staff members on assigned shift.
Participate in patient care plan meetings.
Maintain knowledge in the current practices related to the principles and techniques of dialysis by participating in all scheduled in- services.
Train and orient staff as necessary.
Routinely observe and guide direct patient care staff for appropriate technique and adherence to facility policies and procedures.
Promote and assist with compliance to OSHA programs in order to maintain a safe and clean working environment.
Maintain overall shift operation in a safe, efficient, and effective matter.
With Facility Administrator conduct staff meetings at least monthly or as needed to keep patient care staff informed of changes in patient care needs or operations to improve delivery of care.
Meet routinely with the Facility Administrator to discuss personnel and patient care status, issues, and information.
Supervise all documentation of patient information.
Coordinate Charge Nurse duties with Staff RNs acting in the relief charge capacity.
Assess daily patient care needs and develop and distribute patient care assignments appropriately.
Assume primary responsibility in an emergency situation.
Assess patient needs, respond to dialysis treatments, and communicate concerns to rounding physician. Implement changes in patient care/treatment as directed.
Monitor and supervise all patient care activity during dialysis and assist as necessary.
Collaborate with direct patient care team in making decisions to benefit patient care.
Continuously monitor patient's condition with regards to problems and potential complications associated with dialysis.
Administer medications to patients per physician's orders.
Act as the subject matter expert and as a resource for staff members.
Supervise and participate in completion of short and long term care plans.
Admit new patients according to facility procedure.
Ensure educational needs of patients are met and educate the patient and family about End Stage Renal Disease, dialysis therapy, diet and medication.
Supervise the safe and effective use of all equipment involved in direct patient care.
Operate all dialysis related and emergency equipment safely and efficiently when needed.
Perform required testing and verification and initial the checklist for start-up and shut-down procedures as outlined in the Technical Services Manual.
Complete Nurse's Technical Training Program/Water Quality Facility
Assist with special projects or other duties as assigned by the Facility Administrator
Assist with the interviewing of potential direct patient care staff
Promote efficient use of medical supplies.
Attend and participate in monthly Quality Assurance meetings.

What You'll Do:
Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
Provide regular visits to re-assess the patient's needs and update the plan of care
Perform clinical pronouncement of death and provide support to the family
Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
Qualifications:
Graduate from an accredited registerednursing program
Licensed as a registerednurse in the state of practice
Must possess current CPR, First Aid Certification
Must be knowledgeable of quality assessment and assurance procedures
Valid driver's license and proof of insurance is required

Legacy Meridian Park Medical Center is the South Metro area's full-service medical center, with the friendly feel of a community hospital and the skill and services of a much larger medical center. Among our specialties: stroke, chest pain, minimally invasive and robotic surgery, cancer care and orthopedic and total joint surgery. Plus a birth center and breast health center.
Responsibilities:
Responsible for patient care within an assigned department/unit and participates as a member of the health care team in cooperation with and under the direction of the clinical manager or designee as defined by the relevant Standards and Scope of Practice for the RegisteredNurse. The RN Staff Nurse assesses, analyzes, plans, implements and evaluates patient care following established patient care standards for Legacy Health System including those for patient assessment and transfer.
Qualifications:
Education:
As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO.
Experience:
One-year registerednursing experience or completion of either the Legacy Health RN Residency program or a nursing program that is equivalent in content, curriculum and duration. Additional clinical experience requirements may vary dependent upon the specific department in which the position is utilized.
Additional QUualifications:
AHA BLS required
AHA/AAP Advanced NRP is mandatory (for initial or upon renewal). May be hired with AHA/AAP Essential NRP, but must obtain Advanced NRP within 6 weeks or completion of orientation, whichever is longer. Travel nurses may continue to have Essential NRP if not performing in Charge, Neonatal Resuscitation, Baby, or NICU RN roles.
If recovering OB or Surgical patients AHA ACLS within 6 months of hire.
STABLE within 6 months of hire.
Advance Fetal Monitoring Course within 6 months of labor training.
Ability to Scrub or scrub training within 6 months of hire (specific for Legacy Good Samaritan Hospital).
Refer to Womens Education Plan for ongoing competencies.
Knowledge/Skills:
Demonstrated ability of positive interpersonal and communication skills with colleagues, patients, families, physicians and the community.
Keyboard skills and ability to navigate electronic systems applicable to job functions.
