Intervention Specialist Key Responsibilities:
Lesson Plan Development: Create effective lesson plans that include accommodations and consultation with general education teacher as needed. Write updates for each student's annual IEP in coordination with the state standards, school district expectations, and enter all IEP updates (Present Levels, BIP, ITP, and Goals) into the dedicated IEP documentation system prior to the meetings. Maintain documentation on IEP goals, student academic and social growth, prepare progress reports. Ensure documentation is accessible and provided upon request in a timely manner. Collaboration: Work collaboratively with other staff and outside agencies to effectively educate each student.
Assistive Technology: Support implementation and use of assistive technology including, but not limited to, augmentative communication devices.
Instructional Planning: Plan instruction and design learning experiences based on data. Conduct summative assessments with each student in accordance with IEP goal reporting timelines. Administer, transfer, and interpret educational assessment data for supporting educational decision making and goal development.
Professional Development: Participate in ongoing professional development activities as directed by the district including teacher and all staff trainings.

What You'll Do:
Monitor and coordinate delivery of necessary evaluations (e.g., psychiatric, medical, vocational), surveys and assessments in order to identify each person's needs.
Coordinate and participate in the development of an Interdisciplinary Service Plan (ISP) that shall include services and assistance necessary to achieve specific objectives, as well as an explanation of the availability, intensity and duration required of each service. Write up the “ISP” and distribute to necessary participants for approval and sign-off.

Behavioral Health Inpatient Unit
The Behavioral Health Safety Specialist is responsible for promoting and maintaining a safe environment for patients, families, and staff. They will conduct proactive and continuous safety rounding in patient care areas. They will also assist as requested by clinical staff in interventions with patients demonstrating escalated, threatening, or aggressive behaviors utilizing evidenced based de-escalation and aggression management techniques.
Responsibilities:
* Serve as a safety expert in patient care areas. This includes a strong knowledge of and adherence to established safety policies, procedures, and standards of care.
Perform continuous safety rounds in patient care areas and proactively identify and communicate with clinical staff any patient/family dynamics that could result in an unsafe environment.
Develop supportive interpersonal relationships with individuals and groups of patients in accordance with treatment plan goals.
Respond to staff requests for assistance with patients demonstrating escalated, threatening, or aggressive behaviors.
Demonstrate expertise in the application of de-escalation techniques and interventions.
Assist in physical restraint techniques necessary in an attempt to control violent, combative patients and maintain a safe environment with respect to age-specific competencies and guidelines in conformance with seclusion and restraint policies and positive approaches training.
Initiate debriefing sessions and provide feedback to staff after seclusion and restraint situations and any crisis.
Partner with Public Safety and Safety Technicians in monitoring and maintaining a safe treatment environment for patients, families, and staff. Intervene during escalating situations applying skills and expertise to reduce the likelihood of physical altercations.
Assess for changes in patient behaviors and provide assistance with early intervention.
Document patient/family safety encounters in medical record/progress note.

Skilled in communicating to diverse audiences during escalating and crisis situations.
Experience in applying evidence-based de-escalation and aggression management techniques.
Experience in project management methodology, including managing large and/or multiple projects and tools is required.
Basic knowledge of child and adolescent development and psychopathology.
Experience working with electronic medical record software, such as EPIC, is preferred.
Education and Experience
2 years of relevant experience working with child and adolescents in behavioral health settings required.
Training in de-escalation and aggression management in healthcare settings required.
Education: Bachelor' s degree is required; bachelor's degree in human services field is preferred.

Under the general supervision of a Clinical Supervisor, this individual provides ongoing recovery focused community rehabilitation services to individuals eligible for the Community Transition Program (CTP), Returning Home Ohio (RHO) and Blue Herron Program. The Scattered Site PSH CaseManager is a service broker, coordinator, monitor and planner of services, offering practical help, building community support, teaching recovery-based skills and providing active outreach, advocacy and crisis assistance based on the strengths, needs, abilities and preferences of the individuals.
What You'll Do:
Responsible for monitoring and coordinating the delivery of necessary evaluations (e.g., psychiatric, medical, vocational, substance abuse) surveys and assessments in order to identify each person's needs.
Coordinate and participate in the development of an Interdisciplinary Service Plan (ISP) that shall include services and assistance necessary to achieve specific objectives, as well as an explanation of the availability, intensity and duration required of each service. Write up the “ISP” and distribute to necessary participants for approval and sign-off.
Coordinate services and treatments including acquisition of services, crisis management and stabilization, and client advocacy for the purpose of promoting clinically sound interventions. Serves as liaison between the Department of Corrections, Corporation for Supportive Housing, landlords, psychiatrists and other mental health and substance abuse rehabilitation professionals.
Collaborate with local housing providers to develop additional housing resources in the community. Advocates with local landlords and serves as a customer service representative for landlords to the CTP, RHO and Blue Herron Program.
Recovery principles are in compliance with federal and state standards of practice and ethical codes of conduct.
Provide clinical services to clients as needed.
Assist the individual in increasing social support networks of relatives, friends, and/or volunteers that ameliorate life stresses resulting from the individual's disability. Conducts treatment planning with clients living in scattered site PSH.
Accompany individuals to appointments and activities and assist in daily living skills training as needed.
Attend trainings as required by various funding sources.
Maintain relevant documentation and provide statistical reports in accordance with agency documentation.
Attend regularly scheduled staff and supervisory meetings.
Maintain confidentiality of clinical and sensitive administrative information.
Work assigned schedule and manage schedule in a professional and responsible manner.
Assume other duties and responsibilities as assigned by supervisor, which may include serving on various agency committees.

JOB SUMMARY:
We are hiring a part-time CaseManager who will be responsible for supporting Bellefaire JCB's Foster Youth Mentoring Program. This employee will be responsible for maintaining contact with their caseload and providing support on a monthly basis. This is a nontraditional part-time (20 hours/ week) position, as working some weekends/ evenings will be expected to maintain contact with mentorship matches.
The Foster Youth Mentoring Program is a grant-funded, community-based program that follows all the same standards and practices as Big Brothers Big Sisters of America. The program serves youth ages 10-21 that are in permanent custody of Cuyahoga County and has the capacity for up to 45 matches at a time.
ESSENTIAL DUTIES:
Providing supportive supervision to mentors, mentees, guardians and contract agency, as appropriate, to help maintain the integrity of mentoring matches through the entire match life cycle (interview, training, introduction, supervision, assessment and closure).
Remain in compliance with mentoring program standards, including specified documents, policies, practices, procedures and materials.
Responsible for all administrative aspects which enable the job to be carried out including, but not limited to, record keeping, supervision, attending staff meetings, monthly group and individual meetings with mentors, and other meetings and functions of agency, staffing of volunteer committee, attendance at agency events and outings.
Serving as a resource person and advocate to Bellefaire JCB and other appropriate community services.
