Case Manager, Chemical Dependency Counselor Assistant (CDCA), will perform treatment planning, assessment, crisis intervention, individual and group counseling, case management, and education services as they relate to abuse of or dependency on alcohol and other drugs under the Program Director or designee. They will serve as an advocate and liaison for and on behalf of residents rights and benefits. They assist in helping each resident navigate treatment and support services, acting as an advocate and coordinating care.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Orients patients to the treatment programs operation and procedures.
Conducts alcohol and drug abuse counseling sessions on a regular basis per state regulation, or as part of the individual treatment plan.
Conduct weekly assessment of individual client needs and assist with a plan to address those needs.
Conducts intake interviews with clients to gather information about their substance abuse history, mental health status, and other relevant factors.
Based on the information gathered during assessments, they will help to develop treatment plans with clients that will address their unique needs and goals.
Develops, within (10) days of entry or as required by state guidelines, whichever is more stringent, a formal treatment plan documenting patient goals and updates utilizing the same guidelines.
Implements or revises patient treatment plans with the patient as required per state regulation and record a review of the patients progress.
Monitor clients' progress throughout their treatment and adjust their treatment plans as needed.
Provide counseling services to clients, either one-on-one or in group settings. They may help clients to identify triggers for substance use, develop coping skills, and work through emotional issues related to their addiction.
Participates in the Treatment team and documentation in EMR.
Assist patients in identifying, maximizing, and relating their strengths to appropriate social, educational (academic and vocational), and occupational goals.
Help clients to access social services, such as housing or employment assistance, that can support their recovery.
Helps patients prepare and adjust to a drug free existence, and to offer assistance (emotional) in problematic issues.
Maintains patient records that are complete, comprehensive and well organized. Assure documentation is completed per OMHAS, regulatory, and agency requirements following best practices.
Maintains a case load of patients, meeting all requirements for sessions and documentation.
Attend required meetings, including individual and group supervision, treatment team meetings, training, and community events as appropriate. Participate in at least one hour of supervision per week.
Evaluate, facilitate and monitor services and clients progress in relationship to established goals and objectives.
Work to obtain aftercare placements such as sober living, partial hospitalization, or outpatient placement for aftercare services.
Reviews and coordinates with the team and provider on decisions involving hospitalization, respite and transfers/level of-care changes. Implement plan of action. Maintains ongoing dialogue with intake regarding status changes.
Facilitate community resource availability based on client needs (e.g., day program, guardians) to ensure maximum social integration.
Complete thirty, ninety, and annual follow up calls.
Facilitates client benefits (e.g., SSI, Medicaid, SSB, Medicare).
Holds a basic understanding of alcohol/drug abuse and addiction. Additionally, keep updated on current drug abuse and treatment information, especially current trends in Opioid Maintenance Treatment.
Ensure compliance with the Ohio state regulations and ethical guidelines for CDCA practice.
Other duties may be assigned.
QUALIFICATIONS:
Must have High School Diploma or GED.
CDCA certification or higher through the Ohio CDP Board is required.
An associates degree in human services, Chemical Dependency, or another related field highly preferred.
CPR/BLS Certification required.
1-2 years experience working with substance use preferred

SCOPE OF ROLE

* Provide supervision case management services for defendants placed on bond supervision by the court.

* Schedule and conduct drug use detection tests of defendants, following established procedures and protocols.

* Respond to judicial officer's request for information promptly.
* Testify in court as to the basis for factual findings.
* Serve as a resource to the court.

Upon completion of a six-month probationary period and contingent on proficient evaluation scores and performance, the probation department supports a hybrid schedule environment allowing staff to work both onsite and remotely, on a rotation basis, if the position lends itself to remote work.

POSITION SUMMARY:
The Case Manager is responsible for supporting Bellefaire JCB's Jewish Big Brothers Big Sisters Association (JBBBSA). The Case Manager maintains contact with their caseload and provides monthly support. Evenings and weekend availability will be expected to maintain contact with mentorship matches and support program events.
RESPONSIBILITIES INCLUDE:
Provide supportive supervision to mentors, mentees, and guardians, as appropriate, to maintain the integrity of mentoring matches (community based) through the entire match life cycle (i.e., the interview, training, introduction, supervision, assessment and closure).
Collaborate with colleagues and the JBBBSA Program to ensure timeliness of program and match support the inception of new matches.
Support and attend program events including group outings, program fundraisers, volunteer appreciation meetings, etc.
Remain in compliance with JBBBSA program standards, including specified documents, policies, practices, procedures and materials.
Responsible for all administrative tasks including, but not limited to, record keeping, supervision, attending staff meetings, monthly individual meetings with mentors, guardians and mentees, and other meetings and functions of agency, overseeing volunteer committees, attendance at program/ agency events and outings.
Serve as a resource, referral source, and advocate for Bellefaire JCB to appropriate community services.
Contribute to the development and maintenance of the clinical record through the timely completion of assigned documentation in accordance with applicable licensing and accreditation regulations and program standards.
OTHER DUTIES:
Assist with and attend special events and annual meetings as required.
All required trainings, certifications and licensure must be kept current in accordance with applicable licensing and accreditation regulations and standards.
Respect the privacy of mentees and clients and hold in confidence all information obtained during the client's treatment. All client-related documents should be handled in accordance with Agency guidelines on confidential material.
Maintain high standards of ethical and professional conduct and adhere to Agency policies and procedures.

