For NP/PA: Approximately $121,000 - $167,000 per year. All individual pay rates are calculated based on the candidate's experience and internal equity.

Overview of the Role:
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Communicate the organizational vision and strategy and assists with maintaining focus on the strengthening of our team-based advanced primary care model.
Partners with VP of Medical Services to oversee the strategic clinical planning and population health management.
Demonstrates effective leadership through strategic focus, active listening, coaching and collaboration at all levels of the organization.
Presents relevant topics at various leadership meetings.
Expands and maintains rapport with the medical community, health agencies, local governments, and community organizations across the continuum of care.
Serves as the clinical lead for the following areas:
Medical Quality, Patient Satisfaction, Safety and Clinical Risk program (AAAHC PCMH accreditation, CLIA program, NCQA PCMH certification), primary care medical quality committees, electronic health record (EHR), business intelligence team providing clinical expertise to optimize functionality of program and tools, and clinical quality measurement system including report and measure design.
Analyzes data, in partnership with quality, to solve problems and manage population health.
Develops clinical decision-making tools, including the CarePlanner, and evaluates effectiveness.
Leads care team workflow and clinical quality improvement initiatives.
Creates and maintains medical care related policies, protocols, guidelines, and procedures.
Must complete credentialing and privileging processes within assigned timelines.
Identify new service lines that align with the CFHW strategic plan.
Manage schedule for off-hours medical coverage
Provides direct patient care as outlined in the NPPA or MDDO job descriptions for half of the FTE.
Provides input into the annual clinical services budgeting process.
Supports leadership in responding to patient complaints related to informatics and clinical decision-making tools.

You will triage and collaborate with outside healthcare providers and Clinica Family Health & Wellness (CFH&W) staff.
Job Summary:
You will provide skilled health care to clients while addressing, referring and clarifying medical concerns including administration and education regarding medication and a range of health issues.
Essential Functions:
Ensures continuity of psychiatric care through provisions of services including discharge and referral
Triages healthcare situations to determine needed level of intervention in crisis situations
Verifies and carries out prescriber orders
Records changes in medications, laboratory tests and prescriber recommendations
Communicates current psychiatric medication orders, missed medications or other pertinent psychiatric information with clients, prescribers and nursing staff, clinicians, and family members as appropriate
Administers medications, including injections, and educates clients regarding medications (risks, benefits and side effects and medication compliance)

As our Behavioral Health Tech (BHT) you will provide direct care and support to individuals with mental health and/or substance use disorders on a multidisciplinary team within our Acute Care Services (ACS) team. You will assist with identifying appropriate ACS services and enrolling clients into the best program that meets the client's needs. You will use crisis de-escalation skills to assist with unregulated clients to ensure client and staff safety.
You will perform client registration into the appropriate behavioral health services, including crisis and urgent psych, within the ACS network. You will refer clients who appear to be intoxicated or in substance withdrawal to a SUD specialist to assess for appropriate level of care. You will also serve as the contact point, advocate, and informational resource for clients, treatment team, family/caregiver(s), and community resources.

Welcome clients to the Clinic and assess client needs.
Complete client registration and program enrollment(s).

You will be a coach and supervisor for the Medical Assistants who work on the pod care delivery teams in collaboration with the Clinic Medical Assistant Manager and Clinic Director, providing the highest level of quality medical care, health education and preventive services. You will provide workflow coordination and subject matter expertise to MAs and the care team. You are self-motivated and assist the provider/MA teams to provide pro-active clinical support, workflow, and team and patient-centered care.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Provide day-to-day direct supervision/coaching of Medical Assistants, including hiring/firing authority in conjunction with the CMAM and performance appraisals
Manage pod/patient flow for all provider or MA visit types
Manage tasks and patient/provider follow up with assistance from team members
Oversee patient prep and room set up for clinical care
Accomplish orientation, education and training of new clinical pod staff in clinical support activities, in partnership with Clinic Medical Assistant Manager (CMAM)
Supervise proper routine cleaning and storage of medical equipment
Ensure the stocking and audit of exam rooms is standard and complete

