The Dual Clinical Case Manager II position provides psychosocial/rehabilitative services to both adults as well as children/adolescents with severe and persistent mental illness as well as serious emotional disturbance. ResponsibilitiesSpecific Responsibilities of the Job: Complete the HIV Case Management training series for case management annually. The services are designed to build independent living and tenancy skills and connect people with community‐based health care, treatment and employment services. The Stewpot's PSH program is designed to house 50 chronically homeless individuals and families throughout Dallas County. This is a grant funded position that serves to make homelessness rare, brief, and non‐reoccurring. Essential Functions Maintain a caseload of 25 households in The Stewpot's PSH Program. Assist participants in locating potential properties and communicate with landlords. About the Program and Case Manager Position: Our Home Study and Post Release Services (HS/PRS) program is dedicated to making a significant impact on the lives of unaccompanied children (UC) aged 0- 17 ½.
