We treat each person with compassion and human dignity, which leads the homeless community to trust our team and rely on them for help.” ~ Boulder Community Member Our goal is to meet people where they are and build trusting relationships over time that encourage and empower people experiencing homelessness in our community to ultimately establish care in one of our Health Homes and access the benefits of our integrated health clinics. You will provide care, education and resources for our patients, and you will be an important and visible team member in the community. We are urgently hiring for a Registered Nurse and we'd love to have you on our team. Job Profile: Are you passionate about building deep relationships with vulnerable people to provide care and coordination with dignity? Do you build strong relationships with both patients and community partners? Do you enjoy the challenge of identifying possible opportunities to expand the reach of who we are encountering in our communities while re-envisioning an outreach program? Are you experienced in clinical care and acute care needs while working independently in the community? Do you desire to leave a mark on the community you serve in a tangible way? What You Get to Do: You will go out into the community, visit shelters and provide care where people experiencing homelessness are staying or accessing services. You will provide triage assessments that include providing health education, helping to coordinate care and trying to simplify navigation through a complex system. Build relationships based on safety and trust with people experiencing homelessness. You will attend meetings with community partners and represent the work of the outreach program and the values of Clinica to bring patients into the clinics. Compensation: Approximately $72,176 - $91,416 annually. All individual pay rates are calculated based on the candidate's experience and internal equity. What We Need to for this Job: Registered Nurse with a current Colorado license. An eagerness and willingness to accept and meet challenges. Genuineness and an ability to connect with people who are experiencing homelessness in a way that allows you to be seen as a safe and trusted partner. Being honest and truthful in how you are able to help them and following through on your commitments. A willingness to be creative and consider alternative ways to try to meet the needs of the person in order to coordinate their care. Perseverance to keep showing up even in situations where either the person or they system is not yet ready or able to meet the needs of the person. The ability to recognize when there is nothing else that can be done in that moment. One (1+) years' experience of care coordination for individuals and documenting workflow. Past experience with intensive casework and self-care required to show up as your best self for a challenging job. You get to do Co-visits where you will evaluate patients, in collaboration with the provider, and treat patients with acute needs to help create access for patients. You will triage any walk-in patients. You will build teamwork by attending huddles before the start of shifts to review any anticipated nurse needs during that shift. Compensation: Approximately $34.70 - $43.95 per hour. All individual pay rates are calculated based on the candidate's experience and internal equity.
