BASIC FUNCTION: Performs a variety of administrative functions. Process and maintain consumer files and data base, ensure consumers files are in compliance with State and Federal Regulations
ESSENTIAL DUTIES AND RESPONSIBILITIES
Primary contact for Consumers/Legal Representatives/Guardians and family members
Prepare documentation and attend Consumers initial and annual assessments
Process New Consumer intake documentation
Request Verification of Diagnosis (VOD) from consumers doctor(s)
Request/Process Consumer supplies per request to venders (Geneva Woods ext.)
Request/Process Consumer products such as Life Line Services
Fax completed documentation to the State of Alaska to process information
Complete Consumers sample timesheet (PCA/Waiver) for signature
Prepare Personal Care Assistant Tool (PCAT) signature page/fax to the State of Alaska
Assist Consumer with hiring/terminating Personal Care Assistant(s)/Caregivers
Participate in consumers Fair Hearings
Write and process expedited assessments and amendments
Process incoming and outgoing transfers
Schedule and attend Home Visits with Consumers two times per year
Facilitate Consumer meetings ensuring that all necessary program information, training and agency policy/procedures as reviewed as needed.
Ensure program compliance, safety procedures contracts, and State and Federal regulations.
Handle confidential and sensitive information with poise, tact, and diplomacy
Complete Critical Incident Reports/fax to the State of Alaska within 24 hours
Organize and prioritize a large volume of information and assignments, including paper and electronic filing through network server
Work independently and within a team on special and ongoing projects when necessary
Projects a positive image of the organization and conducts them self professionally at all times, when dealing with internal and external colleagues, consistent with the values and culture of the organization.
Perform all other duties as assigned
Assist Program Director, Executive Director, Care Coordinator Administrator, and CEO as needed.
KNOWLEDGE, SKILLS AND ABILITIES;
Strong interpersonal skills and customer service
Must have knowledge of the HIPAA laws of 1996 to maintain and safeguard confidential medical and personal records
Knowledge of various Microsoft software applications, such as Microsoft Word, Microsoft Excel.
Must be able to type 65 WPM
Multi-line phone, copiers and fax machines
Effective written and oral communication skills
Ability to consistently be at work and on time
Ability to maintain a confidential working relationship

QUALIFICATIONS:
Must have a high school diploma or GED with 2 years experience in the Healthcare Field
Must be at least eighteen years of age and have a valid Alaska Drivers License
Must be able to pass a Criminal Background Check
Must have a high level of interpersonal skills to handle sensitive and confidential situations.
Position continually requires demonstrative poise, tack, and diplomacy
Must be able to interact and communicate with individuals at all levels of the organization.
Must have knowledge of a variety of computer software applications, word processing, spreadsheets, database and presentation software (Microsoft Outlook, Word, PowerPoint and Excel)

The Work Hours
Full-time court employees are currently scheduled to work 37.5 hours per week. The work hours for this position are Monday - Friday, 8:00 a.m. to 4:30 p.m. with a one-hour lunch break.

