Job Summary
The CaseManager is responsible for facilitating and coordinating support services for students receiving mental health care through the University Counseling Center. This role ensures continuity of care, provides linkage to community resources, develops partnerships with campus community and local stakeholders, and supports outreach and training initiatives. The ideal candidate will bring a collaborative, student-centered approach to managing complex systems of care in higher education.
Responsibilities
CaseManagement and Coordination of Care 40%
* Conduct needs assessments and develops individualized support plans and referrals.
* Coordinate care between students, clinicians, and campus or community providers.
* Provide follow-up services and monitor student progress to ensure continuity of care.
* Manage a caseload of students requiring wraparound support, hospitalization follow-up, or long-term referral.
Community Partnerships and Referrals 20%
* Maintain a current database of referral sources and community resources, including mental health providers, hospitals, crisis centers, and social services.
* Build and sustain relationships with community agencies, treatment programs, and healthcare systems.
* Coordinate referrals for services not offered on campus, such as psychiatry, intensive outpatient programs, and housing or food security supports.
Outreach and Prevention 15%
* Collaborate on campus-wide mental health initiatives and prevention campaigns, including, but not limited to, CARE@GSW.
* Lead CARE efforts across the GSW campus community.
* Serve as the point of contact for the CARE team on campus.
* Participate in wellness-related programming.
* Help identify student populations with increased mental health needs and support targeted outreach efforts.
* Coordinate CARE@GSW referrals and follow-up services, ensuring timely connection between students, campus resources, and external providers to support holistic well-being and academic success.
Training and Consultation 10%
* Provide consultation to faculty, staff, and students on recognizing and responding to mental health concerns.
* Train student leaders, resident assistants, and campus partners on referral processes and crisis response.
* Collaborate in training Counseling Center interns or practicum students on systems-based care and referrals.
Administrative and Documentation 10%
* Maintain accurate, timely case notes and records in the electronic health record system.
* Track data on referrals, hospitalizations, follow-ups, and care coordination outcomes.
* Assist in the creation of annual reports, policies, and procedural updates related to casemanagement.
Professional Development and Collaboration 5%
* Participate in ongoing professional development related to casemanagement, college mental health, and equity in service delivery.
* Attend staff meetings, case consultations, and interdisciplinary planning efforts within Student Affairs.
Required Qualifications
* Bachelor s degree in Social Work, Psychology, Counseling, Public Health, or related field.
* Experience in casemanagement or care coordination within mental health, medical, or educational settings.
* Strong communication, organizational, and advocacy skills.
* Demonstrated ability to work effectively with diverse student populations.
Preferred Qualifications
* Master s degree in Social Work, Counseling, or related field.
* Familiarity with HIPAA/FERPA, Title IX, and other considerations in higher education settings.
* Previous experience in a college/university setting.
