The Behavioral Health Specialist organizes and implements behavioral health services to Senior CommUnity Care participants and families. Provides behavioral health assessments, individual and group counseling in formats appropriate to clinical populations served. Conducts assessments of clients' mental health and cognitive functioning and develops appropriate treatment plan. Collaborates as part of an interdisciplinary team and promotes professional working relationships with both internal and external customers. Represents Senior CommUnity Care PACE in community settings via formal presentations and community committees as appropriate.
QUALIFICATIONS:
Education: Master's Degree from an accredited school of social work or counseling required. Be legally authorized, currently licensed, registered or certified if applicable in the state of employment. Licensed Professional Counselor or Licensed Clinical Social Worker required.
Experience: A minimum of one year's experience working with frail or elderly population required. Experience working on a multi-disciplinary team in a hospital, nursing home or community-based setting is preferable. Experience and training in bio-psychosocial assessments, dialectical behavioral therapy, cognitive behavioral therapy, solution-focused brief therapy, grief counseling, treatment of substance uses disorders and non-violent crisis intervention a plus. Experience with severe and persistent mental illnesses and personality disorders required.
Skills and Knowledge:
Experience with frail/chronically ill elderly people.
Ability to provide psychosocial assessment and individual, family and group counseling.
Thorough knowledge and understanding of the DSM5/ICD-10 required.
Effective verbal/written communication skills with the ability to maintain accurate records and to prepare clear and concise reports, correspondence and other written materials.
Ability to use and navigate on a computer, using MS Word and other applications. Ability to utilize computers and other electronic devices for tasks such as timekeeping, in-servicing and documentation.
Training and/or mentoring experience and ability to complete performance objectives, measures and evaluations.
Good public speaking skills with all size groups.
Ability to communicate clearly and effectively verbally and written.

ESSENTIAL FUNCTIONS:
Upon referral from Interdisciplinary Team (IDT) or Social Worker, performs in person initial assessments of Senior CommUnity Care participants to obtain a complete psychosocial history, which may include descriptions of cognitive status, social supports, family dynamics mental health and substance dependency and other issues and needs. Coordinates with the IDT to develop a comprehensive plan of care for each participant receiving behavioral health services. Reassesses participants as needed to assess progress in therapy. Keeps IDT apprised of changes and progress in treatment.
Functions as a consultant to the IDT. Attends IDT when necessary to present updates related to significant changes with participant status, education related to care planning for participants receiving behavioral health services, or in absence of the social worker.
Ensures that Social Workers are kept informed of behavioral health services offered to participants and coordinates care in conjunction with the Social Work Team.
Provides ongoing support, counsel, and education to participants and family regarding a variety of issues, including but not limited to: mental health disorders, coping strategies, grief and loss, the aging process, and treatment for behavioral health.
Presents recommendations to IDT for recommended services to support participants with mental health needs and psychiatry referrals. Supports Social Workers with coordination of care with psychiatry and external counseling services.
Supports participants with access to community support regarding mental health services, self-help and other resources related to behavioral health.
Follows all mandated reported guidelines regarding reporting of suspected abuse or neglect of an adult and coordinates with social worker as needed.
Acts as a resource to other team members, including nursing, transportation, day center and home care staff, and therapy staff regarding topics such as severe and persistent mental health diagnoses, dementia, difficult behaviors, and difficult personalities.
Completes and ensures completion of documentation of clinical service, in participant's medical records including initial assessments, re-assessments, counseling sessions, home visits and other significant events according to Senior CommUnity Care documentation requirements.
Assists participants and caregivers in filing grievances.
Advises on day center or other programming offered via Senior CommUnity Care to meet the behavioral health needs of participants and caregivers.
Acts within scope of his or her authority to practice.
Follow all Senior CommUnity Care policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants, and families.
Maintains safe working environment. Follows Senior CommUnity Care Safety policies and procedures.
As appropriate, participates in and supports Quality Improvement Initiatives.
Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
Performs other duties as required.

The Housing Case Manager provides information, support, referrals and assistance to RTD passengers and/or unhoused individuals on RTD property who are experiencing mental health, substance use, or related challenges. This position acts as a knowledgeable liaison to community agencies and is responsible for promoting awareness of the RTD outreach program. The position is housed in RTD offices in Denver and performs encounters on and around RTD stations in the Denver-metro area.
Housing Case Managers empower customers to access identified resources and assist with navigating systems of care to meet identified outcomes. Housing Case Managers provide a wide array of clinical services from a generalist perspective and are knowledgeable of community resources (including housing) and benefits. Housing Case Managers work with all program customers to assess, link and follow up on identified resources and benefits at Jefferson Center and in the community.
Essential Duties/Responsibilities:
Provides outreach and community-based case management services to persons experiencing homelessness, housing instability, or other resource/benefits needs.
Conducts in-person visits at encampments on RTD property, and works with clients to identify needs. Actively coordinates care with Jefferson Center, other community agencies, and health providers. Maintains appropriate professional standards and provides appropriate follow-up for consumers.
Maintains spreadsheets or other tracking processes identified by RTD to capture demographic and other needed information regarding clients being served.
Manages confidential matters and serves as a liaison between those who are unhoused and service providers offering shelter, housing, food, medical, identification, employment, and other resources.
Responsible for the coordination and/or completion of needed Jefferson Center paperwork including releases of information, CCARs, and other needed documentation.