Summary of Job Responsibilities:
The Southcentral Foundation (SCF) CaseManagement Support is responsible for performing the coordination and management of administrative duties for a Primary Care Provider (PCP) team as well as the wider Integrated Care Team (ICT). Working together with the core medical team the CMS is a primary contact between the customer-owner and ICT ensuring effective and efficient schedule coordination; verbal and written communication; information research and transfer; and system coordination.
This position has two (2) levels designed to provide progressively more responsible and independent work experiences. Progression between job levels is based on the demonstrated ability to successfully handle more progressively responsible assignments.
Qualifications:
SCF programs are established to serve a primary population comprised of Alaska Native people who are affiliated with Cook Inlet Region, Inc. Essential Job Functions
* When itinerant, works under the direction of the director of Intensive Behavior Supports. When site-based, works under the direction of the building administrator.
* Collaborates with teachers, counselors, and school psychologists.
* Collaborates with special education administration, Elementary School Division, Middle School Division, High School Division, and building administrators.
* Collaborates with the Special Education Department on behavioral programming for special programs.
* Participates in school-level data review meetings, transition meetings, FBA and BIP meetings, and other meetings determined by student need.
* Provides on-site instruction, hands on modeling, and coaching for staff who work with students who require additional intensive behavioral supports.
* Collaborates with teachers, counselors, and school psychologists on methods of instruction for skill acquisition, replacement behavior, and social emotional skills.
* Conducts FBAs in collaboration with school psychologists.
* Develops BIPs in collaboration with teachers, counselors, paraprofessionals, and school psychologists.
* Develops professional development trainings on behavior management, progress monitoring, data collection techniques, skill acquisition, replacement behavior, and social emotional skills.
* Collaborates with special education administration in the design and delivery of developed professional development trainings.
* Articulates understanding of evidence-based practices (EBPs).
* Develops and models the implementation of classroom wide or student specific data systems in alignment with Positive Behavior Interventions and Supports (PBIS).
* Participates in school level PBIS leadership meetings.
* Supports district level PBIS leadership team in trainings and classroom snapshots.
* Collaborates with teachers and psychologists regarding data review of BIP.
* Follows request for assistance process.
* Follows behavioral coaching process.
* Develops individual student behavioral training plan based on BST.
* Develops procedural integrity checklist (i.e., BIP implementation checklists).
* Conducts staff implementation checks during behavioral coaching process.
* Develops social validity surveys to ensure staff are accepting of the interventions written in the BIPs.
* Delegates data collection, individual student BST, and modeling of BIPs to behavior technicians.
* Ensures behavior technicians meet ongoing requirements as required by the BACB.
* Tracks the supervision hours required by the BACB for behavior technicians.
Physical /Mental Demands
The physical demands of this job require frequent standing, walking, sitting, speaking and hearing. Specific visual abilities are also required. The employee is regularly required to reach with his/her hands and arms and occasionally lift items weighing less than 40 lbs. Additionally, the employee must be able to understand vague and implicit instructions, be able to readily recall facts and details, handle conflict and make effective decisions under pressure. The employee must have the ability to effectively manage the stress of working with students, parents, and other employees representing diverse cultures, personalities, and work styles in a dynamic work environment.

Responsible for planning, coordinating, developing, and implementing a professional individual and group counseling program for assigned students. Responsibilities
Essential Duties & Job Functions:
* Provides structured classes for students in employability skills and emotional development. Classes will include grief/loss, handling interpersonal conflict and how to deal with homesickness, among others. Other classes will cover communication, self-care, and other skills focused on developing and improving student retention and success.
* Develop methods for obtaining feedback from staff and students on new topics and subjects needed for curriculum development.
* Maintain curriculum electronically and/or in hardcopy in an organized way. Develop lesson plans for each class. Groups/classes will be interactive to encourage strong student participation.
* Develop activities promoting interpersonal skills that can be easily incorporated into existing meetings or group sessions facilitated by non-counseling staff.
* Conducts periodic focus groups and provides feedback to appropriate entities; modifies programming based on student feedback.
* Provides daily communication to the Counseling Manager and other Counselors regarding any concerns for student issues revealed during evening hours.
* Maintain positive and effective relationships with students. Provide guidance for students to achieve a positive attitude, proper behavior and goal achievement.
* Coordinate and assist with any social development activities promoting self-skills and professional growth with dorm and/or recreation staff.
* Work with the Residential Living Manager to develop and execute a rotating schedule of presenting mini-sessions or activities at dormitory meetings.
* Perform all other related duties as assigned.
Job Requirements
Mandatory:
* Bachelor's degree in human services, psychology, counseling, education, social science, communications, or closely related field, OR,
* Associate's degree in human services, psychology, counseling, education, social science, communications, or closely related field, and two years related experience may be substituted for the Bachelor's degree. Experience may include successful Job Corps outreach and admissions experience or successful outreach and admissions experience with other youth development programs.
* Three (3) years' experience in sales, marketing, or counseling.
* Successfully pass background check and/drug test required on the contract.
* Current valid Driver's License and an acceptable driving record.
