Job Title: Intensive Care Manager I (Chattanooga, TN)
Schedule: Monday to Friday, 8:00 AM - 4:30 PM
JOB SUMMARY: Provide continuous treatment team care management services to children and youth with serious emotional disturbance or severe mental illness, their families, and other team members' consumers. Mentor new care managers on basic skills.
RESPONSIBILITIES:
Continual assessment of consumers' needs, and development of individualized service plan and crisis plan with measurable goals to ensure needs are met.
Assist with referral and linkage to community resources and advocate for consumers whenever necessary.
Participate in rotating on-call schedule, which includes after hours and weekends.
Ensure consumers are seen upon discharge from the hospital and Diversion Services within Agency standards.

Develop and maintain relationships within family unit and supports.
Ensure timely and appropriate documentation as required by the Agency.
Develop and maintain community relations through prompt follow through with contacts.
Continual involvement in the family system, school system and other systems in which the family may be involved (juvenile justice, other supportive agencies, etc.).
REQUIREMENTS:
Bachelor's Degree in a health-related field of counseling, psychology, social work or other behavioral health field.
Candidates with a bachelor's degree in criminal justice must have fifteen college level semester hours of coursework in behavioral health.

Primary Duties and Responsibilities
Facilitates delivery of cognitive behavioral curriculum in individual and group settings focusing on the areas of motivation for change, social adjustment, personal management, social skills development, cognitive restructuring, and development of problem-solving skills.
Conducts periodic reviews of curriculum and resources available to enhance participants' participation in life skills programming and activities.
Maintains class rosters, attendance records, and provides completion certificates for evidence-based programs and other life skills curriculum as required

The Family Centered Treatment Specialist provides quality psychotherapeutic services to children, adolescents, and families utilizing the Family Centered Treatment model.
Essential Duties & Responsibilities:
Provide Family Centered Treatment (FCT) Services as per the service definition and model fidelity requirements.
FCT Certification is required and must be completed within one year of employment in order to continue to provide FCT services and maintain employment with Health Connect America (HCA).
Complete FCT Checkoffs and other internal trainings within designated deadlines. Trainings are assigned and monitored through an e-learning system.
Deliver FCT services to children, adolescents and their families, primarily in their living environments, with a family focus.
Provide 24/7 on-call coverage to a caseload of families and children. Service is regularly provided in the home and community, and will require service provision at the convenience of the child and family requiring a flexible work schedule.
Complete all required Clinical Records and FCT documents with the child and family and ensure the documents are entered/uploaded timely in the Electronic Health Record, according to company standards.
Develop Service Plan in coordination with Treatment Team Process and maintain the plan per designated authorization processes in the clinician's area of service. Work collaboratively on behalf of the child and family with the identified Treatment Team members to ensure effective communication and coordinated service provision.
Conduct ongoing revisions of Service Plan and is responsible for implementation of Service Plan.
Provide psychoeducation as indicated in the Service Plan.
Conduct interventions with the family aligned with the FCT model to include facilitating the movement of the child and family through the four phases of treatment and completing all fidelity documents.
Assist the families and children to establish weekly goals and help them to identify the steps that need to be taken to reach the established goals.
Make referrals to services and resources on behalf of the families and children.

1. Provide career development and educational and occupational information
2. Provide primary prevention instruction in areas such as sexual abuse and substance abuse at
appropriate grade levels
3. Provide enhancement instruction in areas such as self-concept, interpersonal relations, and decision
making appropriate to student development
4. Use teacher and student participation in the implementation of the guidance and counseling
program
C. Evaluation and Assessment
1. Provide academic development of students through the interpretation and use of assessments
2. Provide self-concepts development of students through evaluation and assessment
II. Services to School Staff
A. Planning and Program Management
1. Organize, coordinate, and evaluate learning environments most suited to the developmental needs
of students
2. Plan and establish jointly with educational staff a comprehensive guidance program that responds
to developmental needs of students
3. Provide up-to-date, accurate student records as relates to the guidance program
B. Consultation and Communication
1. Aid school staff in establishing learning environments most suited to the developmental needs of
students
2. Aid school staff in the early identification of student problems
3. Aid school staff in implementing a career development program

TERMS OF EMPLOYMENT
The employee is expected to complete a minimum of the two hundred (200) day work year of certified employees;
additional days are required at the secondary level.
Salary will be established by the years of experience and academic background.
Qualifies for the PROFESSIONAL EXEMPTION from the requirements of the Fair Labor Standards Act
(FLSA) in regard to overtime. The employee is not entitled to the overtime rate of pay (time and a half) when the
employee works over forty (40) hours in the defined work week (from Sunday 12:00 a.m. and continues through the
following Saturday at 11:59 p.m.).