This position is responsible for designing, developing, and implementing a self-sufficiency program at AHA for families receiving rental subsidies. Once established, the Self-Sufficiency Case Manager will carry a caseload of 20-25 families, working with them to identify goals that lead income progression and helping families to break the cycle of poverty. The person in this position will resource these families and connect them to various community agencies and opportunities that increase the likelihood of successful outcomes leading to greater self-sufficiency.

Research and identify best practices for the development and implementation of a self-sufficiency program.

The FSOP Case Manager will provide case management services to children, adolescents, and their families. Case manager will assist client(s) in identifying their needs and use a variety of modalities (referring, monitoring, service planning etc.) to assist clients in achieving these goals. Case manager will provide ongoing assessments to determine if clients and families are at the appropriate level of care based on medical necessity and utilize psychosocial rehab services in the community to teach skills and help youth and families generate coping skills and independence.
Education, Knowledge, Skills & Experience Required:
* Bachelor's degree in Psychology, or related field required.
* Two-year's experience working with youth and families in the mental health or related field in a case management or care coordination role.
* Ability to engage youth and families quickly and creatively.
* Ability to work in community or home settings with youth and family members.
* Good communication and problem-solving skills.
* Experience working with underserved or marginalized communities.
* Bilingual (English/Spanish) Preferred.
Essential Duties:
* Engage client and family in the treatment process and goal setting. Identifies risk factors including lethality.
* Work constructively with client and family to reach agreed upon outcomes, and coordinates care with internal and external providers.
* Provide case management services including attending psychiatric appointments as needed, consulting with psychiatrists on clinical issues, attending school staffing and staffing at hospitals and other community agencies to monitor and plan treatment as well as advocate for clients.
* Facilitate process/support groups and/or Welcome Workshops
* Act as NOMs point person for clients on FSOP and complete outreaches and assessments as outlined by grant requirements
* Collaborate with ongoing FSOP staff to support clients in between sessions and provide outreach to re-engage clients.
* Maintain appropriate professional standards and provide appropriate follow-up for clients and families.
* Help engage and support families in the treatment process.
* Implement motivational interviewing techniques to engage clients in treatment.
* Exhibit enthusiasm, courtesy, adaptability, flexibility, and spirit of cooperation in the work environment.
* Maintain effective interpersonal relations with consumers, peers, subordinates, upper management, visitors, and the general public.
* Effectively respond to client/consumer needs and problems, initiates, and maintains positive interactions, timely response to phone calls, pages, email and other requests.
* Effectively use collaborative documentation with clients and families
* Work cooperatively with other community agencies, as appropriate, and in agreement with supervisor. Volunteers to work on Center committees and/or task forces.
* Help link families to resources both internally and externally.
* Coordinate with internal and external providers to assist family in reaching their goals

Job Description
This position is responsible for designing, developing, and implementing a self-sufficiency program at AHA for families receiving rental subsidies. Once established, the Self-Sufficiency Case Manager will carry a caseload of 20-25 families, working with them to identify goals that lead income progression and helping families to break the cycle of poverty. The person in this position will resource these families and connect them to various community agencies and opportunities that increase the likelihood of successful outcomes leading to greater self-sufficiency.
ESSENTIAL DUTIES:
Research and identify best practices for the development and implementation of a self-sufficiency program.
Develop policies and procedures for the self-sufficiency program from initial application to exit planning (e.g., eligibility criteria, determining participation expectations, selection process, intake and orientation, case management, savings incentive program, etc).
Maintain a caseload of 20-25 once fully implemented.
Develop a case management plan with participants including objectives, goals, and outcomes.
For families who are eligible, connect residents to appropriate mainstream benefits offered through governmental programs.
Identify realistic outcomes for the self-sufficiency program and develop the tools to track those outcomes.
Provide monthly, quarterly and annual statistical reports that also measure individual household as well as program outcomes.
Identify and develop partnerships with community resources (e.g., healthcare, mental health, etc.) and supportive services (e.g., child care, transportation, education, job training, employment counseling, financial literacy, etc) that will assist residents to achieve their goals.
Collaborate with and communicate effectively with residents, AHA staff members and community colleagues.
Qualifications
Master's degree in a related field preferred (e.g., Social Work, Education, Psychology, Human Services, etc.). Equivalent experince could be considered.
Experience in organizing and coordinating variety of events for diverse groups.
Knowledge and experience in case management.
Strong interpersonal skills.
Strong organizing and multitasking skills.
Excellent communication skills (verbal and written) and excellent customer services skills.
Experience with Microsoft Excel, Word, Access, and Teams.