ESSENTIAL RESPONSIBILITIES:
1. Provide behavioral interventions and support to school aged students in their respective educational setting to assist them in developing and maintaining effective and productive behaviors.
2. Assist administrative and educational staff by providing consultation services related to the client in the areas of behavioral modification, crisis resolution, and healthy social/emotional development.
3. Monitor and coordinate the delivery of necessary evaluations (psychiatric, medical, vocational, health, etc.) and assessments in a timely manner to ensure that client's needs are adequately identified.
4. Utilize Trauma Informed care when appropriate with client/students.

6. Make appropriate referrals to services needed in conjunction with client/student, parent/guardian and school personal.
7. Develop Individual Service Plans that assist in achieving success in the educational setting.
8. Assist the individual in achieving ISP objectives.
9. Maintain productive and professional relationships with school personnel and other agency staff.
10. Work effectively within a school setting following their rules and regulations and following the lead of the school administrator when necessary.
11. Document all encounters in Electronic Health Record. Utilize appropriate documentation for billing and encounters. Maintain 50% productivity monitored through appropriate documentation.
12. Attend all trainings required by school, and AxcessPointe and/or any other required training.
13. Complies with agency policies and procedures, COA regulations, federal and state requirements, and educational/certification/registry requirements.
14. Job assignment is contingent upon partnering with a school and can be severed by the school or AxcessPointe at any time. Continued employment is at the sole discretion of the employer.
15. Perform other assigned and related duties specific and customary to the department in which they are assigned.

QUALIFICATIONS:
1. The position requires the skills and knowledge in the areas of behavioral modification, behavioral health issues, de-escalation techniques, community support and the educational process.

The Opportunity: Under the general supervision of a Clinical Supervisor the Community Rehabilitation Specialist (CRS) provides ongoing recovery focused community rehabilitation services to individuals with severe and persistent mental illnesses primarily residing at one of the site based Permanent Supportive Housing projects (PSH). The CRS is a service broker, coordinator, monitor and planner of services, offering practical help, building community support, teaching recovery-based skills and providing active outreach, advocacy and crisis assistance based on the strengths, needs, abilities and preferences of the individuals.
What You'll Do:
* Monitor and coordinate delivery of necessary evaluations (e.g., psychiatric, medical, vocational), surveys and assessments in order to identify each person's needs.
* Coordinate and participate in the development of an Interdisciplinary Service Plan (ISP) that shall include services and assistance necessary to achieve specific objectives, as well as an explanation of the availability, intensity and duration required of each service. Write up the "ISP" and distribute to necessary participants for approval and sign-off.
* Assist individuals to identify and minimize the effects of the psychiatric symptoms, enabling individuals to self-manage their illness.
* Assist individuals to develop/increase interpersonal and community coping skills individually or in groups.
* Review and monitor services and activities and note progress.
* In cooperation with the individual, reassess strengths, needs, abilities and expressed preferences as required by the "ISP" standards specified in Rule 5122-27-04 of the Administrative Code, and/or at the request of the individual.
* Available to the individual to provide, coordinate and/or assist in crisis intervention and stabilization services during regularly scheduled working hours and available for "after hours" emergencies in accordance with established agency on-call procedures.

We are hiring a part-time Case Manager who will be responsible for supporting Bellefaire JCB's Foster Youth Mentoring Program. This employee will be responsible for maintaining contact with their caseload and providing support on a monthly basis. This is a nontraditional part-time (20 hours/ week) position, as working some weekends/ evenings will be expected to maintain contact with mentorship matches.
The Foster Youth Mentoring Program is a grant-funded, community-based program that follows all the same standards and practices as Big Brothers Big Sisters of America. The program serves youth ages 10-21 that are in permanent custody of Cuyahoga County and has the capacity for up to 45 matches at a time.