Performs point of care testing like rapid strep, urinalysis, hematocrits, hemoglobin A1Cs, random blood sugars, urine drug screening, and fecal occult testing
Gives and reviews patient care plans
Refills medications and prior authorizations as directed by the provider and guideline
Performs MA visits that support pod flow
Completes tasks and other follow-up as needed for the team
Stocks exam rooms and makes sure rooms are cleaned after each visit
Helps with opening and closing labs, including autoclaving and packaging of instruments
Prepares appropriate set-up for procedures according to protocol and needs of provider
Demonstrates the ability to effectively prioritize and be flexible to the needs of the pod
Collaborate with CMAM and CD regarding Pod supervision and with Nursing Services Program Manager- MA.

The Justice Services Jail Re-Entry Therapist is responsible for the provision of therapeutic interventions for persons referred from justice organizations to Clinica Family Health & Wellness.
Develop and carry out a consistent plan of treatment, from intake and assessment to closure. Coordinate treatment with legal entities including probation officers. Participate in staffings with internal and external partners to coordinate care.
Essential Functions:
Provide a variety of case management services, including managing Social Security payeeships, assisting clients to apply for various types of public assistance and providing supporting documentation, attending multi-agency clinical case consultations, and facilitating referrals to other organizations.
Participate in internal consultation and supervision with team members and other professionals in the organization.
Pursue professional growth and development by seeking out learning opportunities and integrating new learning into daily work practice.
Maintain working knowledge of Evidence Based Practices, current Co-Occurring Disorder treatment recommendations and trends in community mental health, including health care reform.

- Jail Re-Entry
Specialized ongoing caseload comprised of clients currently in custody in Boulder County Jail requiring therapeutic treatment.
Develop plans of transition for clients to successfully transition back into community from jail/prison while achieving mental wellness.
Provide mental health and co-occurring disorder consultation to assist with integration of mental health and substance use services within the court system.
Work collaboratively with internal and external partners such as: Jail Programs, Probation, Public Defender, District Attorney, law enforcement, monitoring agencies, and other treatment providers to assist clients in their recovery process and to ensure the continuity and quality of care.
Provide case management services to support connections to resources in coordination with the Jail Re-Entry Peer Support Specialist and Justice Services Case Manager.
Attend regular staffings with community partners, including as needed meetings in criminal justice settings such as court or jail.
Facilitate contact with active clients of CFHW who are incarcerated in the Boulder County Jail.
SUD
Provide mental health and co-occurring disorder clinical services, using best practice standards.
Knowledge of motivational interviewing, stages of change theory, stage-wise treatment strategies to intervene and motivate clients.
Knowledge of psychotropic medications and their side effects and contraindications with drugs of abuse and report any relevant observations to medical personnel.

What We Need:
Bachelor's Degree in Psychology, Social Work, Counseling or a related field OR one-year experience in the behavioral health field for non-related Bachelor's degrees
Experience working with adults in an outpatient setting, interest in working with adults with mental illness and substance use issues
Experience with clinical modalities: Motivational Interviewing (MI) and Dialectical Behavior Therapy (DBT), other Cognitive Behavior Therapies (CBT)
Collaborative and client centered orientation
Manage a caseload of clients and their diverse needs
Collaborative team member who is ready to jump in, support our clients and team
Community outreach experience and interest, occasional ability to transport clients to connect to care
Familiarity with insurance programs like Medicare and Medicaid

As our Justice Services Assessment Specialist, you will provide initial assessments for our Justice Services team for clients transitioning back into the community. You will work on a multidisciplinary team and liaise with community partners in order to streamline access to services at Clinica Family Health and Wellness.

What we need for this job:
Masters in Psychology, Social Work, Counseling or related field, plus 1+ year internship, and one (1) or more years experience in a clinical setting
Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC) or full licensed acquired within twenty four (24) months (Licensure Preferred).
